Prognostic Value of Serum sST2 and hs-CRP in the Occurrence, Success and Recurrence of NVAF

Objective To investigate the predictive value of serum soluble growth-stimulated expressed gene 2 protein (sST2) and highly sensitive C-reactive protein (hs-CRP) in the occurrence, success and recurrence of nonvalvular atrial fibrillation (NVAF). Methods 1 000 patients with coronary heart disease, hypertension and heart failure admitted to the Second Affiliated Hospital of Qiqihar Medical College from 2021 to 2024 were retrospectively selected and grouped according to whether they were combined with nonvalvular atrial fibrillation. Patients receiving cardioversion therapy were further grouped and analyzed to collect sST2, hs-CRP and related clinical indicators. Results sST2, hs-CRP and left anteroposterior atrial diameter(LAD) in patients with coronary heart disease and AF were higher than those without AF, the difference was statistically significant (P<0.05). sST2, hs-CRP and LAD in hypertensive patients with nonvalvular atrial fibrillation were higher than those in hypertensive patients without nonvalvular atrial fibrillation, and the difference was statistically significant (P<0.05). sST2, hs-CRP and LAD in patients with heart failure and nonvalvular atrial fibrillation were higher than those without heart failure and the difference was statistically significant (P<0.05). The level of sST2 in patients with successful total recovery of different diseases was lower than that of patients without successful total recovery of different diseases, and the difference was statistically significant (P<0.05). The hs-CRP level of patients with different disease types with successful total recovery was lower than that of patients with different disease types without successful total recovery, and the difference was statistically significant (P<0.05). The level of sST2 in the relapse group of different diseases was higher than that in the non-relapse group of different diseases, and the difference was statistically significant (P<0.05). The level of hs-CRP in the relapsed group was higher than that in the non-relapsed group, and the difference was statistically significant (P<0.05). Conclusions ST2 may reflect myocardial fibrosis, while hs-CRP may indicate inflammation, which jointly affect the occurrence and outcome of nonvalvular atrial fibrillation. Monitoring sST2 and hs-CRP levels is helpful to evaluate the condition, guide individualized treatment, and provide biomarker basis for accurate diagnosis and treatment of nonvalvular atrial fibrillation.

Objective To explore the application effect of double-mirror surgical intervention in gallbladder stones combined with common bile duct stones and its impact on the prognosis of patients. Methods A total of 82 patients with gallbladder stones combined with common bile duct stones who were admitted to Anqiu People's Hospital from February 2023 to September 2024 were selected and divided into two groups according to different surgical methods, with 41 cases in each group. The control group was treated with open surgery, while the observation group was treated with endoscopic sphinc terotomy(EST) combined with laparoscopic cholecystectomy (LC). The effects of the patients in both groups were evaluated 2 weeks after the operation. The bilirubin levels, inflammatory factors, stress responses, stone clearance rates and the incidence of postoperative complications were compared between the two groups. Results Before the intervention, there was no statistically significant difference in the levels of total bilirubin (TBIL), indirect bilirubin (IBIL), and direct bilirubin (DBIL) between the two groups (P>; 0.05). After the intervention, the levels of TBIL, IBIL and DBIL in both groups were lower than those before the intervention, and those in the observation group were lower than those in the control group. The differences were statistically significant (P<; 0.05). Before the intervention, there was no statistically significant difference in the levels of tumor necrosis factor -α (TNF-α), interleukin-6 (IL-6), interleukin-17 (IL-17), and C-reactive protein (CRP) between the two groups (P>; 0.05). After the intervention, the levels of TNF-α, CRP, IL-6 and IL-17 in both groups were lower than those before the intervention, and those in the observation group were lower than those in the control group. The differences were statistically significant (P<; 0.05). Before the intervention, there was no statistically significant difference in the levels of norepinephrine (NE), cortisol (Cor), and C-peptide (CP) between the two groups (P>; 0.05). After the intervention, the levels of NE, Cor and CP in both groups were higher than those before the intervention, but those in the observation group were lower than those in the control group, and the difference was statistically significant (P<; 0.05). There was no statistically significant difference in the stone clearance rate between the two groups of patients (P>; 0.05). The incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<; 0.05). Conclusion Surgical intervention based on double mirrors for the treatment of patients with gallbladder stones combined with common bile duct stones has a good effect. It can achieve a stone clearance rate similar to that of open surgery, help reduce the bilirubin level of patients, alleviate the inflammatory response of the body, cause less stress response during surgery, and have a lower incidence of postoperative complications. It is worthy of application.

Objective To explore the effect of pediatric massage combined with Zangfu point massage on children's intussusception. Methods 220 children with intussusception admitted to Erdos Hospital of Traditional Chinese Medicine from June 2022 to December 2023 were divided into control group and observation group by random number table method, 110 cases in each group. Both groups were treated with basic treatment, the control group was treated with pediatric massage on this basis, and the observation group was treated with zang-fu point massage on the basis of the control group. The success rate, reduction time, clinical symptom score, inflammatory factors and recurrence were compared between the two groups. Results The successful rate of reduction in observation group was higher than that in control group, and the reduction time in observation group was shorter than control group, the difference was statistically significant (P<0.05). Before treatment, there was no significant difference in the scores of abdominal pain, vomiting, jam-like bloody stools, irritability and bowel ringing between the two groups (P>0.05). After treatment, the symptom scores of abdominal pain, vomiting, jam-like bloody stool, irritability and bowel sound in 2 groups were lower than before treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). Before treatment, there was no significant difference in serum tumor necrosis factor α (TNF-α) and interleukin-6 (IL-6) levels between the two groups (P>0.05). After treatment, serum TNF-α and IL-6 levels in 2 groups were lower than before treatment, and observation group was lower than control group, the difference was statistically significant (P<0.05). The total recurrence rate of the observation group was lower than control group, and the difference was statistically significant (P<0.05). Conclusion The combination of pediatric massage and zangfu point massage in children with intussusception can significantly improve clinical symptoms and inflammation, enhance the effect of rehabilitation, and reduce the risk of recurrence.

Objective The aim of this study is to explore the effects of different types of crystalloid co-load on parturients and newborns during spinal anesthesia for caesarean delivery. Methods A total of 165 women undergoing cesarean section under spinal anesthesia in our hospital from January 2023 to January 2024 were randomly allocated to the lactated Ringer's solution group (Rgroup, 83) and the sodium potassium magnesium calcium glucose injection group (Ngroup, 82). Group R received a total infusion of 15 mL/kg lactated Ringer's solution administered rapidly within 30 minutes starting with intrathecal injection (crystalloid co-load) while Group N received a total infusion of 15 mL/kg Sodium Potassium Magnesium Calcium Glucose Injection. The incidence of hypotension, hemodynamic indicators, incidence of nausea and vomiting, dosage of vasoactive drugs, and neonatal umbilical artery blood gas during surgery between two groups were recorded. Results The incidence of hypotension and nausea and vomiting after delivery in group N was lower than that in group R, and the difference was statistically significant (P<0.05); The total amount of vasoactive drugs used in Group N and the lactate value in neonatal umbilical cord blood were lower than those in Group R, and the difference was statistically significant (P<0.05). Conclusion The co-load of sodium, potassium, magnesium, calcium, and glucose injection has a better effect on improving spinal anesthesia related hypotension in cesarean section than lactated Ringer's solution, and also has a certain improvement effect on neonatal umbilical cord blood lactate levels.

Objective To explore the effects of the OEC management model based on an information management platform in patients undergoing maintenance hemodialysis (MHD). Methods A total of 156 patients regularly receiving hemodialysis in our hospital in 2022 were selected as the study subjects, and randomly divided into a control group and an experimental group, with 78 patients in each. The control group used the conventional management model, while the experimental group implemented the OEC management model.After 3 months of intervention, the rate of compliance with relevant biochemical indicators, hospitalization rate due to dialysis complications and quality of life were compared between the two groups. Results Before intervention, there were no significant differences in the levels of blood phosphorus, blood potassium, blood calcium, albumin, hemoglobin and 36-item short form(SF-36) scores between the two groups (P>0.05). After intervention, the levels of blood phosphorus and potassium in both groups were lower than before intervention, and the experimental group was lower than control group, the difference was statistically significant (P<0.05). After intervention, the levels of blood calcium, albumin, hemoglobin and SF-36 scores in both groups were higher than before intervention, and the experimental group was higher than the control group, the difference was statistically significant (P<0.05). The hospitalization rate of observation group within 3 months was lower than control group, and the difference was statistically significant (P<0.05). Conclusion The OEC management model based on an information management platform, when applied to the management of MHD patients, can significantly improve the rates of meeting target levels for blood phosphate, albumin, and hemoglobin, reduce hospitalization rates due to dialysis complications, and enhance the quality of life of patients.

Objective To explore the effect of visual management combined with six Sigma management mode in the management of intravenous drug dispensing center (PIVAS). Methods A total of 436,896 prescriptions under the conventional management mode from July to December 2023 were recorded as the control group, and 421,749 prescriptions under the visual management combined with six Sigma management mode from January to June 2024 were recorded as the observation group. A systematic random sampling of 1300 pieces was used. The dispensing efficiency, dispensing quality, incidence of adverse events and satisfaction of medical staff on PIVAS were compared between the two groups. Results The dosage per person per hour in the observation group was higher than that in the control group, and the dosage time per bottle in the observation group was shorter than control group, the difference was statistically significant (P<0.05). The timely dispensing rate, accuracy rate and qualified rate of finished infusion in the observation group were higher than control group, and the differences were statistically significant (P<0.05). The allocation error rate, drug damage rate, unreasonable doctor's order rate and adverse event rate of observation group were lower than control group, and the difference was statistically significant (P<0.05). The satisfaction of medical staff in the observation group was higher than that in the control group on the quality of finished PIVAS infusion, the accuracy of drug delivery, the timeliness of drug delivery, the rationality of liquid distribution in each batch, and the timeliness of solving clinical feedback problems, the difference was statistically significant (P<0.05). Conclusion Visual management combined with six Sigma management can improve the work efficiency and quality of PIVAS, reduce the occurrence of adverse events and improve the satisfaction of medical staff in ward.

Objective To explore the application effect of the teaching mode of pain management after orthopedic surgery based on the integration of rational-virtual-real. Methods A total of 116 orthopedic nurses who practiced in Yantai Yeda Hospital from September 2023 to August 2024 were selected and divided into the study group and the control group according to the random number table method, with 58 in each group. The control group received the traditional teaching of orthopedic postoperative pain management, and the study group received the integrated teaching of rational-virtual and real-real on this basis. The scores of nurses'knowledge and attitude survey regarding pain (NKAS), core competence, stress, self-efficacy and teaching satisfaction were compared between the two groups. Results Before training, there was no difference in NKAS scores between the two groups (P>0.05). After training, the NKAS scores of the two groups were higher than control group, and the study group was higher than control group, the difference was statistically significant (P<0.05). Before training, the scores of pain management ability, clinical skills, pain emergency response ability, doctor-patient communication ability and information collection ability were compared between the two groups, and there was no statistical significance (P>0.05). After training, the scores of pain management ability, clinical skills, pain emergency response ability, doctor-patient communication ability and information collection ability of both groups were higher than before training, and the study group was higher than control group, the difference was statistically significant (P<0.05). The stress score of the study group was lower than control group, and the self-efficacy score of the study group was higher than control group, the difference was statistically significant (P<0.05). The total teaching satisfaction of the study group (96.55%) was higher than control group (86.21%), and the difference was statistically significant (P<0.05). Conclusion The teaching mode of pain management after orthopedic surgery based on the integration of rational-virtual-reality can effectively help practice nurses master pain management knowledge, improve their core competence, self-efficacy, relieve their pressure, increase teaching satisfaction and improve teaching quality.

In the era of rapid advancements in medical-engineering integration, artificial intelligence (AI) is progressively permeating various aspects of medical education, offering unprecedented opportunities for innovation in neurosurgical training. Traditional neurosurgical education is constrained by a steep learning curve, limited practical resources, and challenges in implementing personalized teaching. The integration of AI provides new solutions to these challenges. This paper explores the key applications of AI in neurosurgical education, including virtual and augmented reality-based surgical simulation training, AI-assisted medical imaging analysis, and automated surgical operation assessment. These technologies enhance learning efficiency, optimize surgical training, and provide personalized real-time feedback to trainees, thereby improving overall teaching quality. However, the widespread implementation of AI in neurosurgical education still faces challenges related to data privacy protection, technological adaptability, and educator acceptance. Moving forward, it is essential to promote deeper integration between medicine and engineering, further optimize AI-driven educational models, and establish standardized teaching frameworks to ensure the sustainable development and broad application of AI in neurosurgical education.

Objective This study aims to analyse in depth the knowledge, beliefs and behaviours of the elderly population regarding mental health from an active health perspective, and to identify the underlying mechanisms of influence. Methods Using a convenience sampling method, 311 elderly people were surveyed using the self-designed questionnaire on the current situation of mental health of the elderly from the perspective of active health, and the data obtained were processed using SPSS 25.0 software and analysed using descriptive statistical analysis, t-test, one-way analysis of variance (ANOVA), multivariate linear regression analysis and mediation analysis. Results The mean score of mental health knowledge of older people from an active health perspective was (6.47±1.84); the mean score of attitude was (7.10±1.82); and the mean score of behaviour was (5.78±1.92). Regression analysis showed that ethnicity, annual income, literacy level and medical expenses were the influencing factors of knowledge and behaviour; and the mean score of annual income, literacy level, exercise situation and medical expenses were the influencing factors of attitude. Mediation analysis showed that mental health knowledge predicted attitude (β=0.577, P<0.001), behavioural intention (β=0.190, P<0.01)and behaviour (β=0.373, P<0.001), and that attitude positively predicted behavioural intention (β=0.244, P<0.01)and behaviour (β=0.120, P<0.05). Behavioural intentions positively predicted behaviour (β=0.321, P<0.001). The value of the direct effect of knowledge on behaviour was 0.389 or 68% of the total effect and the value of the indirect effect was 0.183 or 32.01% of the total effect. Conclusion The level of mental health knowledge and attitudes among the elderly is at a medium level, and the level of behaviour change is low; mental health attitudes and behavioural intentions play a partial mediating role between knowledge and behaviour.

Objective To explore the application effect of midwifery care in maternal childbirth. Methods A total of 62 primiparous women who visited Heze Hospital of Traditional Chinese Medicine from June 2021 to August 2024 were selected as the research subjects, and they were divided into control group and observation group by random number table, with 31 cases in each group. The control group was given routine care mode, and the observation group was given responsibility-assisted delivery care mode. The labor time, perineal condition and postpartum bleeding at 2 h were compared between the two groups. Results The time of labor, lateral perineal resection rate, degree of perineal fissure, and postpartum bleeding at 2 h in the observation group were lower than that of the control group,and the difference was statistically significant (P<0.05). Conclusion Compared with the application of routine nursing and midwifery nursing, the midwifery system can effectively shorten the labor process, protect the perineal situation and reduce the amount of bleeding after 2 h postpartum.

Objective To investigate the effect of narrative therapy in patients with postoperative colostomy for colorectal cancer. Methods A total of 80 patients with postoperative colostomy for colorectal cancer treated in our hospital from January 2019 to January 2024 were selected and divided into observation group (40) and control group (40) by random number table method. The control group was given routine intervention, and the observation group was given narrative therapy on the basis of the control group. Negative emotion, coping style and social psychological adaptation level were compared between the two groups. Results Before intervention, there was no significant difference in Beck Anxiety Inventory(BAI) score and Beck Depression Inventory-Ⅱ (BDI-Ⅱ) scores between the two groups (P>0.05). After intervention, the BAI score and BDI-Ⅱ score of the two groups were lower than before intervention, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). Before intervention, there was no difference in positive coping score and negative coping score between the two groups (P>0.05). After intervention, the positive coping scores of both groups were higher than before intervention, and the observation group was higher than the control group, the difference was statistically significant (P<0.05). After intervention, the negative coping scores of both groups were lower than before intervention, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). Before intervention, there was no significant difference in OAI-20 scores between the two groups (P>0.05). After intervention, the Ostomy Adjustment Inventory-20 (OAI-20) scores of both groups were higher than before intervention, and the observation group was higher than the control group, with statistical significance (P<0.05). Conclusion The application of narrative therapy in the intervention of postoperative colostomy patients with colorectal cancer can reduce negative emotions, improve the level of social psychological adaptation, and promote the improvement of coping style.

Objective To explore the comprehensive effectiveness of the maternal and infant home care model based on Rogers' holistic human theory model. Method This study selected 110 maternal and child families from a tertiary hospital from July 2024 to December 2024 as research subjects. They were randomly divided into an observation group and a control group using a random number table method, with 55 cases in each group. The control group used the traditional maternal and child care service model, while the observation group used a home care model carefully constructed based on Rogers' holistic human theory model. Compare and analyze the incidence of maternal and infant complications, psychological status scores, satisfaction, and other indicators between the two groups. Result The incidence of postpartum infection, mastitis, and neonatal umbilical cord infection in the observation group was lower than that in the control group (P<0.05). In the pre admission anxiety self-assessment scale (SAS) and depression self-assessment scale (SDS) evaluations, there was no statistically significant difference between the observation group and the control group; In the 30 day postpartum assessment, the SAS and SDS scores of the observation group were significantly lower than those of the control group (P<0.05); The satisfaction rate of the observation group was higher than that of the control group (P<0.05). Conclusion The maternal and infant home care model constructed and applied based on Rogers' holistic human theory model can accurately and comprehensively meet the physiological, psychological, social and other multidimensional needs of mothers, infants and their families, and has outstanding clinical practice value and broad prospects for application.
The Value of Constructing Multimodal CT Imaging Histology Model to Predict the Occurrence of Unstable Angina in Patients with Aortic Plaque

Objective To analyze the predictive value of a multimodal CT radiomics model in predicting unstable angina in patients with aortic plaques. Methods A total of 236 patients with aortic plaque who underwent multimodal CT examination in our hospital from January 2023 to December 2023 were selected for this study. Combined with 1-year follow-up, they were divided into non-occurrence group (200) and occurrence group (36), and the difference data between the two groups were replaced by Logistic regression equation. The influencing factors of unstable angina pectoris were determined, the predictive model was constructed, and the receiver operating characteristic curve (ROC) was drawn to evaluate the model efficacy. Results The location of aortic plaque, degree of aortic stenosis, CT value of aortic plaque, calcification score of aortic root, and volume of aortic calcification were all influential factors for the occurrence of unstable angina pectoris in patients with major arterial plaque, and the OR values were all greater than 1. By constructing a nomogram, ROC curve was drawn to verify the prediction efficiency of the model and the AUC was found to be 0.990 (95%CI: 0.970-1.000). Conclusion The multi-modality CT image omics prediction model has a high value in predicting unstable angina pectoris in patients with aortic plaque, and can provide evidence for clinical screening of high-risk groups and taking measures.
