    Construction of Bladder Cancer Prognostic Model Based on Mast Cell-related Genes

    Objective Analyzing the relationship between mast cell-related genes and bladder cancer prognosis, screening prognostic key genes, and constructing a bladder cancer prognosis model. Methods Utilizing bulk RNA-seq data from mast cells, differential gene expression analysis was conducted followed by KEGG and GO enrichment analyses as well as Gene Set Enrichment Analysis (GSEA). Mast cell characteristic genes within bladder cancer single cells RNA sequencing were obtained from the literature. After identifying overlapping genes, key prognostic genes were selected using Lasso regression and multivariable COX regression. Based on the risk score, patients were stratified into high-risk and low-risk groups. Finally, through univariable and multivariable COX regression analyses, in conjunction with risk scores and multiple independent prognostic factors, a nomogram was developed to predict the survival rate of bladder cancer patients. Results The prognostic model comprises five genes: WDR45B, EI24, NCOR1, VEGFA, and RNF19A, effectively stratifying patients into high-risk and low-risk groups. Patients in the high-risk group exhibit significantly poorer survival prognosis compared to those in the low-risk group. The model's efficacy was successfully validated using the GSE31864 dataset. Additionally, the forest plot, constructed by integrating T/N staging, risk score, and age, demonstrates excellent predictive performance for bladder cancer patient survival prognosis. Conclusion This study proposes a prognostic risk model based on five mast cell-related genes in bladder cancer patients. It can assist clinicians in evaluating prognosis and providing personalized treatment recommendations for bladder cancer patients.

    Objective To understand the current status of self-management ability of patients after permanent pacemaker implantation, analyze the relevant influencing factors, and construct an intervention model by weighting the influencing factors. Methods A total of 210 patients who underwent permanent pacemaker implantation in the Department of Cardiology of a tertiary hospital in Xinjiang from January 2020 to January 2023 were selected as the study subjects, and the questionnaire method was used to collect the self-management ability of the patients in the 6 months after the operation, and the influencing factors of the patients' self-management ability after the operation were investigated by using univariate and logistic regression analysis. The intervention model was constructed according to the analysis results, and the feasibility of the model entries was verified by using ROC curves. Results The patients' self-management ability was generally poor (score of 60.23%); the results of univariate analysis showed that 9 factors, including age, disease duration, and literacy level, were the influencing factors of the patients' self-management ability after surgery; the results of logistic regression analysis showed that 6 factors, including the presence of complications, the literacy level of elementary school and below, and the absence of chronic diseases, were the independent factors affecting the low self-management ability. By analyzing the results of logistic regression risk factors (weights) and ROC curves, the preliminary model was established to improve the ability of "patients with complications,The intervention model of postoperative self-management ability of patients with "elementary school education or below, chronic disease history and disease duration ≤5 years" was constructed. Conclusion Constructing a self-management intervention model for patients after pacemaker implantation based on evidence-based concepts has good theoretical and practical value for the improvement of self-management ability of postoperative patients.

    Objective To investigate the inhibitory effect of Trillium tschonoskii Maxim N-butanol extract on diethylnitrosamine (DEN)-induced primary liver cancer in rats, explore its impact on the STAT3/VEGF signaling pathway, and study its molecular mechanisms against liver cancer angiogenesis. Methods Rats were randomly divided into five groups: G1 (normal control group), G2 (model group), G3 (silibinin group), G4 (Trillium tschonoskii Maxim low-dose group), and G5 (Trillium tschonoskii Maxim high-dose group). Except for the G1 group, all rats were intraperitoneally injected with DEN to establish a liver cancer rat model. Trillium tschonoskii Maxim N-butanol extract was used to treat liver cancer rats, and the liver index and spleen index of rats were measured. The levels of serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), albumin (ALB), and total bilirubin (TBIL) were determined. ELISA was used to detect the levels of serum transforming growth factor-beta 1 (TGF-β1) and vascular endothelial growth factor (VEGF). Western blot was used to detect the levels of VEGF, kinase insert domain receptor (KDR), STAT3, and phosphorylated STAT3 (p-STAT3) proteins. Results Compared with the G1 group, the levels of liver index, ALT, AST, and TBIL were significantly increased in the G2 group, while the level of ALB was significantly decreased. The levels of serum TGF-β1 and VEGF were significantly increased, and the protein levels of VEGF, KDR, STAT3, and p-STAT3 in liver tissues were significantly increased (P<0.05). Compared with the model group, the liver index of rats in the Trillium tschonoskii Maxim N-butanol extract group was significantly decreased, and the levels of serum AST and TBIL were significantly decreased. The levels of serum TGF-β1 and VEGF were significantly decreased, and the protein levels of VEGF, KDR, STAT3, and p-STAT3 in liver tissues were significantly decreased (P<0.05). Conclusion Trillium tschonoskii Maxim N-butanol extract exerts anti-liver cancer angiogenesis effects by inhibiting the STAT3/VEGF signaling pathway.

    Objective To explore the effects of Five-tone therapy on anxiety, depression and quality of life in snake bites. Methods A total of 106 snake bite patients admitted to the emergency department of a Grade A TCM hospital in Anhui province from January ,2022 to July ,2023 were selected and divided them into observation and control groups according to the random number table method, with 53 cases in each group. Both groups were given basic drug therapy and routine care, and the observation group took Five-tone therapy on this basis. Before treatment and after discharge were assessed using the Anxiety Self Assessment Scale (Self-Rating Anxiety Scale SAS) and Depression Self Assessment Scale (Self-rating depression scale, SDS) and the Dingham Health Questionnaire (nottingham health profile, NHP). Results The observed group was significantly lower than the control group in both SAS and SDS scores, the NHP score lower than the control group,and the difference was statistically significant (P<0.05). Conclusion The application of Five-tone therapy in the treatment of snake bite patients can help reduce negative emotions and promote the improvement of quality of life.

    Objective To observe the effect of acupuncture combined with acupuncture in patients with depression and chronic pain. Methods A total of 80 patients with depression and chronic pain admitted to the First Affiliated Hospital of Xinjiang Medical University from January 2020 to January 2023 were selected as the research subjects of this paper, Divided into study and control groups according to the randomization method, Of 40 patients in each group, Of which the control group was treated with paroxetine hydrochloride, The study group received acupuncture treatment combined with liver capsule on the basis of the control group, Compare the treatment effect of the two patient groups, The Pittsburgh Sleep Quality Index (PSQI) was used to evaluate patient sleep quality before and after the intervention, The Hamilton Depression Scale (HAMD), social function deficiency screening scale (SDSS) were used to evaluate patient depression status and social function before and after the intervention, And compared the quality of life and pain degree before and after the intervention. Results The treatment effectiveness rate of patients in the combination group was 97.50%, while the treatment effectiveness rate of patients in the control group was 80.00%. The response rate of the study group was higher than the control group, statistically significant (P<0.05); the difference of PSQI score between the two groups before treatment was not statistically significant(P>0.05). After 3 months of treatment, the PSQI scores decreased in both groups, and the PSQI score was significantly lower in the control group(P<0.05); the difference between HAMD and SDSS scores in the two groups before treatment(P>0.05).The HAMD and SDSS scores were lower in both groups after 3 months of treatment, and the study group was significantly lower(P<0.05); before treatment, the difference between BI and VAS scores between the two groups was not statistically significant(P>0.05). After 3 months of treatment, the BI score of the study group was significantly higher than the control group, and the VAS score was significantly lower than the control group(P<0.05). Conclusion In the treatment of depression with chronic pain, the combination of shu liver capsule and acupuncture can optimize the treatment effect, reduce the pain, depression and social function defects, and effectively improve the sleep and life quality of patients.

    Objective To explore and analyze the clinical efficacy and safety of Qingjin Huatan Decoction in the treatment of phlegm-heat stagnation lung type bacterial pneumonia. Methods Sixty patients with phlegm-heat pneumonic bacterial pneumonia treated in Department of respiratory medicine, Huishan Hospital of Traditional Chinese Medicine from January 2022 to December 2022 were selected and divided into control group and observation group by random number table method, with 30 cases in each group. The control group was treated with conventional western medicine, and the observation group was treated with Qingjin Huatan Decoction on the basis of the control group. The clinical efficacy, the time for the main symptoms to disappear after treatment and the laboratory indexes before and after treatment were observed. Results The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). The disappearance time of cough, sputum, fever, wheezing and pulmonary moist rales in observation group was shorter than that in control group, and the difference was statistically significant (P<0.05). After treatment, the leukocyte count, serum procalcitonin level and C-reactive protein level in the observation group were lower than those in the control group, and the differences were statistically significant (P<0.05). There were no obvious symptoms or changes in liver and kidney function indexes between the two groups before and after treatment, and there was only 1 case of nausea in the observation group. There was no statistically significant difference in adverse reactions between the control group and the observation group (P>0.05). Conclusion Qingjin Huatan Decoction has outstanding clinical efficacy in the treatment of phlegm-heat stagnation lung type bacterial pneumonia, which can effectively improve related symptoms and signs, improve laboratory indicators, shorten the time for the main symptoms to disappear, and have no obvious adverse reactions during treatment, with high safety.

    Objective To investigate the therapeutic effect of posterior cervical decompression surgery on ossification of the posterior longitudinal ligament in cervical spondylotic myelopathy. Methods A total of 96 patients with ossification of the posterior longitudinal ligament of cervical spondylotic myelopathy who underwent treatment in Dali County Maternal and Child Health Center from April 2020 to October 2022 were selected as the research subjects. They were randomly divided into the study group (n=48, who underwent posterior cervical decompression surgery) and the control group (n=48, who underwent traditional posterior cervical single door laminoplasty) using a random number table method. The surgical parameters such as incision length, operation time, fluoroscopy time and bleeding volume were compared between the two groups, Compare the differences in JOA scores, Nurick grading, cervical curvature, and spinal canal occupancy between the two groups of patients before and 6 months after surgery, and calculate the differences in the incidence of various complications between the two groups of patients at 6 months after surgery; Results The incision length, surgical time, fluoroscopy time, and bleeding volume in the study group were significantly lower than those in the control group (P<0.05); There was no statistically significant difference in JOA score, Nurick grade, cervical curvature, and spinal canal occupancy between the two groups of patients before surgery (P>0.05). At 6 months after surgery, the JOA score and cervical curvature of the study group were higher than those of the control group, while Nurick grade and spinal canal occupancy were lower than those of the control group (P<0.05). There was no statistically significant difference in the incidence of complications between the two groups (4.17% vs 14.58%) (P>0.05). Conclusion The implementation of posterior cervical cone plate decompression surgery for patients with ossification of the longitudinal ligament after cervical spondylotic myelopathy has a positive effect, which can effectively improve the curvature of the patient's cervical spine and the occupancy rate of the spinal canal. Compared with traditional posterior cervical single door laminoplasty, it has the advantages of less intraoperative trauma, faster postoperative recovery, and better manual safety.

    Objective To investigate the effect of volume management combined with intermittent catheterization on patients with spinal cord injury (SCI) complicated with Neurogenic Bladder (NB). Methods A total of 80 patients with SCI combined with NB admitted to Beijing Rehabilitation Hospital Affiliated to Capital Medical University from December 2020 to June 2023 were included as research objects, and were divided into control group and observation group by random number table method,with 40 cases in each group.The control group was given conventional nursing intervention for SCI patients combined with NB,and the observation group was given volume management combined with intermittent catheterization nursing intervention for 8 weeks.The safe bladder volume, residual urine volume,incidence of urinary tract infection,autonomic urination function grade and the generic quality of life inventoy-74 (GQOLI-74) were analyzed before and after intervention in the two groups to observe the effect of intervention. Results Before intervention,there were no significant differences in bladder safety volume,bladder residual urine volume,autonomic urination function grade and GQOLI-74 score between the two groups (allP>0.05).After intervention,the safe bladder volume,autonomic urination function grade and GQOLI-74 scores of both groups were improved,and the observation group was better than the control group.After intervention,the residual urinary volume of bladder in both groups was decreased compared with that before intervention,and the observation group was lower than the control group,the differences were significant(P<0.05).In addition,the incidence of urinary tract infection in the observation group was significantly lower than that in the control group during the intervention(P<0.05). Conclusion Volume management combined with intermittent catheterization can not only effectively improve the bladder function and autonomic urination function in patients with SCI and NB, but also help to improve the quality of life of patients and reduce the incidence of urinary tract infection, which is worth promoting.

    Objective To explore and analyze the application of staged lumbar motion chain training combined with rapid rehabilitation intervention in the treatment of lumbar disc herniation. Methods A random number table method was used to divide 108 patients with lumbar disc herniation admitted from March 2020 to May 2022 into groups, with 54 cases in each group. The control group received routine rehabilitation intervention, while the observation group received additional lumbar motion chain staging training. The visual analogue scale (VAS), ODI index, SF-36 scale score, multifidus muscle cross-sectional area, and gait results of the two groups before and after rehabilitation intervention were compared. Results Compared with the two groups before and after rehabilitation intervention, the VAS score, ODI index, SF-36 scale score, multifidus cross-section area and gait support comparison value were lower. Compared with the control group after rehabilitation intervention, the observation group had lower VAS score, lower ODI index, higher SF-36 scale score, higher multifidus cross-section area and higher gait support comparison value, and the differences were statistically significant (P<0.05). Conclusion On the basis of routine rehabilitation intervention, adding lumbar spine motion chain staging training to patients with lumbar disc herniation can effectively alleviate pain, improve limb dysfunction, and promote the recovery of lower limb function, improving their quality of life.

    Objective Explore the development trends and research hotspots in the field of research related to Wandai Tang in the past 30 years, and conduct bibliometric and visual analysis around Wandai Tang. Methods A bibliometric analysis was conducted on all relevant literature included in the China National Knowledge Infrastructure (CNKI) database from January 1993 to September 2023. Use CiteSpace 6.2.R4 software to conduct co-occurrence analysis of institutions, authors, and keywords in the literature, and draw relevant graphs. Results A total of 331 relevant literature were obtained, and high-yield authors and research institutions were identified through quantitative analysis. The visualization analysis results indicate that Hunan University of Traditional Chinese Medicine and Guangzhou University of Traditional Chinese Medicine are the institutions with the most publications, and You Zhaoling is the author with the most publications. According to keyword analysis, the current research hotspots around Wandai Tang are recurrent vulvovaginal candidiasis, literature research, vaginal microbiota, etc. Conclusion This study utilized Citespace software to analyze the current status and development trends of the research field of Wandai Tang, providing new ideas and references for the clinical application and further development of Wandai Tang.

    Objective To study the application of clinical nursing pathway in laparoscopic total extraperitoneal hernia repair (TEP) for inguinal hernia. Methods 98 cases of patients with inguinal hernia admitted to the Second People's Hospital of Huangmei County from May 2022 to July 2023 were selected as the study subjects, which were divided into control group and study group according to the random number table method, with 49 cases in each group.All the selected patients received TEP treatment,the postoperative control group was given routine care, and the research group gave the clinical care path. Comparing the postoperative clinical indexes, pain degree, complication rate and satisfaction degree of the two groups. Results Postoperative clinical indexes (time of anal exhaust, time out of bed and time in hospital) in the study group were shorter than those in the control group,statistically significant difference (P<0.05). The NRS score of the study group was lower than that of the control group, statistically significant difference (P<0.05). The total complication rate in the study group was lower than that in the control group, and it was significantly different (P<0.05). Nursing satisfaction was higher than the control group, significant (P<0.05). Conclusion The clinical nursing pathway can promote rehabilitation, relieve pain and prevent complications, and also has a positive effect on the satisfaction of patients.

    Objective To explore the effect of Internet combined PBL health education on psychological state and self-management ability in cirrhosis. Methods A total of 87 cirrhosis patients admitted from January 2022 to July 2023 were selected as study subjects into control group (44 patients) and observation group (43 patients). Patients in the control group received routine health education for cirrhosis and the observation group received internet combined with PBL health education. Two months after the intervention, evaluation on their psychological status, self-management ability and satisfaction was performed respectively. Results After the intervention, the self-assessment anxiety scores and the self-assessment depression scores were lower than the control group; the scores of self-management ability in each dimension were higher than the control group; the satisfaction scores were higher than the control group, and the difference was significant(P<0.05). Conclusion Internet combination and PBL health education can improve patients' psychological state and improve their self-management ability.

    Objective Exploring the effect of cloud follow-up on quality of life, pelvic floor function and patient satisfaction after total hysterectomy in patients of different income levels. Methods A total of 144 patients who underwent total hysterectomy from 1 January 2022 to 1 August 2022 were selected for the study, and were randomly divided into the observation group and the control group, and the patients in the observation group were divided into the high-income group and low-income group according to the different levels of income, patients in the control group were divided into high-income group and low-income group,and the control group was given routine discharge instructions and post-discharge telephone follow-up at the time of discharge. On the basis of the control group, cloud follow-up intervention was used to implement continuity of care. Compare the quality of life, pelvic floor function and patient satisfaction between the observation group and the control group, and between the high-income group and the low-income group 3 months after surgery. Results The quality of life and patient satisfaction scores of the observation group were higher than those of the control group (P<0.05), and the pelvic floor function scores of the observation group were lower than those of the control group (P<0.05), with statistically significant differences, and the differences in the quality of life and pelvic floor function scores of the high-income group and the low-income group were not statistically significant (P>0.05). Conclusion Cloud follow-up interventions play an important role in improving quality of life, pelvic floor function, and satisfaction in patients after total hysterectomy, and income level has no significant effect on patients' postoperative recovery.

    Chronic kidney disease (CKD) is a kind of insidious and slow-progressing disease caused by many causes. Its incidence is high and rising year by year.It has become an important public health problem in China. Traditional Chinese medicine has long been used in the treatment of chronic kidney disease, and its exact curative effect has been recognized by the majority of patients.     Objective To investigate the effect of Targeted therapy of epidermal growth factor Receptor tyrosine kinase inhibitor (EGFR-TKI) combined with TC chemotherapy on liver and kidney function in patients with Mutant non-small cell lung cancer. Methods A total of 100 patients with mutant non-small cell lung cancer admitted to Fuping County Hospital from June 2017 to June 2019 were selected as the study subjects for retrospective analysis. According to the treatment methods, the patients were divided into an observation group and a control group, with 50 cases in each group. The patients in the control group were treated with a single TC chemotherapy scheme, and the patients in the observation group were treated with EGFR-TKI Targeted therapy combined with TC chemotherapy scheme. The clinical efficacy, survival and changes of liver and kidney function indexes before and after treatment of the patients in the two groups were compared. Results The Objective remission rate and disease control rate of the observation group were significantly higher than those of the control group (P<0.05); The median PFS of the observation group was 11.26 (2.38-24.13), the median progression-free survival (PFS) of the control group was 9.25 (2.24-17.22), which was higher in the observation group than in the control group (χ2=4.536, P=0.041), and the median overall survival (OS) of the observation group was 21.23 (4.84-62.73). The median OS of the control group was 16.52 (5.27-50.27), which was higher in the observation group than in the control group (χ2=5.262, P=0.022); Before treatment, the levels of alt, AST, SCR and BUN in the two groups had no significant difference (P>0.05). After treatment, the levels of alt, AST, SCR and BUN in the two groups increased, but there was no significant difference between the observation group and the control group (P>0.05).Conclusion EGFR-TKI Targeted therapy combined with TC chemotherapy regimen has a significant effect in the treatment of mutant non-small cell lung cancer, and can improve the progression free survival period and total survival period of patients, and does not increase the degree of liver and kidney function damage compared with the single TC chemotherapy regimen.

    Objective To investigate the effect of ginkgolone ester drops combined with metoprolol in the treatment of heart failure in dilated cardiomyopathy and its effect on ventricular remodeling. Methods The case data of 120 patients with heart failure with dilated cardiomyopathy admitted to Inner Mongolia Autonomous Region People's Hospital were analyzed (January 2019 to December 2022), and divided into control group (n=60) and observation group (n=60) by random number table method. Both groups continued treatment for 6 weeks. Comparing the treatment effect, cardiac function [left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF)], inflammatory factor level [N-terminal brain natriuretic peptide precursor (NTpro-BNP), tumor necrosis factor α (TNF-α), hypersensitivity C-reactive protein (hs-CRP), ventricular remodeling function [serum matrix metalloproteinase-9 (MMP-9), type I precollagen amino-terminal peptide (PI.NP), type III precollagen amino-terminal peptide (PIII.NP)] and the occurrence of adverse effects. Results The total effective rate of treatment in the observation group was significantly higher than that in the control group, and the difference was statistically significant (P<0.05). After treatment, the LVEDD level in both groups was significantly reduced, and the decrease was greater in the observation group than in the two groups, with a statistically significant difference (P<0.05), and the LVEF level in both groups was significantly increased, and the increase in the observation group was greater than that in the two groups, and the difference was statistically significant (P<0.05). After treatment, the levels of NTpro-BNP, TNF-α and hs-CRP were reduced in both groups, and the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). After treatment, the levels of PI.NP, PIII.NP and MMP-9 were reduced in both groups, and the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Ginkgolone ester drops combined with metoprolol have a significant effect in the treatment of dilated cardiomyopathy heart failure, which can improve cardiac function, reduce inflammatory response and inhibit ventricular remodeling.
    Transurethral Holmium Laser Resection of Bladder Tumors for Non Muscle Invasive Bladder Cancer

    Objective To investigate the efficacy of transurethral holmium laser resection of bladder tumors in patients with non muscle invasive bladder cancer, and analyze the factors affecting postoperative recurrence. Methods A total of 100 patients with non muscle invasive bladder cancer who received treatment in Bortala Mongol Autonomous Prefecture People's Hospital were screened out. The relevant medical data were collected from May 2017 to May 2021, and were retrospectively analyzed. They were grouped according to the operation mode. 50 patients were set as the observation group and underwent transurethral holmium laser resection of bladder tumors. The other 50 patients were included in the control group and underwent transurethral plasma resection of bladder tumors. Perioperative indicators, complications and recurrence rates of the two groups were compared. Subsequently, the patients were divided into two subgroups based on their recurrence status within 2 years, namely the recurrence group (n=19) and the non recurrence group (n=81), and the influencing factors of recurrence were analyzed. Results There was no significant difference in surgical time between the two groups of patients (P>0.05). The observation group had significantly lower urinary catheter indwelling and postoperative hospital stay, incidence of complications, and recurrence rate compared to the control group (P<0.05); There were significant differences in TNM staging, surgical methods, degree of tissue differentiation, and number of tumors between the non recurrent group and the recurrent group (P<0.05); The results of Logistic regression analysis showed that TNM stage, operation mode and tissue differentiation were independent influencing factors for postoperative recurrence of non muscle invasive bladder cancer (P<0.05). Conclusion For patients with nonmuscular invasive bladder cancer, transurethral holmium laser resection of bladder tumor has a lower degree of injury, can promote early recovery, and can reduce the incidence of complications and recurrence. In addition, TNM staging, operation mode and tissue differentiation degree are independent factors affecting the recurrence of non muscle invasive bladder cancer after surgery. Therefore, more attention should be paid to patients with low TNM staging and tissue differentiation, and transurethral holmium laser resection of bladder tumors should be selected as far as possible.
