Objective To visualize and analyze the current status, hotspots and trends of research on stroke in young and middle-aged people in the past 30 years with the help of CiteSpace software. Methods The CNKI database was used as the search source, and CiteSpace 6.2.R4 visual analysis software was used to visualize and analyze the studies on stroke in young and middle-aged people in the past 30 years, exploring the number of publications, authors, institutional distribution, and keywords of related studies. Results 891 articles were included, and the author with the highest posting volume was Zhenxiang Zhang, and the School of Nursing and Health of Zhengzhou University is an important institution for stroke in young and middle-aged people, and the main research hotspots are the risk factors, etiology, and typology of stroke in young and middle-aged people. Conclusion This study provides a series of summaries and generalizations of the current stage of research progress on stroke in young and middle-aged people, providing researchers with valuable information that is effective and can be used quickly.
Clinical Efficacy and Adverse Reaction of Secukinumab Combined with Herbal Fumigation in the Treatment of Moderate to Severe Plaque Psoriasis

Objective To observe the clinical efficacy and adverse reaction of secukinumab with herbal fumigation in the treatment of patients with moderate to severe plaque psoriasis. Methods A total of 68 patients with moderate to severe plaque psoriasis were randomly divided into two groups by digital table method, with 34 cases in each group.The control group was treated with self-made traditional Chinese medicine Yangxue Runzao Qufeng soup and tacrolimus ointment, and the treatment group was received treatment with 300 mg of Sikuximab on the basis of the control group. Both groups will require continuous observation for at least 24 weeks. The efficacy was assessed by the changes of Psoriasis Area and Severity Index (PASI) , Body Surface Area (BSA),and Dermatology Life Quality Index(DLQI) before treatment and at weeks 4, 12, and 24. The adverse drug reaction was also analyzed. Results After 24 weeks of treatment, no patient exit from the study due to drug intolerance or adverse drug reactions. After weeks 4, 12 and 24, there were significant differences in PASI score, BSA score and DLQI score in the treatment group compared with the control group (P <0.05); Except that there were no significant difference in PASI 100 response rate at week 4 (P =0.063), PASI 75/90/100 response rates were all statistically significant (P<0.05); and DLQI 0/1 response rates were also significant differences (P<0.001). Conclusion Secukinumab with herbal fumigation in the treatment of patients with moderate to severe plaque psoriasis can obtain high PASI 75/90/100 and DLQI 0/1 response rates and has a relatively high safety profile.

Objective To explore the effect of symptom diary on the rehabilitation period in patients with severe hand trauma. Methods Patients with severe hand trauma in a tertiary orthopedic hospital from January 2023 to December 2023, were divided into intervention group (74 cases) and control group (72 cases) according to the order of admission. The control group received routine care, and the intervention group used symptom diary on the basis of the control group. Six months after the intervention, the complication reporting accuracy and reporting time were compared between the two groups. Comparing the differences in physical function, anxiety, depression, fatigue, sleep distress, and social interaction in group 2. Results The control group (69 cases) and the intervention group (66 cases) completed the study respectively.The proportion of complications (cases) and confirmed symptoms (cases) reported by the intervention group was significantly higher than that of the control group, with a significantly higher reporting accuracy (%) and a significantly shorter reporting time.The differences were statistically significant (P<0.05). Compared to the control group, the intervention group had lower physical functioning, anxiety, depression and fatigue scores after 6 months of follow-up, and the difference was statistically significant (P<0.05). The difference between the two groups in sleep disturbance and social interaction scores was not statistically significant (P>0.05). Conclusion Symptom diary can help improve the early identification of complications in patients with severe hand trauma, reduce the burden of symptoms, and improve their quality of life.

Objective To study the clinical effect of Hanxia Baijiu Tianma Tang combined with Rosuvastatin in the treatment of carotid atherosclerotic plaque formation. Methods A total of 60 patients with carotid atherosclerotic plaque formation were selected, and according to the principle of randomized controlled grouping, 30 patients belonged to the control group and 30 to the observation group; The control group received Rosuvastatin, and the observation group received Rosuvastatin combined with Hanxia Baijiu Tianma Tang , The two groups were administered for four consecutive weeks, and the levels of lipid indexes [blood total cholesterol (TC), low-density lipoprotein cholesterol( LDL-C), Triglycerides (TG)], carotid plaque, Crouse points, and Chinese medicine symptoms were observed before and after the treatment. The clinical efficacy was evaluated by observing the levels of blood lipid indexes (blood TC, LDL-C, TG), carotid plaque, Crouse's score, and TCM symptom scores before and after treatment. Results After the intervention, the lipid index of the two groups was lower than that before the intervention, and the difference was statistically significant (P<0.05), and the lipid index of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). After the intervention, the carotid Crouse points of the two groups were lower than before the intervention, and the difference was not statistically significant (P>0.05), and the carotid Crouse points of the observation group were lower than those of the control group, and the difference was not statistically significant (P>0.05). After the intervention, the TCM Crouse points of the two groups were lower than those before the intervention, and the difference was statistically significant (P<0.05), and the TCM Crouse points of the observation group were lower than those of the control group, and the difference was statistically significant (P<0.05). The total effective rate of treatment in the observation group was 93.33%, which was higher than the total effective rate of treatment in the control group of 66.67%, and the difference was statistically significant (P<0.05). Conclusion Combined treatment of carotid atherosclerotic plaques with Hanxia Baijiu Tianma Tang and Rosuvastatin has significant efficacy in the regulation of lipid metabolism and the alleviation of patients' clinical symptoms.

Objective By analyzing the blood drug concentrations of patients with fungal infections treated with voriconazole and fluconazole alone, we aim to explore the clinical significance of serum drug concentrations in patients' antifungal treatment and promote individualized and precise drug therapy. Methods Blood samples from patients with fungal infections treated with oral or intravenous voriconazole and fluconazole from May 2022 to December 2023 were collected and their blood drug concentrations were detected. Among them, there were 360 blood samples from patients with fungal infections treated with voriconazole alone and 109 blood samples from patients with fungal infections treated with fluconazole alone. Homogeneous enzyme immunoassay (HEIA) was used to detect the serum drug concentrations of voriconazole and fluconazole, and the results were statistically analyzed. Results 469 samples were collected for testing. Among them, 360 blood samples from patients with fungal infections treated with voriconazole alone involved 170 patients. The number of patients with fungal infections treated with voriconazole alone who underwent blood drug concentration testing was relatively high; 109 blood samples from patients with fungal infections treated with fluconazole alone involved 86 patients. The number of patients with fungal infections treated with fluconazole alone who underwent blood drug concentration testing was relatively low; the serum drug concentration of voriconazole was controlled within the effective concentration range; the serum drug concentration of fluconazole was controlled within the effective concentration range; some patients using voriconazole and fluconazole had interactions with other drugs. Conclusion After the application of antifungal drugs in patients with fungal infections, the detection of serum drug concentrations is crucial for guiding the next step of clinical treatment of antifungal infections. It is recommended to adjust the drug regimen according to the serum drug concentrations of patients after medication in a timely manner, and develop individualized drug regimens to ensure the safety, effectiveness, and economy of drug treatment for patients.
Analysis of Influencing Factors of Pediatric Central Venous Access Device-Associated Thrombus and Construction of Nomogram Prediction Model

Objective To analyze the factors affecting pediatric central venous access device-associated thrombosis(CADVT) and construct a nomogram prediction model. Methods The clinical data of 105 children who received central venipuncture catheters in the department of Pediatrics of the hospital from February 2021 to February 2024 were retrospectively analyzed, and they were divided into thrombus group and non-thrombus group according to whether CADVT was complicated during treatment. Clinical data of enrolled patients were collected and recorded, and the risk factors affecting pediatric CADVT were screened by Logistic multivariate regression analysis, a pediatric CADVT risk nomogram model was constructed based on the above risk factors, goodness of fit test and receiver operating characteristic (ROC) curve were used to evaluate the predictive efficacy of the nomogram model. Results According to clinical evaluation, CADVT occurred in 30 of 105 children who received central venipuncture during treatment, with an incidence of 28.57%. There were significant differences in critical case score (PCIS) score, catheterization duration ≥7 d, vasoconstrictor use ≥2 types, sedation days ≥7 d, D-dimer (D-D) and fibrinogen (FIB) levels between thrombus group and non-thrombus group (P<0.05), there were no significant differences in sex ratio, age, body mass index (BMI), placement, number of catheters and WBC (P>0.05). Logistic multivariate regression analysis assigned the following values: PCIS score (OR=3.408, 95%CI: 1.498~7.752), catheter placement duration ≥7 days (OR=3.971, 95%CI: 1.746~9.033), vasoconstrictor use ≥2 (OR=2.507, 95%CI: 1.102~5.702), sedation days ≥7 days (OR=2.323, 95%CI: 1.021~5.285), D-D (OR=3.459, 95%CI: 1.521~7.869), FIB (OR=3.304, 95%CI: 1.452~7.515) were independent risk factors for pediatric CADVT. The measured values of the pediatric CADVT risk nomogram model constructed according to the above risk factors were basically consistent with the predicted values, and the C-index was 0.895 (95%CI: 0.796~0.937). Conclusion PCIS score, catheterization duration, vasoconstrictor use, sedation days, D-D and FIB were the risk factors affecting the occurrence of pediatric CADVT, and the pediatric CADVT nomogram model constructed based on this was effective in predicting the occurrence of pediatric CADVT.

Objective To study the therapeutic effects of different surgical procedures on pregnant women with acute appendicitis and their impact on perioperative indicators. Methods A random number table method was used to divide 58 pregnant women with acute appendicitis at Shouguang People's Hospital from January 2020 to February 2023 into two groups (29 cases each). The control group received open surgery, while the study group received laparoscopic surgery. The clinical efficacy of the two groups was compared. Results The study group had shorter bed activity, anal exhaust, and hospital stay compared to the control group (P<0.05). On the 1st day after surgery, the levels of inflammatory factors, malondialdehyde, vasoactive intestinal peptide, and incidence of complications in the study group were lower than those in the control group, while the levels of oxide dismutase, gastrin, and cholecystokinin were higher than those in the control group (P<0.05). There was no significant difference between the two groups in terms of gestational age, newborn body weight, and A-score during delivery(P>0.05) . The preterm birth rate and cesarean section rate in the study group were lower than those in the control group, with statistically significant differences (P<0.05). There was no statistically significant difference in the comparison of abortion rates between the two groups (P>0.05). Conclusion Laparoscopic surgery has a milder inflammatory and stress response, less impact on gastrointestinal function, and faster postoperative recovery in patients.

Objective To analyzed the application effect of new ideas for teaching and improve the quality of teaching conducted research on the combination of micro courses and problem-based learning on the DING-DING platform. Methods Sixty intern nursing students from the Department of Otolaryngology at Weifang Traditional Chinese Medicine Hospital from July 2023 to April 2024 were selected as the research subjects. Divided into a control group of 30 and a study group of 30 according to the random number table method. The control group was taught using traditional teaching methods, while the research group was taught using a combination of DING-DING platform micro courses and problem-based learning. Before the end of the nursing internship, evaluation will be conducted through theoretical and operational assessments, teaching effectiveness evaluation forms, and survey questionnaires. Results The theoretical and operational exam scores of the two groups of nursing students were higher in the study group than in the control group, and the difference was statistically significant (P<0.05). The evaluation of teaching effectiveness by nursing students in this department showed that the research group was higher than the control group, and the difference was statistically significant (P<0.05). Conclusion The application of the DING-DING platform micro course problem-based learning model in otolaryngology nursing teaching has significantly improved the theoretical and operational level of nursing students, and better stimulated their learning interest.

Objective To review and analyze the online to offline (O2O) model medication consultation service of Beijing Hospital of Traditional Chinese Medicine, Capital Medical University in the first half of 2021, share the O2O medication consultation working mode of our hospital, and promote the O2O medication consultation service level. Methods Based on the drug consultation service platform of our hospital, all records of outpatient, telephone and online drug consultation services provided by pharmacists in Beijing Hospital of Traditional Chinese Medicine, Capital Medical University hospital from January 1 to June 30, 2021 were summarized, and the workload, consultation content, drug type, traditional Chinese medicine decoction consultation, number and frequency of consultation questions, radiation range of O2O drug consultation services were counted and analyzed. Results In the first half of 2021, the O2O medication consultation service in our hospital served 5 614 person times and answered 6 832 questions. The workload of outpatient and telephone consultation was large, and the number of online consultation was relatively small; The consultation contents mainly include basic information of drugs (56.40%) and decoction of traditional Chinese medicine (34.17%), and the consultation services of basic information of drugs mainly include preparations in our hospital (72.31%). The decocting and taking problems of traditional Chinese medicine decoction were mainly about decoction (50.47%) and special decocte herbs(31.26%). Most consultations are single question and single consultation. Conclusion The integrated O2O medication consulting service can provide patients with more simple and cheap pharmaceutical care, and there is still room for improvement. The relevant standards, laws and regulations and service charge standards still need to be improved.

Objective Exploring the effect of nutritional supplements on improving endurance running ability in handball players with low hemoglobin levels. Methods Twelve female handball players from Anhui Province who participated in the preliminary round of the 14th National Games were selected. Five athletes whose endurance running test results were lower than the threshold standard of the National Games were selected as the observation group, and the other seven athletes were selected as the control group. Both the control group and the observation group were trained according to the pre competition training plan, and their diet was managed uniformly. They ate three meals a day at the athletes’ restaurant as usual.The observation group added ass hide glue, folic acid, Vitamins With Minerals Tablets(29), ferrous sulfate and other nutritional supplements for comprehensive intervention, and the intervention training was 8 weeks. Compare the biochemical indicators of hemoglobin, serum ferritin, blood urea nitrogen, serum creatine kinase, serum testosterone, cortisol and physical fitness indicators of endurance running performance in athletes before and after intervention. Results After 8 weeks of intervention, the endurance running performance of the experimental group improved (P<0.05), and compared with the control group, the improvement in performance was higher (P<0.05); The hemoglobin level in the experimental group increased (P<0.05), and the increase was higher than that in the control group; The serum transferrin level in the experimental group increased (P<0.05), and the increase was higher than that in the control group (P<0.05). Conclusion Drug intervention combined with lactate tolerance training can effectively improve the endurance running ability and hemoglobin level of female athletes.

Objective To solve the difficulties encountered by elderly people in outpatient treatment and improve the outpatient experience of elderly patients. Methods A total of 200 elderly patients in the outpatient department of Zibo Central Hospital from October 2020 to June 2023 were selected and divided into pre-improvement group and post-improvement group (100 cases in each group) according to the order before and after the implementation of the project. The "Elderly outpatient Convenience and service perception Evaluation Scale" was adopted, and the data were processed by SPSS26.0 statistical software. Result After the improvement, the average convenience of booking was 2.23, higher than the average 1.07 before the improvement, and the difference was statistically significant (P<0.05). The average value of service attitude of medical staff after improvement was 1.92, higher than the average value of 1.07 before improvement, and the difference was statistically significant (P<0.05). After the improvement, the mean value of road signs and indications in the hospital was 2.32, higher than the mean value of 1.17 before the improvement, and the difference was statistically significant (P<0.05). The mean value of convenience of outpatient procedures after improvement was 2.29, higher than the mean value of 1.09 before improvement, and the difference was statistically significant (P<0.05). The mean value of overall satisfaction after improvement was 2.10, higher than the mean value before improvement of 1.11, the difference was statistically significant (P<0.05). Conclusion Guided by the needs of patients, taking management as the means, and taking diagnosis and treatment as the core, the "10+2" convenience measures formulated for the elderly are implemented through continuous improvement of all aspects of medical treatment and optimization of all processes, and at the same time, new methods and new measures are actively explored to meet the new needs of medical services for the elderly, make the medical treatment process more convenient, and improve the perception and evaluation of outpatient services.
