    Observation of Clinical Efficacy of Traditional Chinese Medicine Intervention under Thought of Acrid Dispersing and Bitter Descending on Subclinical Hypothyroidism of Hashimoto's Thyroiditis

    Objective To clinically observe and evaluate the intervention efficacy of Chinese herbal medicine under thought of acrid dispersing and bitter descending on subclinical hypothyroidism of Hashimoto's thyroiditis. Methods From January 2023 to January 2024, 30 patients with subclinical hypothyroidism of Hashimoto's thyroiditis treated at Tongzhou Hospital, Dongzhimen Hospital, Beijing University of Chinese Medicine were randomly divided into control group and treatment group according to a random number table, with 15 cases in each group. The control group was given oral levothyroxine sodium tablets plus Chinese medicine placebo, and the treatment group was given Chinese herbal medicine treatment with Chinese medicine under thought of acrid dispersing and bitter descending on the basis of Western medicine treatment in the control group, with a course of 8 weeks. The changes in thyroid hormone levels and thyroid antibodies levels before and after intervention were compared between the two groups. Results Compared with before intervention, both the treatment group and the control group could effectively reduce the levels of thyroid stimulating hormone (TSH) after intervention(P<0.05), and the total effective rate of the treatment group was significantly higher than that of the control group(P<0.05). After the intervention, the TCM total symptom scores in both groups were significantly reduced compared to before the intervention, and the treatment group showed an improvement in the total symptom score after the intervention, which was significantly better than that of the control group(P<0.05). Conclusion Chinese herbal medicine under thought of acrid dispersing and bitter descending can improve the levels of TSH in subclinical hypothyroidism of Hashimoto's thyroiditis and enhance the clinical efficacy.

    Objective A systematic evaluation of the clinical efficacy of enhanced extracorporeal counterpulsation technique combined with traditional Chinese medicine in the treatment of chronic heart failure using reticulation meta-analysis. Methods A thorough search was conducted across PubMed, the Cochrane Library, Web of Science, Wanfang database, China Knowledge Network, and Wikipedia library. All Randomized Controlled Trials (RCTs) up to March 31, 2023, examining the combined use of EECP technique and TCM in the treatment of chronic heart failure were scrutinized and studied. All data was processed using RevMan 5.3 software. Results Fifteen studies were ultimately included, encompassing a total of 1,265 patients. The outcomes showed that: ① In the experimental group, Left Ventricular Ejection Fraction (LVEF) performed better (95%CI: 3.72~9.47, P<0.001);② The results of the 6-minute walk test (6MWT) were also superior to the control group (95%CI: 27.62~65.99, P<0.001);③ N-terminal pro-Brain Natriuretic Peptide (NT-proBNP) was lower in the experimental group compared to the control group (95%CI: -353.25~-18.61, P=0.03);④ Cardiac function was more efficacious than that of the control group (95%CI: 1.01~1.50, P=0.04);⑤ The efficacy of Traditional Chinese Medicine was superior to the control group (95%CI: 1.05~1.38, P=0.009);⑥ Nitric Oxide (NO) levels increased compared to the control group (95%CI: 1.34~23.04, P=0.03);⑦ Overall clinical efficacy was also superior to the control group (95%CI: 1.11~1.30, P<0.001). Conclusion Adding EECP and TCM to a solely Western medical treatment regimen has shown improvements in LVEF, 6MWT, cardiac function, the efficacy of TCM, NO levels, and overall clinical efficacy. It also effectively reduced the levels of NT-proBNP in patients with chronic heart failure.

    Objective To compare the recovery of the radial rotation arch in the treatment of radial shaft fractures with steel plates and intramedullary nails. Methods A retrospective analysis was conducted on 60 hospitalized patients at Dongping County People's Hospital from October 2021 to September 2023. All included patients had unilateral radial shaft fractures (with or without ipsilateral ulnar shaft fractures) and chose surgical methods, with steel plates or intramedullary nails as the main internal fixation methods for treatment.According to the internal fixation method, A total of 32 patients chose to use steel plate internal fixation.Record the surgical time, incision length, maximum radial curvature after surgery, fracture healing time, range of motion of elbow wrist joint, forearm rotation at 6 months after surgery, DASH score, Grace Eversman score, etc. for two groups of patients. Results All patients underwent forearm anteroposterior and lateral X-ray films at the Radiology Department of Dongping County People's Hospital after surgery. After observation and research, it was found that the intramedullary nail group was superior to the steel plate group in terms of surgical time and incision length, and the difference was statistically significant (P<0.05). There was no statistically significant difference (P>0.05) between the two groups in terms of the fixed point value of maximum radius curvature. In terms of the ratio between the affected and healthy sides, there was no statistically significant difference (P>0.05) between the two groups in terms of fracture healing time and elbow wrist joint range of motion (last follow-up). There was no statistically significant difference (P>0.05) between the two groups in terms of forearm rotation (pronation and supination) in the neutral position at 90 ° elbow flexion 6 months after surgery. There was no statistically significant difference (P>0.05) between the two groups in terms of DASH and Grace Eversman scores. Conclusion Both internal fixation methods for treating radial shaft fractures can achieve good recovery of the radial rotation arch. The choice of intramedullary nail treatment has a smaller incision and shorter surgical time compared to steel plate treatment.

    Objective To investigate the clinical efficacy of acupuncture and dragon jar moxibustion in the treatment of peripheral facial palsy with wind-cold attack. Methods A total of 72 patients with peripheral facial palsy with wind-cold attack admitted to the Acupuncture Department of Hubei Hospital of Traditional Chinese Medicine from October 2023 to July 2024 were selected as the study subjects,and were divided into the control group and the observation group according to the method of randomised numerical table,with 36 cases in each group.On the basis of evidence-based care,the control group was treated with conventional acupuncture,and the observation group was treated with acupuncture combined with integrated moxibustion with dragon pots.H-B grade, FDI score and clinical efficacy were compared before and after intervention. Results After 4 weeks±2 days of intervention,H-B score of observation group was better than control group,the difference was statistically significant (P<0.05).after intervention,the FDI score was better in the observation group,the difference was statistically significant (P<0.05).The clinical effect was significantly better than that of the observation group (P<0.05). Conclusion Acupuncture combined with integrated moxibustion with fire dragon pot is beneficial to promote the recovery of facial nerve function in patients with peripheral facial paralysis of wind-cold attack type and improve the cure rate, which is worthy of clinical promotion and application.

    Myasthenia gravis presents a complex challenge in the field of medicine, for which both traditional Chinese medicine and Western medicine have yet to find complete solutions. Drawing from the principles of Qi blood essence spirit in Huangdi Neijing, Song Fuyin developed a differentiated theory of Qi blood essence spirit based on contemporary societal characteristics, comprehensive psychosomatic syndrome, functional disease syndrome, pathological features, and pathogenesis of challenging diseases. By applying the theory of syndrome differentiation, Song Fuyin distinguished between "qi disease" and "essence disease" treatments and posited that certain cases of myasthenia gravis may stem from an imbalance or stagnation of "essence" caused by liver channel obstruction.

    Objective To explore the effects of Qianhu Zhuye Tang of inflammatory factor levels, lung function and quality of life on stubborn cough in the elderly patients. Methods 120 cases of stubborn cough in the elderly patients were admitted to Central Health Center of Shanwanzi Township, Weichang Manchu and Mongolian Autonomous County, Hebei Province from January 2022 to January 2023 were seclect, Divided into control group (60cases) and observation group (60cases) using a random number table method. Control group and observation group were treated with conventional western medicine and qianhu zhuye decoction. The changes of inflammatory factor levels indexes, lung function indexes and quality of life were compared between two groups before and after treatment. Results There was no significant difference in the levels of interleukin-1 (IL-1), tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) between the two groups before treatment (P>0.05). Compared with the control group after treatment, The observation group had lower levels of IL-1, TNF-α and IL-6 (P<0.05). There was no significant difference in the Forced expiratory volume in the first second (FEV1), FEV1/forced vital capacity (FVC) and peak expiratory flow (PEF) between the two groups before treatment (P>0.05). Compared with the control group after treatment, the observation group had higher levels of FEV1, FEV1/FVC and PEF (P<0.05); There was no significant difference of short-form health survey-36 (SF-36) scores between the two groups before treatment (P>0.05), Compared with the control group after treatment, The observation group had a higher SF-36 scores (P<0.05). Conclusion Qianhu zhuye decoction in the treatment of stubborn cough in the elderly, which can effectively reduce inflammatory factors level in patients, improve lung function and the quality of life.

    Objective To observe the current status of nursing teaching in the emergency department,conduct teaching at the group level,and analyze the application effect of this model. Methods Using statistical software to divide 60 intern nurses in the emergency department of our hospital from January 2022 to December 2023 into two groups,of which 2022 intern nurses served as a control group and received routine nursing guidance As a research group,the 2023 intern nurses accept group-level guidance.After the teaching of the two groups,the department used the comprehensive ability evaluation system questionnaire to evaluate the comprehensive ability,assessment results and teaching satisfaction. Results The evaluation grades of pre-examination and diagnosis, condition observation, hands-on handling, and emergency coordination of patients in the research group were higher than those in the control group (P<0.05); The total assessment score of the internship nursing students in the research group was higher than that of the control group, and the difference was statistically significant (P<0.05);The satisfaction of the intern nurses in the research group with teaching was higher than that in the control group (P<0.05). Conclusion Nursing practice in emergency departments implements group-level teaching, and the results of out-of-hospital examination are satisfactory, which can effectively improve the comprehensive ability and teaching satisfaction of nursing students.

    Objective To understand the incidence level and trend of acute events of cardiovascular and cerebrovascular diseases among residents in Jiangyin City from 2017 to 2022, and to provide scientific basis for formulating effective measures for the prevention and treatment of cardiovascular and cerebrovascular diseases. Methods According to the data provided by the chronic disease collection system of Jiangsu Province, the monitoring results of acute events of cardiovascular and cerebrovascular diseases from 2017 to 2022 were statistically analyzed, and the differences in the incidence and incidence rate of cardiovascular and cerebrovascular diseases in different years, disease types, genders, and seasons were analyzed. Results From 2017 to 2022, a total of 76 429 cases of cardiovascular and cerebrovascular diseases were reported in Jiangyin, with an average annual incidence rate of 1 008.99/100 000. The incidence rate of acute events of cardiovascular and cerebrovascular diseases was 1 032.40/100 000 in 2017, 1 055.12/100 000 in 2019, and then decreased year by year. The acute events of stroke and coronary heart disease were 71806 and 4639, respectively, with an average annual incidence rate of 948.03/100 000 and 6.12/100 000, respectively. The incidence of stroke in the whole population, men and women increased first and then decreased; However, the overall incidence of acute coronary heart disease has slightly increased, with the incidence remaining relatively stable in the past five years. The difference of incidence rate between men and women in different years was statistically significant (χ2=125.98); There is a significant difference in the incidence of acute cardiovascular and cerebrovascular events among residents in Jiangyin City from 2017 to 2019 compared to 2020 to 2022. The number of cases varies greatly in winter and spring, and the difference in incidence is the smallest in summer. Conclusion There is a decreasing trend in the incidence of acute cardiovascular and cerebrovascular diseases in Jiangyin City from 2020 to 2022 compared to 2017 to 2019; Men have higher incidence than women.

    Objective Taking acute chest pain patients admitted to the chest pain center as the research object, this study explores the application value of rapid and accurate triage emergency nursing process, and evaluates the impact on patient triage evaluation time. Method 78 patients admitted to the Chest Pain Center from January 2023 to December 2023 were selected as the research subjects. According to nursing measures, the subjects were divided into a control group and an observation group. Patients who received routine nursing care were included in the control group, while patients who received rapid and accurate triage and emergency nursing care were included in the observation group. The nursing effects were compared, and evaluation indicators included emergency time, triage quality, and nursing satisfaction. Result The comparison of time taken for emergency stay, triage evaluation, first electrocardiogram, establishment of venous access, and cTnⅠ assay showed that the observation group had a shorter duration, and the difference was statistically significant compared to the control group (P<0.05). Through comparison, it was found that the observation group had a higher quality of triage, with scores for preliminary diagnosis, rapid triage, and treatment arrangement higher than the control group (P<0.05). Compared with the control group, the observation group showed higher satisfaction with nursing services, and the differences in various indicators were statistically significant (P<0.05). Conclusion The application of fast and accurate triage emergency nursing process during the emergency treatment of chest pain patients has a significant effect on shortening waiting and triage evaluation time, improving rescue effectiveness and triage quality, accurately identifying the cause of chest pain in patients, and adopting targeted treatment.     Objective To observe the effect of virtual reality (VR) intervention on HBO treatment of occupational noise deafness (ONID) patients with pre-treatment psychological stress. Methods 96 cases of ONID patients admitted to our hospital from February 2021 to February 2024 were selected and divided into control group (single number) and experimental group (even number) with 80 cases each according to the admission sequence number. Routine nursing intervention and nursing intervention using VR technology were respectively implemented. The scores of self-rating Depression Scale (SDS), self-rating Anxiety Scale (SAS) and General Self-efficacy Scale (GSES), treatment compliance, and hearing threshold levels of the two groups were observed under pure tone audiometry at different frequencies (500-4000 Hz). Results After intervention, the GSES scores of the control group were increased but far less than that of the experimental group, and the SDS and SAS scores were decreased, but not significantly (P<0.05). Compared with the experimental group, the treatment compliance in the control group was significantly worse (P<0.05). The hearing threshold levels of experimental groups were lower than those of control group at different frequencies after intervention (P<0.05). Conclusion The application of VR technology in the HBO treatment of ONID patients can significantly relieve the psychological stress before treatment, enhance the self-efficacy of patients, have a positive impact on improving the willingness of patients to accept HBO treatment, and promote the improvement of their hearing function, which is of great application and promotion value.

    Objective To explore the effect of applying concept reconstruction cognitive nursing and Roy adaptation mode nursing in elderly patients with CHF. Methods A total of 102 elderly CHF patients from October 2020 to October 2023 who were admitted in Neimenggu Autonomous Region People's Hospital were selected, and randomly divided into the intervention group (n=51, concept reconstruction cognitive care and Roy adaptation mode care) and the control group (n=51, routine care). The disease cognition, self-efficacy, and self-management ability in both groups were compared. Results After intervention, the disease cognition score, self-efficacy score scale (GSES) score, and heart failure self-care index scale (SCHFI) of both groups of patients increased compared with before intervention, and the differences were statistically significant(P<0.05). Moreover, the intervention group had a greater increase in the above indicators than the control group, and the differences were statistically significant(P<0.05). Conclusion Concept reconstruction cognitive nursing and Roy adaptation model nursing used in elderly CHF patients can enhance disease awareness, enhance self-efficacy and self-management ability.

    Objective To study and analyze the intervention effect of oral functional rehabilitation nursing on salivation in children with cerebral palsy. Method A total of 80 children with cerebral palsy and salivation admitted to Nanhai District Maternal and Child Health Hospital in Foshan City, Guangdong Province from August 2023 to February 2024 were randomly divided into an observation group (n=40, receiving oral function rehabilitation care) and a control group (n=40, receiving routine care). Compare the intervention effects, fine motor activity rate, developmental quotient, and swallowing function between the two groups. Result The total effective rate and fine activity rate of the observation group were higher than those of the control group (P<0.05); There was no difference (P>0.05) in the scores of the Gesell Infant Development Scale, Oral, Pharyngeal, and Esophageal Stages before intervention. After intervention, both groups showed changes in the above scores. The observation group had higher scores on the Gesell Infant Development Scale than the control group, while the DDS scores at each stage were lower than the control group (P<0.05). Conclusion Oral function rehabilitation nursing can promote the development of children with cerebral palsy who salivate, improve swallowing function, and improve fine motor ability.

    Objective To explore the effect of timing visceral acupoints on Yin deficiency constipation in peritoneal dialysis. Methods elect 60 patients with functional drainage disorders of peritoneal dialysis catheters who visited the Nephrology Department and Peritoneal Dialysis Center of a tertiary traditional Chinese medicine hospital in Changchun City as the research subjects. They were randomly divided into a control group (n=30) and an observation group (n=30) according to a random number table. The control group was given routine care, while the observation group was given timing acupoints on the basis of the control group. After 4 weeks of intervention, the Patient Assessment of Constipation Symptoms (PAC-SYM), clinic constipation score (CCS), patient assessment of constipation quality of life(PAC-QOL), and Connor-Davidson resilience scale (CD-RISC) were observed in both groups of patients. Result Before intervention, there was no statistically significant difference in the PAC-SYM, CCS, PAC-QOL, and CD-RISC scores between the two groups of patients (P>0.05). After 4 weeks of intervention, the PAC-SYM, CCS, PAC-QOL scores of the observation group decreased, while the CD-RISC scores increased, and the difference was statistically significant (P<0.05). Conclusion Timing acupoints on organs can alleviate the symptoms and severity of constipation in patients with peritoneal dialysis yin deficiency type constipation, improve their quality of life and psychological resilience.
