    Objective With the reform of medical system putting forward higher requirements for internal operation and management of public hospitals, it is necessary to strengthen refined cost management and reduce resource waste. Increasing revenue and reducing expenditure is an important part of internal control. Methods The differences between multidisciplinary diagnosis and treatment model and traditional diagnosis and treatment model were compared, and the principles and ideas of multidisciplinary diagnosis and treatment cost accounting were proposed on the basis of sorting out the management process of multidisciplinary diagnosis and treatment, and the specific accounting methods were put forward according to the cost of multidisciplinary diagnosis and treatment center, project cost and disease cost. Results Multidisciplinary diagnosis and treatment cost accounting not only provided the basis for multidisciplinary diagnosis and treatment project charges, but also provided support for hospital internal performance distribution, and provided ideas for disease cost management. Conclusion The cost management of disease MDT model of public hospitals is a gradual process. Only by focusing on efficiency, taking into account fairness, and combining the public welfare of the hospital and scientific management methods,can the economic benefits be maximized.

    Meta-analysis of Clinical Efficacy of Duzhong Yaotong Pill on Lumbar Disc Herniation

    Objective The efficacy of Duzhong Yaotong Pill in the treatment of lumbar disc herniation was systematically evaluated. Methods According to the requirements of Meta-analysis, CNKI, CBM, WanFang, VIP, PubMed, Web of Science, Cochrane Library and Embase were searched for literatures on randomized controlled trials of Duzhong Yaotong Pill in the treatment of lumbar disc herniation published from the establishment of databases to May 2022. The Cochrane Collaboration bias risk tool was used for risk assessment, and the modified Jadad scale score was used for literature quality assessment. Review Manager 5.3 statistical software was used for Meta-analysis of literatures with modified Jadad scale score ≥ 3, and publication bias was analyzed by Begg’s test. Results Finally, eight eligible literatures were retrieved, of which five were Jadad ≥ 3, with a total of 652 patients ; meta-analysis results showed that compared with other conservative treatment control groups, Duzhong Yaotong pill combined with traditional Chinese and western medicine could significantly improve the effective rate of main clinical symptoms in patients with LDH [RR=1.11,95% CI(0.97,1.26), P>0.05 ] ; in addition, VAS score (MD=-0.65,95% CI(-0.93, -0.37), P<0.05 ) and JOA score (RR=0.87,95% CI(0.60,1.13), P<0.05 ) were significantly better than the control group. Begg’s test showed that there was no significant publication bias between studies [clinical effective rate (Z=1.042, P=0.296) ;VAS score (Z=0.000, P=1.000); JOA score (Z=0.000, P=1.000)]. Conclusion The existing evidence shows that Duzhong Yaotong Pill has certain advantages in the treatment of lumbar disc herniation, which can alleviate the pain of patients, significantly reduce the VAS score of patients and improve the JOA score and worthy of clinical application.

    As a common endocrine disease in gynecology, primary dysmenorrhea seriously affects the quality of life of contemporary women with its high incidence and periodicity. Acupuncture is one of the essential treatment methods of Chinese medicine, and it is widely used in the treatment of modern primary dysmenorrhea. However, how to improve the efficacy of acupuncture in the treatment of primary dysmenorrhea and whether it is necessary to get Deqi is worthy of further discussion. In this article, the author of this article has made a concise description of the effect of acupuncture to Deqi and its effect on the treatment of primary dysmenorrhea by consulting a large number of ancient and modern literatures.

    Objective To observe the clinical efficacy of Xuanbai Chengqi decoction in improving (sepsis-associated acute lung injury, S-ALI). Methods A total of 80 S-ALI patients admitted to Dongzhimen Hospital of Beijing University of Chinese Medicine from January 2020 to December 2021 were selected as research subjects, and were randomly divided into treatment group and control group according to random number table method, with 40 patients in each group. The control group was treated with conventional treatment, and the treatment group was treated with Xuanbai Chengqi supplementing and reducing prescription on the basis of conventional treatment ,both groups were treated for 7 days.. The oxygenation index (PO2/FiO2), respiratory rate (R), 24-hour stool volume of 2 groups were observed before and after treatment. Changes of white blood cell (WBC), C-reactive protein (CRP), alanine aminotransferase (ALT), serum creatinine (SCR), acute physiology and chronic health score (APACHE Ⅱ score), serum iron ion (Fe) and other indicators. Results On the 7th day of treatment,the oxygenation index (PO2/FiO2), 24-hour stool volume and respiratory rate in the treatment group were higher than those in the control group and the differences were statistically significant (P<0.05); WBC, CRP, APACHE Ⅱ score and Fe in the treatment group were lower than those in the control group, the difference was statistically significant (P<0.05). There were no significant differences in ALT and SCR between the treatment group and the control group. Conclusion Xuanbai Chengqi supplementing and reducing prescription can help the patient to improve lung function, gastrointestinal function, and suppress the inflammatory response. At the same time, the prescription has a certain security to liver and kidney.

    Effect of Nursing Intervention Based on Cross Theoretical Model on Improving Lifestyle of Patients with Acute Myocardial Infarction

    Objective To explore the effect of nursing intervention based on cross theoretical model on improving the lifestyle of patients with acute myocardial infarction. Methods 100 patients with acute myocardial infarction treated in our hospital from April 2021 to September 2021 were randomly divided into control group (n = 50) and test group (n = 50) according to the numerical table method. Both groups were given routine treatment. The control group was given routine nursing intervention based on routine treatment, and the test group was given nursing intervention based on cross theoretical model. The scores of disease management ability, Health Promoting Lifestyle Scale (Health Promotion Lifestyle Profile, HPLP) and incidence of adverse cardiac events and readmission rate. Results The scores of emergency management, self-care knowledge and clinical symptoms in the test group were significantly higher than those in the control group (P<0.05); the scores of nutrition management, rehabilitation exercise, health responsibility, interpersonal relationship, self realization and stress management in the test group were significantly higher than those in the control group (P < 0.05); the incidence of adverse cardiac events and readmission rate in the test group were significantly lower than those in the control group (P<0.05). Conclusion Nursing intervention based on cross theoretical model can help to improve the self-efficacy and lifestyle of patients with acute myocardial infarction, improve their disease management ability, so as to reduce adverse cardiac events and reduce the readmission rate, which is worthy of clinical application.

    Objective To investigate the role and transcriptional level of histone deacetyltransferase Sirt1 in liver regeneration after hepatectomy. Methods Three mice were randomly selected and executed at 12 h, 24 h, 48 h and 72 h postoperatively in the experimental group, and the mice in control group were executed immediately after the sham operation.The ability of liver regeneration was assessd by liver weight/body weight ratio, the degree of liver injury was detected by ALT activity detection and the transcriptional level of histone deacetyltransferase Sirt1 in mouse liver tissue was detected by real-time quantitative PCR (RT-qPCR). Results Aftter hepatectomy, the liver weight/body weight ratio in the experimental group increased gradually. compared with the control group, ALT levels in serum were higher at 12 h, 24 h and 48 h, and recovered at 72 h. At 48 h, the transcriptional level of histone deacetyltransferase Sirt1 decreased. Conclusion Liver regeneration after hepatectomy in mice might be related to the decrease of histone deacetyltransferase Sirt1 transcription, which resulted in the increase of acetylation of some proteins.

    Study on the Application Effect of Qi and Collateral Manipulative Nursing on Non-Specific Low Back Pain

    Objective Study on the application effect of qi and collaterals manipulative nursing on non-specific low back pain. Methods A total of 126 patients with non-specific low back pain who were diagnosed and treated in the Department of Traditional Chinese Medicine Rehabilitation of the Fourth Hospital of Baotou City, Inner Mongolia from January 2019 to January 2022 were retrospectively selected as the research subjects. According to the nursing methods, they were divided into a Qi and collateral manipulative group and a traditional group with 63 cases each. The traditional group were given routine functional exercise nursing, and the Qi and collateral manipulative group were given the Qi and collateral manipulative nursing on the basis of the treatment of the traditional group. Both groups were given nursing observation for 2 months. The overall curative effect of the two groups was observed, and the overall pain of myofascial trigger points, the activity of the waist joints, and the surface electromyography were evaluated before and after nursing. Results Before nursing, the visual analogue scale (VAS) of myofascial trigger point pain, the oswestry disability index (ODI) score of lumbar joint activity, the surface electromyography(sEMG) of flexion and dorsi-extension states, the difference was not statistically significant(P>0.05). After nursing, the VAS of myofascial trigger points, lumbar joint activity ODI scores and flexion state sEMG value of the two groups of patients were significantly lower than those before nursing, and the Qi and collaterals manipulation group was also significantly lower than the traditional group, the difference was statistically significant(P<0.05). The total effective rate of the Qi and collateral manipulative group after nursing which were significantly higher than the traditional group, the difference was statistically significant(P<0.05). Conclusion The application of Qi and collateral manipulative nursing in non-specific low back pain can relieve the pain of myofascial trigger points, improve lumbar function and surface EMG level, thereby improving the overall curative effects.

    Association between Cognitive Functions and Grip Strength among the Older Adults: Based on CHARLS Data

    Objective To explore the association between cognitive functions and grip strength among Chinese older adults. Methods This study analyzed the baseline data of China Health and Retirement Longitudinal Study (CHARLS) collected from May 2011 to March 2012, with 7 833 adults aged 55 years and above recruited, and to explore the association between grip strength and general information, cognitive functions as well as other health factors. Descriptive statistics and multiple linear regression analysis were performed using the software Stata 14.1. Results The sample of this study included 49.4% women, and the average age and grip strength was 64.4 years and 31.1kg. Men (β=-11.004, P<0.001), adults with younger age (β=-0.347, P<0.001), with higher education level (β=0.805, P<0.001; β=1.971, P=0.003), and with urban hukou (β=0.720, P=0.001) had greater grip strength. The grip strength of the older adults with two or more chronic diseases (β=-0.514, P=0.012) was significantly lower than that of the older adults without any chronic disease, and the grip strength of the older adults with disability in activities of daily living (β=-2.238, P<0.001; β=-1.331, P<0.001), depressive symptoms (β=-1.161, P<0.001), and no social activities (β=0.399, P=0.026; β=0.684, P=0.008) was smaller. Telephone interview for cognitive status (TICS) (β=0.139, P=0.001), word recall (β=0.334, P<0.001) and drawing (β=0.781, P<0.001) were significantly associated with grip strength. Conclusion The three cognitive functions of TICS, word recall and drawing are positively associated with the older adults' grip strength. The grip strength can be used as an indicator to identify the cognitive functions of the older adults.
