    Improving the Use of Perclose ProGlide Vascular Suture System Through Simulator Training: A Retrospective Queue Study

    Objectives This study aims to assess whether training with simulators can improve the learning efficiency of the Perclose ProGlide vascular closure system, thereby accelerating proficiency and mastery of the closure system. Methods This study adopts a retrospective cohort design, selecting 11 physicians trained at this center from September 2020 to September 2022 as the study subjects. They are divided into a research group and a control group. Different training methods are employed for their use of the Perclose ProGlide vascular closure system. The control group learns only through manuals and instructional videos, while the research group adds simulator training. After the training, both groups of physicians use the Perclose ProGlide system to close the puncture site of the femoral artery in 100 cases, and the outcomes are compared. Results There is no statistically significant difference in the success rate of using the Perclose ProGlide vascular closure system between the two groups(P>0.05). The research group shows a significantly shorter device release time(P<0.05)and a reduced bleeding volume during closure(P<0.05). The comparison of research results between the other two groups showed no statistically significant difference (P>0.05). Conclusion The use of simulators contributes to accelerating vascular surgeons' mastery of the Perclose ProGlide vascular closure system, enabling physicians to operate more proficiently in actual procedures and reducing patient bleeding.

    Objective To clarify the structural characteristics and changes of intestinal microbiota in neonatal hemolytic jaundice. Methods Newborns born in Suqian Hospital Affiliated to Xuzhou Medical University from October 2022 to March 2023 and diagnosed with neonatal hemolytic jaundice were selected as the case group. Healthy newborns born at the same time in the hospital served as the control group. We extracted the fecal genomic DNA of all newborns and conducted quality testing. After passing the test, 16S rDNA amplicon sequencing was performed. Subsequently, diversity, abundance, and structural analysis were conducted on the biological information obtained through sequencing. Results α Diversity analysis showed an increase in microbial diversity in the case group, but there was no significant change in abundance. β Diversity analysis showed differences in gut microbiota similarity between the two groups. LEfSe analysis showed changes in the gut microbiota structure of the case group, mainly manifested as an increase in the abundance of Escherichia, Klebsiella, Bacteroides, and Rothia, while a decrease in the abundance of Streptococcus. Conclusion Neonates with hemolytic jaundice have intestinal microbiota disorder, mainly characterized by an increase in the abundance of conditional pathogenic bacteria. The results may provide some theoretical basis for targeted microecological intervention in neonatal hemolytic jaundice.

    Noninfectious Chronic diseases(NCD) are characterized by high prevalence rate with complicated etiology and difficult treatment, which threaten the health of Chinese community residents and cause the waste of medical resources. In China, general practitioners(GPs) are responsible for the responsibility of diagnosis and management of NCD in primary health care, and play an important role in providing and coordinating medical services for NCD patients. Based on SWOT analysis, we discusses the strengths, weaknesses, opportunities and threats of GPs in the diagnosis and mangement of NCD under the current conditions and social environment, and explores specific and efficient strategies to improve the ability of GPs and promote the quality of primary health care.

    Objective To compare the effects of different anesthesia methods on cognitive function in elderly patients undergoing urologic surgery by using combined epidural anesthesia and tracheal intubation general anesthesia. Methods Sixty-five elderly urological patients who underwent titanium laser lithotripsis for ureteral calculi admitted to our hospital from January 2022 to December 2022 were selected as the study objects, and were divided into general anesthesia group (32 cases) and combined anesthesia group (33 cases) according to randomization rule, and received tracheal intubation general anesthesia and combined epidural anesthesia, respectively. Anesthesia-related indicators, VAS pain score, brief Mental State Examination Scale (MMSE), and incidence of cognitive dysfunction were evaluated between the two groups. Results The rate of excellent and good anesthesia in combination group was 87.88% higher than that in general anesthesia group (59.38%) (P<0.05). Compared with the preoperative results, the VAS score of the general anesthesia group and the combined group was lower (P<0.05), and the VAS score of the combined group was lower than that of the general anesthesia group (P<0.05). Compared with the preoperative results, the MMSE scores in both the general anesthesia group and the combined group were lower after operation (P<0.05), and the MMSE scores in the combined group were higher than those in the general anesthesia group (P<0.05). The incidence of cognitive dysfunction 6 h and 24 h in the combined group was lower than that in the general anesthesia group (P<0.05). Conclusion Compared with tracheal intubation general anesthesia, lumbo-epidural anesthesia has better anesthetic effect, less influence on cognitive function, lower rate of cognitive dysfunction, and can promote postoperative recovery of patients.

    Objective To study the efficacy and safety of lifestyle control combined with Nattokinase Red yeast rice(NK-RYR) capsules in reducing blood lipids. Methods Hyperlipidemia volunteers were recruited according to inclusion and exclusion criteria, and randomly divided into NK-RYR capsules group (lifestyle control + NK-RYR capsules) with 20 cases and lifestyle control group (lifestyle control alone) with 10 cases according to the ratio of 2:1. Lifestyle control was carried out in both groups during the experiment. The NK-RYR capsules group received Chongjian NK-RYR capsules (1.35g bid) at the same time, for a total of 90 days. All subjects were interviewed on 0 d, 30 d, 60 d and 90 d after enrollment, and the four items of blood lipids, liver and kidney function and adverse events at each site were recorded on 0d and 90d. Four items of blood lipids, changes of liver and kidney function and adverse events before and after the two groups were compared. Results Low density lipoprotein cholesterol (LDL-C) was significantly decreased in the NK-RYR capsules group before and after the test (P<0.05). total cholesterol (TC) decreased significantly (P<0.05); There was no significant difference in LDL-C and TC levels in lifestyle control group (P>0.05). There was no significant difference in High density lipoprotein cholesterol (HDL-C) between the two groups before and after each test (P>0.05). There was no significant difference in triglyceride (TG), alanine aminotransferase (ALT), aspartate aminotransferase (AST), and serum creatinine before and after the trial between the two groups (P>0.05), and no adverse events occurred during the trial between the two groups. Conclusion Compared with lifestyle control alone, the combination of lifestyle control and NK-RYR capsules can reduce LDL-C and TC levels in hyperlipidemia patients.

    Objective To explore the efficacy of Qingfei Fuzheng Quyu method combined with ambroxol hydrochloride injection in the treatment of patients with bronchopneumonia and its influence on erythrocyte sedimentation rate and C-reactive protein (CRP) levels. Methods 80 bronchopneumonia patients admitted to Yantai Penglai People's Hospital from June 2018 to June 2022 were selected, and all patients were divided into observation group and control group by random number table method, with 40 cases in each group. The control group was treated with ambroxol hydrochloride injection, and the patients in the observation group were treated with clear lung fuzheng and removing stasis method combined with ambroxol hydrochloride injection. The clinical efficacy, white blood cell count (WBC), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR) expression before and after treatment were compared, and the disappearance time of lung rale/wheezing and cough were compared between the two groups Total days of fever, hospital stay and adverse reactions. Results The total effective rate of the observation group was significantly higher than that of the control group (P<0.05); There was no significant difference in the levels of WBC, ESR and CRP between the two groups before treatment (P>0.05). After treatment, the levels of WBC, ESR and CRP in the two groups decreased, and the observation group was lower than the control group (P<0.05); The disappearance time of lung rale/wheezing, cough, total days of fever and hospital stay in the observation group were significantly lower than those in the control group (P<0.05); There were no significant adverse reactions in the two groups, and there was no significant difference in the adverse reactions (P>0.05). Conclusion The method of Qingfei Fuzheng Quyu and ambroxol hydrochloride injection has significant effect in the treatment of bronchopneumonia pneumonia, which can further reduce the inflammatory reaction of the patient, reduce the clinical symptoms as soon as possible, and promote the early recovery of the patient, with high safety and no obvious adverse reactions.

    Objective To study the therapeutic effect of Qushi Weiling decoction combined with mometasone furoate cream on patients with eczema. Methods Selected 82 cases of patients with eczema who were treated in our hospital from May 2020 to May 2022, divided into two groups randomly. The control group was treated with mometasone furoate cream, while the observation group was treated with dehumidifying Weiling decoction. Evaluate the symptom scores and quality of life of both groups, and detect serum interleukin-2 (IL-2), aromatic hydrocarbon receptor (AhR) γ Interferon (IFN-γ) And interleukin-4 (IL-4) levels. Results After treatment, the skin lesion morphology, itching degree, skin exudation degree, and skin lesion area score of the two groups were significantly reduced (P<0.05), and the observation group had lower skin lesion morphology, itching degree, skin exudation degree, and skin lesion area score (P<0.05). After treatment, the serum levels of AhR and IL-4 in both groups were significantly reduced (P<0.05), IL-2, IFN-γThe level significantly increased (P<0.05), and the serum AhR and IL-4 levels in the observation group were significantly lower(P<0.05), IL-2,IFN-γ level was significantly higher (P<0.05). After treatment, the scores of economic burden, clothing, exercise, psychology, work learning, physiology, sexual life, daily life, social entertainment, and family relations in both groups were significantly reduced (P<0.05), and the scores of economic burden, clothing, exercise, psychology, work learning, physiology, sexual life, daily life, social entertainment, and family relations in the observation group were lower (P<0.05).     Objective To study the effect of combined treatment with Huazhuoxingxue Decoction on lipid metabolism and serum high-sensitivity C-reactive protein (hs-CRP) in patients with primary hyperlipidemia. Methods A total of 80 patients with primary hyperlipidemia were selected from June 2021 to May 2022 in Weifang Hospital of Traditional Chinese Medicine. Using the random number table method, they were divided into a control group (40 cases) and an observation group (40 cases). Patients in the control group received routine treatment, while those in the observation group received routine treatment combined with Huazhuoxingxue Decoction. The levels of total cholesterol(TC), triglyceride(TG), high density lipid-cholesterol(HDL-C), low density lipoprotein cholesterol(LDL-C), and hs-CRP before and after treatment were compared between the two groups. The TCM syndrome scores of obesity, drowsiness, and physical laziness before and after treatment were evaluated and compared between the two groups. Results Before treatment, there was no difference in the levels of blood lipid level between the two groups ( P>0.05). Following treatment, the levels of TC, TG, and LDL-C in the observation group were lower than those in the control group, while the level of HDL-C was significantly higher than that in the control group(P<0.05). Prior to treatment, there was no difference in the levels of hs-CRP between the two groups(P>0.05). Following treatment, the level of hs-CRP in the observation group was significantly lower than that in the control group(P<0.05). Before treatment, there was no difference in the TCM syndrome scores of obesity, drowsiness, and physical laziness between the two groups(P> 0.05). After treatment, the TCM syndrome scores of obesity, drowsiness, and the levels of physical laziness in the observation group were significantly lower than those in the control group(P<0.05). Conclusion Routine treatment combined with Huazhuoxingxue Decoction can effectively improve lipid metabolism disorder and reduce systemic inflammation response, thereby relieving TCM syndrome and is worth promoting for patients with primary hyperlipidemia.

    Objective This study was designed to investigate the multiple intelligence and sense of alienation among the pediatric liver transplantation patients. Methods The questionnaire was made based on Gardner's theory of multiple intelligence and Youth Student Alienation Scale. 34 cases was investigated by convenience sampling. Results There were 19 males (55.9%) and 15 females (44.1%), respectively. The average age was (13.8±2.0)y, and the average height and weight was (160.5±11.6)cm and (49.1±15.6)kg, respectively. The average score of multiple intelligence was (56.8±12.4), while the one of sense of alienation was (136.7±44.6). Several domains of multiple intelligence were median negative correlated with several domains of sense of alienation, and P value was less than 0.05. Conclusion The multiple intelligence of pediatric liver transplantation patients locates at a moderate to upper level, and sense of alienation locates at a moderately lower level.
    Comparison of Therapeutic Effects of Aspirin on High-risk Pregnant Women with Preeclampsia at Different Gestational Weeks

    Objective To compare and analyze the efficacy of aspirin in different gestational weeks on high-risk pregnant women with preeclampsia, and to provide reference for pregnancy safety. Methods Eighty pregnant women with high risk of preeclampsia who received treatment in the department of obstetrics,Jiangyin Hospital Affiliated to Nantong University from September 2019 to September 2022 were selected as the main body and randomly divided into group A (28 cases), Group B (28 cases) and blank control group (24 cases, without taking aspirin). Patients in group A received 100 mg aspirin since the 12th week of gestation, and patients in group B received 100 mg aspirin since the 16th week of gestation. The drug was stopped at the 28th week of gestation in both groups, and the blood pressure control was 130-155/80-105mmHg. If the complication was organ dysfunction, the control was 130-139/80-89mmHg. Preeclampsia occurrence, pregnancy outcome, mode of delivery, neonatal related outcome and urine misfolded protein positive rate were compared among the three groups. Results There was no significant difference in the incidence of preeclampsia and adverse pregnancy outcomes between Group A and Group B (P>0.05); The incidence of preeclampsia and adverse pregnancy outcomes in Group A and Group B was lower than that in the control group(P<0.05). There was no significant difference in cesarean section rate and spontaneous labor rate between Group A and Group B (P>0.05); The cesarean section rate in Group A and Group B was lower than that in the control group, and the spontaneous delivery rate was higher than that in the control group (P<0.05). There was no significant difference in Apgar score and incidence of adverse neonatal outcomes between Group A and Group B (P>0.05); The Apgar of newborns in Group A and Group B was higher than that in the control group, and the incidence of adverse neonatal outcomes was lower than that in the control group (P<0.05). Urine misfolded protein positive rates in group A at 28 weeks and 32 weeks of pregnancy were significantly lower than those in control group, and the positive rate in group A was lower than that in group B (P<0.05). Conclusion Aspirin has a significant effect in the prevention of preeclampsia. Taking aspirin at 12 and 16 weeks of gestation in high-risk pregnant women can effectively reduce the incidence of preeclampsia, while also improving the rate of spontaneous labor and reducing the incidence of adverse maternal and infant outcomes.
    Correlation Analysis between High-Risk HPV Infection and Vaginal Microecological Environment

    Objective To study the correlation between high-risk HPV infection and vaginal microecological environment. Methods A total of 280 women with gynecological outpatient health checkups, subjects aged 20~60 years, were randomly collected and divided into case and control groups, grouped based on the presence or absence of high-risk HPV infection. The case group: 153 cases were positive for high-risk HPV, and the control group: 127 cases were negative for HPV. The vaginal flora density, diversity, pH value, vaginal microecological function test, vaginal pathogenic microorganisms, and Nugent score were analyzed and compared between the two groups. Results The difference in the density and diversity of vaginal flora between the case group and the control group was not statistically significant (P>0.05); In the case group, the proportion of vaginal pH 4.5, catalase positive rate, sialidase positive rate and leukocyte esterase positive rate were all higher than the control group, and the differences were statistically significant (P<0.05). The BV detection rate was 20.00% (56/280), TV detection rate was 1.43% (4/280), and VVC detection rate was 5.00% (14/280) in all 280 subjects. The BV positivity rate was 33.33% (51/153) in the case group and 3.94% (5/127) in the control group, with a significant difference between the groups (P<0.05). There were no significant differences in the rates of trichomonas vaginalis and Pseudomonas vulvae positivity in the case group compared with the control group (P>0.05). The risk of high-risk HPV infection increased 4.986-6.791-fold with increasing Nugent score (χ21=21.489, P1<0.05, OR1=4.986, 95% CI1: 2.424-10.258; χ22=17.573, P2<0.05, OR2=6.791, 95% CI2: 2.503-18.420). Conclusion High risk HPV infection is closely related to vaginal microecology, especially BV is a risk factor for high risk HPV infection.

    Objective To analyze the expression of serum 25-hydroxyvitamin D (25-hydroxyvitamin D, 25-OH-D) and antineutrophilic cytosolic antibody (Anti-neutrophil cytosolic antibodies, ANCA) in patients with systemic lupus erythematosus (systemic lupus erythematosus, SLE). Methods In the study, 62 patients with SLE were admitted to the hospital from April 2022 to April 2023 were the study observers, and 60 healthy volunteers in the same period were included as the control group to detect 25-OH-D and ANCA between the groups, and compare 25-OH-D and ANCA between the active patients in the observation group. Meanwhile, the factors of SLE disease activity were analyzed by Logistic. Results 25-OH-D was lower than the control group, and the difference was statistically significant (P<0.05); active SLE patients 25-OH-D lower than inactive SLE, ANCA higher than inactive SLE patients, and the difference was statistically significant (P<0.05); 25-OH-D expression and ANCA as independent variable, continuous variable; SLE activity degree as dependent variable (mild=0, severe=1), Logistic regression analysis. The results showed that 25-OH-D and ANCA influenced the degree of SLE activity (P<0.05); with SLE disease as the independent variable, 25-OH-D, ANCA and the joint levels, the optimal cut-off value was 0.089 (area under curve, AUC) was 0.702 (0.646~0.758), 0.713 (0.651~0.779), 0.879 (0.782~0.991). Conclusion 25-OH-D has low expression level in SLE patients, and ANCA shows high expression level. 25-OH-D and ANCA are the main influencing factors of SLE disease activity period, with high application value in clinical SLE disease diagnosis and disease activity degree prediction, so clinical use is recommended.

    Objective To explore the application effect of traction combined with rehabilitation exercises in the rehabilitation treatment of patients with lumbar disc herniation. Methods A total of 100 patients with LDH admitted to the Department of Orthopedic Trauma of the Third People's Hospital of Longgang District, from June 2022 to June 2023 were selected for the study. All patients were divided into the control group and the observation group randomly with 50 cases in each group. The control group was treated with traction, and the observation group was treated with rehabilitation exercises on the basis of the control group. The two groups were treated for 12 weeks, and the total effective rates of the two groups were compared with the Japanese Orthopaedic Association (JOA) Lower Back Pain Score, Visual Analog Synthesis (VAS) Score, Self-Assessment Scale for Anxiety (SAS), Self-Assessment Scale for Depression (SDS) Score, and the total effective rates of the two groups before and after the treatment. Results The total effective rate and JOA score of the observation group after treatment were higher than those of the control group, and the difference was statistically significant (P<0.05). The VAS score, SAS and SDS scores of the observation group were lower than those of the control group after treatment and the difference was statistically significant (P<0.05). Conclusion Traction combined with rehabilitation exercises can more effectively relieve lumbar pain symptoms in patients with lumbar disc herniation, reduce the overall pain problem of the patient, and better regulate the patient's anxiety, depression and other negative emotions due to the disease and pain,the treatment is more efficient.

    Objective To conduct a study on the visual health status of primary and secondary school students in Yiyuan County, to clarify the visual health problems of primary and secondary school students, and to preliminarily analyze the risk factors triggering visual health problems to provide data support for youth myopia prevention and control. Methods Between April and August 2021, the "Yiyuan County Comprehensive Myopia Prevention and Control Promotion Plan for Children and Adolescents" was developed based on the "Appropriate Technology Guidelines for Prevention and Control of Myopia in Children and Adolescents" (2019 version) issued by the General Office of the National Health Commission, and vision screening was conducted among school students in primary and secondary schools in Yiyuan County. Results A total of 47,496 people from 4 high schools (10,791), 18 middle schools (16,909), and 51 elementary schools (19,796) in Yiyuan County participated in this vision census, and a total of 43,434 (91.45%) people were included in the final analysis after invalid data were excluded. There were differences in vision health among different grades (χ2=10,597.452, P<0.001), and among adolescents of different genders(χ2=32.710, P<0.001). Conclusion Parents and schools should intervene jointly as early as possible when adolescents develop visual abnormalities to protect their visual health.

    Objective To compare the nursing effects of comprehensive nursing and routine nursing in patients with psoriasis. Methods A total of 100 patients with psoriasis in Jiaozhou City Traditional Chinese Medicine Hospital from January 2022 to March 2023 were included. The control group (50 patients were included, providing routine care) and the observation group (50 patients were included, providing comprehensive care) were divided according to the random number table. Psychological score, social support rating scale (SSRS) score, psoriasis area and severity index (PASI), dermatology quality of life index (DLQI), Pittsburgh Sleep Quality Index (PSQI), self-care ability scale (ESCA) score, satisfaction rate were compared. Results Before nursing, there was no difference in psychological score, SSRS score, PASI, DLQI, PSQI, and ESCA score between the groups (P>0.05); after nursing, the observation group had lower psychological score, PASI, DLQI, PSQI, and higher SSRS and ESCA (P<0.05); Compared to the control group, the total satisfaction rate of the observation group was higher (P<0.05). Conclusion Implementing comprehensive nursing care for psoriasis patients can create a harmonious nurse patient relationship, reduce psychological burden, enhance self-care ability, alleviate the condition, improve quality of life and sleep, and is worth promoting.

    Femoral shaft fracture is a common and frequently occurring disease in clinic. The treatment methods of femoral shaft fracture in children include conservative treatment and surgical treatment. During treatment, it is required to give strong fixation to the broken end of the fracture to ensure good alignment and alignment, so as to achieve fracture healing on schedule and avoid complications. If treatment method and internal fixation are selected incorrectly, there will be delayed fracture healing, nonunion and even residual deformity, which will bring disastrous harm to patients and families. Based on the experience of a child with femoral shaft fracture after six operations, this paper analyzes the whole treatment process, finds out the errors and deficiencies in the treatment process, summarizes the lessons of failure, and rings an alarm for later clinicians in the treatment of children with femoral shaft fracture.
