    Study on the Clinical Effect and Mechanism of Moxibustion on the Du Meridian on Functional Constipation Due to Deficiency of Qi

    Objective Study the role and mechanism of Du moxibustion in the treatment of functional constipation due to qi deficiency. Methods The cases were sourced from the Outpatient Department of Traditional Chinese Medicine Prevention and Health Care of Tai'an City Traditional Chinese Medicine Hospital in Shandong Province. The treatment period was from September 2022 to September 2023, and 120 patients were diagnosed with Qi deficiency functional constipation. Divide patients into Du moxibustion group, Western medicine group, and Chinese medicine group according to a the random number table method. Du moxibustion group: Du moxibustion once every 10 days, 3 times per course, a total of 2 courses and 6 times of Du moxibustion. Western medicine group: Lactulose oral solution, 10 mL/time, 3 times/day, taken for 8 days with 2 days of interruption, a course of treatment per month for 2 months. Traditional Chinese Medicine Group: Take Sijunzi Tang orally, once in the morning and once in the evening, and take 200 mL warm after meals, using the same method as the Western Medicine Group.Observation indicators: Compare the clinical efficacy of three groups,before and after treatment, the conversion score of Qi deficiency in the three groups, the total score of the main and secondary symptoms of Qi deficiency constipation in patients with traditional Chinese medicine, and the laboratory observation indicators: serum motilin (MTL) and serum substance P (SP) levels. Results After the completion of two treatment courses, the total effective rate was 87.50% in the moxibustion group, 72.50% in the western medicine group, and 75.00% in the traditional Chinese medicine group. The score of Qi deficiency quality significantly decreased in the Du moxibustion group (P<0.05), while there was no statistical significant difference between the Western medicine group and the Chinese medicine group before and after treatment (P>0.05); The total scores of the main and secondary symptoms of traditional Chinese medicine constipation in the Du moxibustion group, Western medicine group, and Chinese medicine group were significantly reduced compared to before treatment (P<0.05), and the levels of serum motilin (MTL) and substance P (SP) were significantly increased compared to before treatment (P<0.05); Compared between groups, the conversion score of Qi deficiency and substance, as well as the total score of main and secondary symptoms of traditional Chinese medicine constipation in the Du moxibustion group were significantly lower than those in the Western medicine group and the Chinese medicine group (P<0.05), and the serum levels of motilin (MTL) and substance P (SP) were higher than those in the Western medicine group and the Chinese medicine group; There was no statistical significant difference between the Western medicine group and the Chinese medicine group in terms of the transformation score of Qi deficiency and substance, the total score of the main and secondary symptoms of traditional Chinese medicine constipation, and the changes in serum motilin (MTL) and substance P (SP) levels. Conclusion The Du moxibustion group, the Western medicine group, and the Chinese medicine group all have therapeutic effects on Qi deficiency functional constipation. However, the Du moxibustion group can also correct the constitutional bias of patients with Qi deficiency functional constipation, improve the internal environment of the human body, and further increase serum levels of motilin (MTL) and substance P (SP). Correcting the biased state of qi deficiency is the target for curing qi deficiency constipation from the root cause.

    Objective To compare the treatment effectiveness of two kinds of Vanbeek activators with different load-points of extraoral force in the treatment of children’s skeletal class Ⅱ malocclusion. Methods Thirty cases were included from January 2019 to December 2020. In the experimental group, improved Vanbeek activators with facebows extending between central and lateral incisors were used. In the control group, conventional Vanbeek activators with facebows extending between first and second premolars were used. Pretreatment and post-treatment lateral cephalometric radiographs of patients were collected for measurement. Results After treatment, the MP-FH (P=0.014), ANS-Me/Ar-Go ratio (P=0.042) and OP-FH (P=0.013) in the experimental group decreased significantly than the ones in the control group. between two groups in the change of SNA, SNB, ANB, Y axis angle, ANS-Me/N-Me ratio, U1-SN, and L1-MP, the difference is not statistically significant(P>0.05). Conclusion The load-point of improved Vanbeek activator was in front of the resistance centers of upper arch and maxilla, which led to counter clockwise rotation of jaws and was effective in vertical control in moderate to high angle children with skeletal class Ⅱ malocclusion.

    Objective To understand the relationship between marital status and lumbar disc herniation in adult residents in Gansu Province. Methods From May to July 2022, a total of 4598 people were surveyed in Gansu Province by stratified cluster sampling according to altitude (≥3500 meters, 1500~3500 meters, <1500 meters). All subjects were investigated by questionnaire, physical examination, blood glucose and blood lipid test. Univariate analysis and unconditional binary logistic regression model were used to analyze the correlation between marital status and lumbar disc herniation. Results The final effective sample of this survey was 4545 cases. The prevalence of lumbar disc herniation in spouses was 24.49% higher than that in non-spouses, and the difference was statistically significant (P<0.05). Among those with spouses, the prevalence of lumbar disc herniation was higher in women than in men, higher in the <45 age group than in other age groups, and higher in Tibetans than in other ethnic groups, with statistically significant differences (P<0.05). The results of binary logistic regression analysis showed that female, age ≥45 years old, agriculture, forestry, animal husbandry and fishery, workers, other occupations, living altitude ≥1500 meters and hypertension were risk factors for lumbar disc herniation (OR>1). Conclusions The status of spouses of adult residents in Gansu Province is associated with the prevalence of lumbar disc herniation, and the prevalence of spouses is relatively high.

    Objective To explore and analysis the effects and mechanism of Shenling Baizhu Powder on intestinal mucosal channel proteins in a young mouse model of diarrhea. Methods Young rats with diarrhea (n=42) were randomly equally divided into three groups - model group, low-dose Shenling Baizhu Powder group, high-dose Shenling Baizhu Powder group. The three groups were given Shenling Baizhu Powder 0.0 g/kg(i.e. 0.9% sodium chloride solution), 1.2 g/kg, 4.6 g/kg by gavage, with the dose volume of 10 mL/kg, for 14 consecutive days. The expression of intestinal mucosal channel AQP8, CFTR in transmembrane transport protein NHE3 were detected. Results The body weight of young mice in the low-dose Shenling Baizhu Powder group and high-dose Shenling Baizhu Powder group on the 7th and 14th day of treatment were significantly higher than that in the model group (P<0.05), and the diarrhea index were significantly reduced compared to the model group (P<0.05). There were also a statistical significant difference between the high-dose Shenling Baizhu Powder group and the low-dose Shenling Baizhu Powder group (P<0.05). The ratio of urine Lactulose and Mannitol in the low-dose and high-dose Shenling Baizhu Powder groups were significantly lower than that in the model group on the 7th and 14th days of treatment (P<0.05), and the high-dose Shenling Baizhu Powder group were also lower than that in the low-dose Shenling Baizhu Powder group (P<0.05). On the 14th day of treatment, the villus length of small intestine tissue in the low-dose and high-dose Shenling Baizhu Powder groups were higher than that in the model group (P<0.05), and the crypt depth were significantly lower than that in the model group (P<0.05). There were also statistical significant difference compared between the high-dose and low-dose Shenling Baizhu Powder groups(P<0.05). On the 14th day of treatment, the relative expression levels of AQP8 protein and NHE3 protein in the small intestine tissue of the low-dose and high-dose Shenling Baizhu Powder groups were higher than those of the model group (P<0.05), while the relative expression levels of CFTR protein were lower than those of the model group (P<0.05). There were also significant difference compared between the low-dose and high-dose Shenling Baizhu Powder groups (P<0.05). Conclusion Shenling Baizhu Powder can promote the recovery of body weight, reduce the ratio of Lactulose and Mannitol in urine and diarrhea index, prolong the villus length and reduce the recess depth of small intestine, promote the expression of AQP8 protein and NHE3 protein in small intestine, and inhibit the expression of CFTR protein.

    Objective To explore and analysis the effects of ginger separated purulent moxibustion and acupoint application on traditional Chinese medicine scores and CAT scores in the treatment of COPD. Methods From January 2017 to December 2019, 74 caes of stable COPD patients who underwent treatment at the Rehabilitation Department of Kangda Hospital in Siyang, Jiangsu Province were selected as the study subjects. Accorded to the 1:1 random envelope lottery principle, 74 patients were divided into the study group of 37 cases and the control group of 37 cases. The control group were treated with acupoint application, while the study group were treated with ginger separated purulent moxibustion. The treatment observation time for both groups of patients were 3 years, Determine and record the patient's Traditional Chinese Medicine score, and calculate the total effective rate based on the TCM symptom score, CAT scores, and changes in annual colds, while also recording the patient's lung function. Results After 3 years of treatment, the total effective rate of the study group were 97.30%, while that of the control group were 78.38%. The total effective rates of the study group after 3 years of treatment were significantly higher than that of the control group (P<0.05). The forced Vital capacity and forced breathing volume in the first second of the two groups after 3 years of treatment were higher than those before treatment (P<0.05), and the forced Vital capacity and forced breathing volume in the first second of the study group after 3 years of treatment were also significantly higher than those of the control group (P<0.05). The CAT symptom scores of both groups after 3 years of treatment were lower than before treatment (P<0.05), and the CAT symptom scores of the study group after 3 years of treatment were significantly lower than those of the control group (P<0.05). The annual number of colds in both groups after 3 years of treatment were lower than before treatment (P<0.05), and the annual number of colds in the study group after 3 years of treatment were also significantly reduced compared to the control group (P<0.05). Conclusion Ginger separated purulent moxibustion treatment for COPD can reduce patients' traditional Chinese medicine scores, improve lung function and overall efficacy, reduce patients' CAT symptom scores, and reduce the number of colds per year. It has a good application effect.

    Objective To investigate the efficacy of Shengxuebao mixture in the treatment of anemia during pregnancy and its effects on red blood cell count (RBC), hemoglobin (Hb), mean red blood cell volume (MCV) and serum ferritin (FER) levels. Methods A total of 80 cases of anemia during pregnancy from May 2020 to August 2022 were randomly divided into two groups, 40 cases in each group. The control group was treated with ferrous fumarate, and the experimental group was treated with Shengxuebao mixture. RBC, Hb, MCV and FER levels were detected before and after treatment to evaluate clinical efficacy and safety, and adverse perinatal outcomes were recorded. Results The levels of Hb, RBC, MCV and FER in the two groups were significantly increased after treatment (P<0.05), and the levels of Hb, RBC, MCV and FER in the experimental group were significantly higher than those in the control group after treatment (P<0.05). The clinical effect of experimental group was significantly better than that of control group (P<0.05). There were no significant difference in the incidence of fetal developmental delay, fetal distress, premature delivery, low birth weight and puerperal infection between the two groups (P>0.05). The incidence of adverse reactions in experimental group was 5.00% and 22.50% in control group, and the incidence of adverse reactions in experimental group was significantly lower than that in control group (P<0.05). Conclusion Shengxuebao mixture can improve clinical efficacy and reduce gastrointestinal symptoms caused by iron treatment,and had no effect on maternal and infant outcomes,. It is safe and effective.

    Objective To study the significance of serum carcinoembryonic antigen (CEA) and procalcitonin (PCT) levels before and after radical surgery for medullary thyroid carcinoma (MTC). Methods The clinicopathological data and follow-up data of 80 MTC patients who underwent radical surgery in our hospital from January 2020 to January 2023 were retrospectively analyzed, and the relationship between serum CEA and PCT levels and preoperative pathological characteristics was analyzed. The predictive efficacy of CEA and PCT for MTC lymph node metastasis was analyzed by receiver operating characteristic curve (ROC). According to the postoperative follow-up, patients were divided into cure group and recurrence/metastasis group, and the serum CEA and PCT levels of the two groups were compared in March and June after operation. Results There was no significant difference in serum CEA and PCT levels between patients of different sexes and ages before operation (P>0.05), but there were significant differences between patients with single lesion and multiple lesions, tumors < 2 cm and ≥2cm, and patients with and without lymph node metastasis (P<0.05). The cutoff value of preoperative serum CEA for predicting MTC lymph node metastasis was 35.75 ng/mL, with sensitivity of 84.61% and specificity of 72.53%. The cutoff value of PCT for predicting MTC lymph node metastasis is 3.65ng/mL, with sensitivity of 86.52% and specificity of 80.77%. Follow-up showed that the serum CEA and PCT levels in patients with recurrence/progression were higher than those in patients with cure at 3 months and 6 months after operation, and the difference was statistically significant (P<0.05). Conclusion Preoperative serum CEA and PCT levels can better reflect the tumor size and the number of lesions in MTC patients, and have a good predictive ability for lymph node metastasis, while postoperative serum CEA and PCT levels can better reflect the progress of the disease.

    Objective To explore the predictive value of the expression of serum abdominal fat Serpin (Vaspin) and Cathelicidin cathelicidin (LL-37) on the early changes of renal function in children with diabetes. Methods Eighty hospitalized children with diabetes admitted to the First People's Hospital of Zunyi City from July 2020 to July 2023 were selected as the study subjects. They were divided into normal group (urinary protein<30 g/g Cr, n=31), micro group (urinary protein 30~300 g/g Cr, n=24), and large group (urinary protein>300 g/g Cr, n=25) according to their urinary protein level. Compare the expression levels of serum Vaspin and cathelicin in three groups of children, compare the routine renal function injury indicators of uric acid (UA), urea nitrogen (BUN), blood creatinine (Scr), and cystatin C (CysC) in three groups of children, analyze the correlation between serum Vaspin and cathelicin with UA, BUN, Scr, and CysC levels, and establish a subject characteristic (ROC) working curve to analyze Vaspin The critical value and diagnostic efficacy of cathelicidin in the diagnosis of early renal function changes in children with diabetes. Results There were significant differences in the expression levels of Vaspin and cathelicin among the three groups of children. The sensitivity of combined diagnosis of early renal function changes in children with diabetes was 93.15%, and the specificity was 88.34%, higher than that of single diagnosis. Conclusion Vaspin combined with cathelicidin can improve the diagnostic efficacy of early renal function changes in children with diabetes. For Vaspin>7.97 ng/mL, cathelicidin<22.14 μg/L and take relevant measures for intervention in a timely manner.     Objective Evaluate the value of the combination of Carolizumab and Stereotactic Body Radiotherapy (SBRT) on survival and treatment safety in the non-surgical treatment of advanced lung cancer. Methods Retrospective analysis was conducted on 300 patients with advanced non-surgical treatment of lung cancer admitted to the Radiotherapy Department of Chifeng City Hospital from April 2020 to October 2022. According to different clinical treatment methods, they were divided into a study group (150 cases, treated with SBRT) and a control group (150 cases, treated with intravenous injection of Carolizumab every three weeks, a total of four times). Both groups were treated for 3 courses (21 days per course). Collect two sets of clinical information to compare treatment efficacy, changes in inflammatory factors (IL-6, IL-10), tumor necrosis factor (TNF-a), vascular endothelial factors (bFGF, Ang-2, VEGF, MMP-9) before and after treatment, immune status (CD4+, CD8+, NK), and adverse reactions during treatment. Results The treatment effectiveness rate of the research group was higher than that of the control group, with lower levels of IL-6 and IL-10, and higher levels of TNF-a than the control group (P<0.05); The levels of bFGF, Ang-2, VEGF, and MMP-9 in the study group were lower than those in the control group (P<0.05); After intervention, the immune T cell subpopulations CD4+, CD8+, and NK in the study group were higher than those in the control group (P<0.05); There was no statistically significant difference in adverse reactions between the two groups (P>0.05). Conclusion Karelizumab combined with SBRT in the treatment of advanced non-surgical lung cancer can improve clinical treatment efficacy, improve immune status, suppress tumor related inflammatory reactions, and reduce the incidence of adverse reactions during treatment. It has high clinical application value.

    Objective To explore the diagnostic value of postpartum three-dimensional ultrasound of pelvic floor combined with pelvic floor muscle tension assessment technology for stress Urinary incontinence.     Objective To investigate the effect of sacubitril/valsartan (ARNI) combined with dapagliflozin(SGLT2i) on cardiac function and prognosis after emergency percutaneous coronary intervention (PCI) in patients with acute ST-segment elevation myocardial infarction (STEMI). Methods A total of 140 eligible patients with STEMI who received emergency PCI were enrolled in the study and randomly assigned to receive treatment from January 2019 to December 2020. The patients presenting with left ventricular ejection fraction (LVEF)≥40% and NT-proBNP≥600 ng/L who were continuously enrolled. The patients were randomly divided into combination therapy group, ARNI group and ARB group. The data changes of NT proBNP, left ventricle ejection fraction (LVEF), left ventricular end diastolic diameter (LVEDD), left ventricular end systolic diameter (LVESD) and left ventricular fractional shortening (FS) before and after administration were taken as the primary endpoint. Follow up to June 2021, the occurrence of major adverse cardiovascular events (MACE) will be the secondary end point of this trial. Results There was no significant difference in clinical treatment data between the three groups. Compared with the ARB group, the decrease of NT-proBNP in the combination treatment group was more obvious, and the increase in LVEF and the reduction of LVEDD, LVESD were more obvious(P<0.05). A total of 23 patients developed MACE during follow-up, and there was no significant difference in the incidence of MACE between the three groups (P>0.05). Multivariate Cox regression analysis showed that the degree of lesion, intraoperative use of tirofiban and dopamine were independent risk factors for MACE (P<0.05). Conclusion Sacubitril/valsartan combined with dapagliflozin can more effectively reduce NT-proBNP levels, which may further increase the LVEF and reduce LVEDD, and improve the structure and function of the heart. Lesion complexity, intraoperative use of tirofiban, and dopamine are suggestive of a poor prognosis.

    Objective To investigate the effect of electromyobiofeedback therapy combined with transcranial magnetic stimulation (rTMS) on hand dysfunction in stroke patients. Methods A total of 84 stroke patients diagnosed in China Rehabilitation Research Center from October 2022 to October 2023 were selected and divided into combined group (n=42) and electromyography group (n=42) according to different treatment methods. Both groups underwent routine stroke drug therapy and rehabilitation training. The EMG group and the combined group were combined with rTMS (1Hz on the healthy side, 10Hz stimulation on the affected side). EMG values, upper limb motor function (FMA-UE), activities of daily life (MBI), upper limb and hand Brunnstrom stages, and wrist active back extension mobility (AROM) scores were compared between stroke patients before and 1 month and 2 months after treatment. Results After 1 month and 2 months, the combined group had higher EMG values than the EMG group(P<0.05); after 1 month and 2 months, the FMA-UE and MBI of the combined group scores were higher than the EMG group (P<0.05); after 1 month and 2 months, the Brunnstrom stage and AROM of the combined group were higher than the EMG group(P<0.05). Conclusion EMG biofeedback therapy combined with rTMS can improve upper limb motor function and wrist mobility, and improve daily living activities in stroke patients.

    Objective Study on the cognition status of doctors' diagnosis and treatment of gout in the outer suburbs of Pudong,Provide targeted gout management mode and re education content. Methods General practitioners in 12 community hospitals in the outer suburbs of Pudong were surveyed with questionnaires about gout diagnosis and treatment,The differences of doctors on gout concept, treatment plan and diagnosis and treatment norms were analyzed by factors such as professional title and education background. Results Among the general practitioners surveyed in the suburban areas,84.28% had bachelor degree or above, 85.53% had intermediate professional title or above, 51.57% were 41-50 years old, and only 3.78% were doctors under 30 years old. Attention should be paid to the construction of talent echelon.There is no difference in the concept of gout among doctors with different professional titles, but there are differences in the diagnosis and treatment norms among doctors with different educational backgrounds (P<0.05). The cases in the outer suburbs and rural areas are rich, but 50.31% of the doctors recommended that hyperuricemia patients choose long-term uric acid lowering treatment.Due to the differences in medical facilities and drug allocation,there are shortcomings in the standardization of treatment scheme. Conclusion At present,general practitioners in the outer suburbs of Pudong have a high understanding of the basic concepts and diagnosis and treatment norms of gout, but there are deficiencies in the drug treatment of gout.     Objective To explore the effect of rehabilitation nursing on swallowing function and quality of life in postoperative patients with laryngeal carcinoma. Methods A total of 62 patients with laryngeal carcinoma, who underwent partial laryngectomy in the Facial Sense Center of Weifang Hospital of Traditional Chinese medicine from January 2019 to October 2023 were randomly into the observation group and the control group, 31 cases in each group, the control group were given routine nursing, the observation group were given rehabilitation nursing. The emotional status, quality of life and swallowing function were assessed with MSSNS, Qlq-c30 and SSA before and after intervention, and compare the two groups of patients with complications. Results : after nursing, the scores of MSSNS, Qlq-c30 and SSA in the observation group were higher than those in the control group, and the incidence of complications in the observation group was lower than that in the control group, the difference was significant (P<0.05). Conclusion The application of Rehabilitation Nursing in partial laryngectomy can improve the patients' psychological status, quality of life and swallowing function, and reduce the incidence of postoperative complications.

    Objective To investigate the effect of touch nursing combined with ear canal care in newborn hearing screening on the accuracy of screening results. Methods A total of 112 newborns born in Weifang Maternal and Child Health Hospital from August 2022 to April 2023 were randomly divided into 56 cases in the control group and 56 cases in the observation group. The control group was given routine ear canal care, and the observation group was combined with touch care on the basis of the control group. The results of the first screening and re-screening of newborns' hearing test, the excellent rate of newborns' status during the test, the time from emotional agitation to stabilization during the test, and the scores of Newborn Infant Pain Scale (NIPS) were compared between the two groups. Results The first and second screening pass rates, as well as the good condition rate during the testing process of the newborns in the observation group with touch combined with ear canal care were higher than those in the control group with single ear canal care, and the difference was statistically significant (P<0.05), and the time of emotional stabilization and the NIPS scores of the newborns in the observation group were lower than those in the control group (P<0.05). Conclusion Touch nursing combined with ear canal care can effectively appease the emotional state of newborns during hearing screening, reduce the false-positive rate of newborn hearing screening results, and improve the accuracy of hearing screening.

    Objective To explore and analysis the values of reproductive health education interventions for women of childbearing age in enhancing their self-protection awareness. Methods From September 2019 to December 2022, 120 cases of women of reproductive age who underwent reproductive intervention at Yulin Maternal and Child Health Hospital were selected as the research subjects. Accorded to a 1:1 random number table method, 120 cases women were divided into the study group of 60 cases and the traditional group of 60 cases. The traditional group were received routine intervention measures, while the study group received reproductive health education intervention on the basis of the traditional group. The observation period for the two groups were 6 months, recorded and compared the reproductive health cognitive scores, proportion of healthy behaviors, satisfaction, and quality of life scores between the two groups. Results The reproductive health cognitive scores of both groups after 6 months of intervention were higher than those before intervention (P<0.05), and the reproductive health cognitive scores of the study group after 6 months of intervention were also significantly improved compared to the travditiond group (P<0.05). After 6 months of intervention, the proportion of regular cleaning of the vagina and vulva, attention to menstrual hygiene, reasonable handling of abnormal vaginal discharge, and healthy sexual behavior in the study group were 98.33%, 95.00%, 95.00%, and 98.33%, which were significantly higher than the traditional group's 75.00%, 73.33%, 70.00%, and 68.33% (P<0.05). After 6 months of intervention, the satisfaction rate with intervention in the study group were 100.00%, compared to 88.33% in the traditional group. The study group showed that there were significant improvement compared to the traditional group (P<0.05). After 6 months of intervention, the study group's quality of life scores such as physical function, psychological function, social function, and material life were significantly higher than those of the traditional group (P<0.05). Conclusion Reproductive health education intervention for women of childbearing age can enhance their self-protection awareness, improve reproductive health cognitive scores, increase intervention satisfaction, and also improve the quality of life of women of childbearing age.

    Objective To analyze the diagnostic value of mammography combined with Doppler ultrasound in the screening of early breast cancer precancerous lesions. Methods From May 2020 to May 2023,120 patients of highly suspected breast cancer were selected. All the patients were diagnosed with Doppler ultrasound examination. This paper took pathological results as the gold standard, and the diagnostic accuracy, specificity and sensitivity were calculated according to different examination methods. Results 120 patients were diagnosed as 100 positive and 20 negative by pathological examination. Ultrasound examination was positive in 88 cases and negative in 32 cases. There were 95 cases with positive mammography and 25 cases with negative mammography. 98 cases were positive in the joint examination, and 22 cases were negative. The diagnostic accuracy of combined examination is 93.33%, sensitivity is 95.00%, and specificity is 85.00%; The diagnostic accuracy of ultrasound examination is 76.67%, the sensitivity is 80.00%, and the specificity is 60.00%; The diagnostic accuracy of breast molybdenum target examination is 87.50%, the sensitivity is 90.00%, and the specificity is 75.00%. The combined ultrasound examination has a significant advantage in the diagnostic results, and the difference in results is statistically significant (P<0.05).     Objective To explore and analyze the clinical efficacy of sodium zirconate cyclosilicate powder combined with traditional potassium lowering therapy in the treatment of hyperkalemia in patients with chronic renal failure. Methods 70 patients of chronic renal failure with hyperkalemia admitted to Qianxi People's Hospital from January 2021 to August 2023 were selected as the study subjects, and divided into control group and observation group by random number table method, with 35 cases in each group.The control group was treated with traditional potassium lowering regimen, while the observation group was treated with sodium zirconium silicate powder combined with traditional potassium lowering regimen. The two groups of patients were compared before treatment and after 24 hours of treatment The blood potassium level, serum creatinine, urea nitrogen and other renal function indicators at 48 hours, the incidence of adverse reactions during treatment, and the total effective rate of treatment. Results There was no statistically significant difference in blood potassium levels between the two groups before treatment(P>0.05). At 24 and 48 hours of treatment, the observation group was lower than the control group(P<0.05); There was no statistically significant difference in the levels of serum creatinine, urea nitrogen and other renal function indicators between the two groups before treatment(P>0.05).     Objective Toinvestigate the clinical effect of primary open reduction plate internal fixation in the treatment of Sanders type IV calcaneal fracture with bone grafting and without bone grafting. Methods 42 cases of Sanders type IV calcaneal fracture treated from January 2020 to September 2021 were reviewed, all of which underwent open reduction and internal fixation in one stage. They were divided into two groups according to whether bone grafting was performed during the operation, with 21 cases in each group. The control group did not have bone grafting while the study group had bone grafting. Preoperative and postoperative AOFAS scores of Bohler Angle, Gissane Angle, ankle and hind foot. Results There were no significant differences in AOFAS scores of calcaneus Bohler Angle, Gissane Angle and ankle and hind foot between the two groups before and 3 months after surgery. One year after operation, the AOFAS scores of Bohler Angle, Gissane Angle, ankle and hind foot of the experimental group were higher than those of the control group.     Objective To explore the application effect of Ketamine+Dexmedetomidine+Nicardipine in the anesthesia of hip Joint replacement for elderly patients with hypertension. Methods 80 elderly patients with hypertension undergoing hip replacement surgery admitted to Yanqi Hospital of the Second Division of Xinjiang Production and Construction Corps from June 2021 to May 2023 were selected for research and analysis.They were divided into control group (n=30), observation group A (n=25), and observation group B (n=25) based on different anesthesia methods. All patients underwent routine general anesthesia induction, while the control group received nicardipine continuous intravenous infusion of 0.5 μg/kg • min before induction, observe group A using nicardipine continuous intravenous infusion of 0.5 μg/kg • min and 0.5 μg/kg dexmedetomidine intravenously pumped for 10 minutes before induction, followed by maintenance of continuously pump at a speed of 0.4 μg/kg • h, observe Group B, in observation group B, in addition to the measures taken in observation group A, 0.35mg/kg of Ketamine was added during induction, and maintained at a rate of 0.3~0.4mg/kg · h. HR, DBP, and SBP in Group B were lower than those in Group A and the control group at T2 and T3 time (P<0.05); There was no significant difference in surgical time and postoperative recovery time among the three groups of patients (P>0.05).     Objective To explore and analyze the effect of doxofylline on the number of eosinophils in blood during acute exacerbation of chronic bronchitis. Methods From January 2017 to December 2022,82 patients with acute attack of chronic bronchitis treated in the Fifth People's Hospital of Wujiang District, Suzhou were selected as the study subjects. According to a 1:1 random number table method, the patients were divided into the observation group of 41 cases and the control group of 41 cases. Both groups were treated with expectorant, anti infection. The control group were treated with compound Ipratropium bromide, while the observation group were treated with doxofylline on top of the control group. Both the doxophylline group and the control group were treated for 7 d to detect the total response rate of patients after 7 d of treatment, the duration of disappearance time of dyspnea, cough, sputum, lung rales, the count of FEV1/ FVC, the level of FEV1 and peripheral blood acidophilia granulocytic content were compared before and 7 d after treatment in each group and between the groups. Results After 7 days of treatment, the total effective rate of the study group was higher than that of the control group, The difference is statistically significant (P<0.05). The duration of disappearance of symptoms such as dyspnea, cough and phlegm, and lung rales in the observation group were significantly reduced compared to the control group (P<0.05). The percentage of forced expiratory volume (FEV) in the first second to forced vital capacity( FEV1/ FVC) and FEV1 in both groups after 7 days of treatment were significantly higher than those before treatment (P<0.05). The FEV1/ FVC and forced expiratory volume in the first second in the observation group after 7 days of treatment were significantly higher than those in the control group (P<0.05). The content of eosinophils in the peripheral blood of both groups after 7 days of treatment were significantly lower than before treatment (P<0.05), while the content of eosinophils in the peripheral blood of the observation group after 7 days of treatment were significantly lower than that of the control group (P<0.05). Conclusion The application of doxofylline in acute exacerbation of chronic bronchitis can promote the improvement of clinical symptoms, improve treatment effectiveness, improve lung function, and reduce the content of eosinophils in the patient's peripheral blood.
