    Observation on the Curative Effect of Mini Plate Fixation Combined with Surgical-Shujin Lotion Washing Fumigation in the Treatment of Metacarpal Fracture

    Objective To explore the clinical efficacy of internal fixation of miniature steel plate combined with ‘surgical shujin washing’ fumigation in the treatment of metacarpal fracture. Methods Select January 2020-202365 patients with metacarpal fracture admitted to Heyuan Hospital of Traditional Chinese Medicine from January 2020 to January 2023 were selected as the study objects, and all of them were treated with microplate fixation after surgery. Random number table method grouping, control group (33 cases), observation group (32 cases, add surgical shujin wash fumigation treatment), 3 months after the intervention, compare the fracture healing effects, hand function recovery effects, and quality of life scores between two groups of patients. Results The FMA hand function scores of both groups were significantly higher after treatment, and the observation group was higher than the control group, the TCM evidence scores were lower, and the observation group was lower than the control group, and the difference was statistically significant(P<0.05), the fracture healing time and the time for TAM to return to the normal range of motion of the observation group was shorter than that of the control group (P<0.05), at the end of intervention, PSQI score of observation group was lower than that of control group, and ESCA score was higher than that of control group(P<0.05). Conclusion After internal fixation of metacarpal fracture with microplates, the addition of ‘Surgical Shujin Wash’ fumigation can accelerate the speed of fracture recovery, promote the healing of fracture foci, control the incidence of postoperative complications, and ensure the recovery of the metacarpal bone, which is of value for popularisation.

    Objective To explore the feasibility and safety of early ambulation in patients after the percutaneous renal biopsy. Methods Relevant randomized controlled trials(RCTs)were collected using databases of China National Knowledge Infrastructure(CNKI), VIP, Wanfang Database, CBM, PubMed, Cochrane Library, Emba-se, Web of Science from inception of databases to April 2024. Quality evaluation of the included literature were performed. Data were analyzed with Revman 5.3 software. Results Thirteen studies were recruited, including 9 RCTs and 4 CCTs, and 2125 patients were analyzed. The Meta-analysis showed that compared with bed rest,early ambulation reduced the incidence of uroschesis in patients after the percutaneous renal biopsy[early activity: 51/368; bed rest: 151/559; RR=0.44, 95%CI (0.22, 0.91), the incidence of low back discomfort[early activity: 118/637; bed rest: 265/941; RR=0.52, 95%CI(0.43,0.61)], and the incidence of sleep disorder[early activity: 15/92; bed rest: 41/92; RR=0.37, 95%CI(0.22, 0.60)]. There were no statistically significant difference on gross hematuria and perirenal hematoma in patients after the percutaneous renal biopsy. Conclusion Early ambulation did not increase the incidence of gross hematuria and perirenal hematoma in patients after the percutaneous renal biopsy compared with bed rest. However, it can reduce uroschesis in patients after the percutaneous renal biopsy. Besides, early ambulation would be propitious to increase comfort of patients.

    Objective This study aimed to construct a research project management model based on the theory of the entire life cycle and explore its application effects. Methods A qualitative-quantitative comprehensive research method was employed, with qualitative grounded theory research as the core, to establish a research project management model based on the theory of the entire life cycle. Quantitative research was also conducted to analyze changes in research project approval and output from 2019-2020 under different management modes and evaluate the quality and effectiveness of these modes. Results Under the new mode, there was a significant increase in the absolute number of approved research projects, with a trend towards younger project leaders. The numbers for reasonable use of project funds, on-time completion of projects, and output of research results increased by 887.50%, 167.74%, and 82.61% respectively, with statistically significant differences (P<0.05). Conclusion The research project management model based on entire life cycle theory can effectively guide the scientific research management work at municipal-level hospitals while improving both quality and efficiency in research projects,and is worth applying to hospitals at the same level.

    Objective To share the blood group identification and analysis process of a patient whose ABO blood type does not match the positive and negative typing, so as to improve the recognition and processing ability of the clinical laboratory on difficult blood types and ensure the safety of clinical blood use. Methods The blood samples were collected from a patient whose ABO blood group did not match the positive and negative types in the Department of Critical Care Medicine of Guiyang Second Hospital. Serological test and gene detection were used to identify the ABO blood group of the patient. Results The positive type of ABO blood group in patient samples is B type and the negative type is AB type. Irregular antibody screening direct anti-human globulin test and indirect anti-human globulin test were negative. Absorption-emission test suggested that red blood cells contained A antigen PCR gene amplification and sequencing showed that the genotype of ABO*A1.02 and ABO*B.01 was A1/B type and A1B type; ABO gene mutations in exon 6 297A>G and exon 7 467C>T, 526C>G, 657C>T, 703G>A, 796C>A, 803G>C, 874C>G, 930G>A of the ABO gene have not been recorded in the International Blood Transfusion Association. Conclusion The ABO blood group of the patient was finally determined as A1B by serological test and molecular biological technology. The 874C>G gene mutation on the patient's exon 7 may be a new discovery revealed by this study for the first time. Gene site mutation interferes with the correct identification of blood type, and the combination of serological tests and molecular biology technology is of great significance for the correct identification of blood type and ensuring clinical safe blood transfusion treatment.

    Objective To explore and analyze the clinical efficacy and safety of Yiqing Capsules combined with Viamine Capsules in the treatment of oral lichen planus. Methods A random number table method was used to group 238 cases of lichen planus admitted to Department of Stomatology, Jiaozhou Hospital from April 2020 to February 2024, with 119 cases in each group. The control group was treated with vitamin E capsules, while the observation group was treated with Yiqing capsules. The two groups were treated continuously for 3 months. The efficacy, pain degree, lesion area and inflammatory response were compared between the two groups, and the adverse reactions were compared between the two groups. Results The total effective rate of the observation group was higher than that of the control group. Compared with the control group after treatment, the VAS score of observation group was lower, the lesion area was reduced, and the serum levels of SP, IL-8 and IL-12 were lower, with statistical significance (P<0.05). There was no significant difference in adverse reactions between the two groups (P>0.05). Conclusion The combination of Yiqing capsule and viaminate capsule showed a higher clinical total effective rate of oral lichen planus, which can promote the regulation and improvement of symptoms and signs of patients, reduce the pain caused by the disease, shrink the lesion site, and also regulate the levels of serum SP, IL-8 and IL-12 during treatment, so as to obtain a better prognosis. The overall security is also high.

    Objective To analyze the early adverse reactions of patients with differentiated thyroid cancer(DTC) after initial iodine-131 treatment and its influencing factors. Methods 106 patients who were treated with iodine-131 for the first time after DTC surgery in the nuclide treatment ward of Chifeng Municipal Hospital from January 2022 to December 2022 were retrospectively collected. Adverse reactions within 4 days after taking oral iodine-131 were recorded, and the influencing factors were analyzed by Logistic regression analysis. Results Among the 106 patients, 41 had neck swelling or pain, 36 had stomach discomfort or vomit, 32 had dry mouth or canker sores, 11 had abnormal taste, 10 had dry eyes, and 3 had chest tightness and shortness of breath. These adverse reactions were related to iodine absorption rate, iodine-131 dosage, TgAb level, postoperative stage and other multi-factors. The multiple factor regression equation of neck pain and swelling and its influencing factors was y = 2.223 × 4h iodine uptake rate + 20.802 × postoperative staging-28.689. The predictive accuracy of the equation was 86.8%. The multiple factor regression equation of the total adverse reaction was y = 0.084 × iodine dosage + 0.597 × 24h iodine uptake rate-11.499. The predictive accuracy of the equation was 82.1% . Conclusion Early adverse reactions of DTC patients after iodine-131 treatment have some clear influencing factors, and early drug or nursing intervention can be carried out to reduce the occurrence of early adverse reactions.

    Objective This study aims to explore the independent risk factors for pulmonary thromboembolism in acute exacerbation of chronic obstructive pulmonary disease (AECOPD), and to construct a mathematical model to evaluate predictive efficacy through in-depth analysis of these factors. Method 385 detailed medical records of AECOPD patients treated at Inner Mongolia Autonomous Region People's Hospital from January 2019 to March 2023 were selected for analysis. The research subjects were divided into a group without pulmonary thromboembolism and a group with pulmonary thromboembolism. Using univariate and multivariate analysis strategies to identify the determining factors of pulmonary thromboembolism in AECOPD. Result Among the 385 patients involved in the study, 53 were diagnosed with concomitant pulmonary thromboembolism. Univariate and multiple logistic regression analysis confirmed that Pap's index, length of bed rest, lower limb venous thrombosis, right heart dysfunction, arterial oxygen pressure, fibrinogen, and C-reactive protein were independent risk factors for AECOPD complicated with pulmonary thromboembolism (P<0.05). By using a logistic regression model that includes independent influencing factors and P-values, a ROC curve was constructed to predict the occurrence of pulmonary thromboembolism in AECOPD. The areas under the curve showed discriminative abilities of 0.707, 0.660, 0.575, 0.654, 0.719, 0.712, and 0.669, respectively. The P-value prediction was particularly outstanding, reaching 0.905. Conclusion Pulmonary thromboembolism in AECOPD patients is closely related to factors such as Pap's index. The established prediction model exhibits significant predictive ability in terms of specificity and sensitivity.

In recent years, it has been widely used in various fields of biomedicine, such as preoperative personalized plan planning, intraoperative navigation, surgical guide production, clinical teaching of doctors, surgical skills training, doctor-patient communication, personalized in vivo implant printing, and in vivo drugs scaffold implantation, human tissue and organ regeneration, cell culture proliferation, etc. It has also been widely used and in-depth researched in breast surgery, such as preoperative tumor localization in breast-conserving surgery, intraoperative personalized guide navigation surgery and stent placement, postoperative placement of local chemotherapy drugs and precise positioning of radiotherapy target areas; 3D printing of inferior epigastric vessel perforator flap, volume calculation of breast prosthesis and 3D printing bioscaffold combined with adipose tissue regeneration in breast reconstruction surgery.     The thought of "spleen and stomach is the foundation" runs through the development of traditional Chinese medicine. The disease and syndrome of spleen and stomach system is an important part of internal medicine of traditional Chinese medicine. Of the 31 medical cases of‘Acupuncture Dacheng' Volume 9, 11 were related to spleen and stomach diseases. In the medical case of spleen and stomach disease, Yang used the five elements to diagnose and treat the disease, explained the nature of the disease according to the hexagram, and treated the disease based on the pathogenesis of the disease. In the process of diagnosis and treatment, Yang distinguishes the specimens and priorities; regulates Qi; gets a small number of acupoints to choose but a clever idea, matches the extraordinary points and points of the right meridian; adjusts the conception vessel and governor vessel, selects specific acupoints; uses acupuncture and moxibustion together, pays attention to the technique.     Objective To explore the potential for integrating traditional Chinese medicine (TCM) principles with mindfulness-based stress reduction (MBSR) therapy in the treatment of psychosomatic conditions. Methods By reviewing relevant literature on TCM and MBSR, this study compiles their respective roles and advantages in psychosomatic treatments and discusses the potential for their integration. Results TCM is centered on the holistic view and syndrome differentiation-based treatment, emphasizing the unity of body and mind. MBSR primarily utilizes body scan, mindfulness yoga, and sitting meditation to mitigate negative emotions, enhance psychological resilience, and promote overall health. Both approaches emphasize the dual treatment of body and mind. In therapeutic applications, MBSR can reduce perceived psychological stress, improve medication adherence, and facilitate better absorption and utilization of traditional Chinese medicines. Conclusion The integration of TCM and MBSR in psychosomatic treatments holds promise for enhancing the overall efficacy of disease management.

    Objective To analyze the physical fitness monitoring results of different body fat and muscle mass in children aged 3 to 6 years old, and to understand the influence of body fat and muscle mass changes on physical development level. Methods Using random cluster sampling, 573 children aged 3 to 6 from four township kindergartens in Fangshan District were selected for monitoring of body morphology and physical fitness indicators using bioelectrical impedance analysis, and variance analysis was conducted. Results There were statistically significant differences in the physical fitness monitoring quality indexes of different body fat and muscle mass in children aged 3 to 6 years old: grip strength, standing long jump, continuous jump with both feet, 15-meter obstacle course, and balance beam (P<0.05). With different degrees of increase in body fat and muscle mass, the trends of increasing or decreasing of different physical quality indexes were different. Conclusion With the increase of body fat in a certain range, it is beneficial to the monitoring results of physical quality indexes: grip strength, 15-meter obstacle course, standing long jump, continuous jump with both feet, and balance beam. With the increase of body muscle mass, the monitoring results of physical quality indexes: grip strength, standing long jump show an increasing trend.

    Objective To understand nurses' beliefs and attitudes towards nurse's prescription rights in traditional Chinese medicine hospitals,so as to provide reference for medical decision-makers. Methods Convenience sampling was used to select 1 397 nurses in traditional Chinese medicine hospitals that were assigned the scale of nurses' attitudes towards and belief in nurse's prescription right, and the results were statistically analyzed. Results The average score of the 1 397 nurses' belief and attitude towards nurse prescription right was (95.86±19.03);Nurse position and outpatient service were the influencing factors of nurses' beliefs and attitudes towards nurse's prescription rights, which together explained 5.2% of the variation;Nurses have high intention of prescribing with chronic diseases' patients useing long term drug(49.2%) and medical consumables(48.5%); Nurses in traditional Chinese medicine hospitals suggested that traditional Chinese medicine specialized nurses be given the right to prescribe traditional Chinese medicine nursing techniques; It is suggested that the prescription right should be given to traditional Chinese medicine specialized nurses after standardized training. Conclusion Nurses in traditional Chinese medicine hospitals have positive beliefs and attitudes towards nurses' prescription rights; For the qualification of nurses' prescription right access, factors such as working years, professional titles and positions of nurses should be comprehensively considered; Formulate policies, laws and regulations on nurses' prescription right as soon as possible; To construct the nurse drug formulary applicable to our country as soon as possible.It is suggested that traditional Chinese medicine specialist nurses should learn standardized theory and practice,have clinical syndrome differentiation ability and be strictly examined to obtain prescription right.

    Objective Study on the effect of combined intervention of bundled nursing and traditional Chinese medicine nursing on abdominal distension and constipation in orthopedic bedridden patients. Methods From November 2021 to November 2023, a total of 60 orthopedic patients with abdominal distension and constipation who were admitted to the orthopedic department of our hospital were selected as the study subjects. The patients were divided into two groups using odd and even number method: the control group (n=30) received bundled nursing care, while the observation group (n=30) received combined traditional Chinese medicine nursing intervention based on the control group's nursing. After nursing, the symptoms of abdominal distension and constipation in both groups and the overall intervention effect were evaluated Comparative analysis of quality of life and complications. Results After intervention, the observation group had lower scores on the Self Rating Scale for Abdominal distension and Constipation Symptoms (PAC-SYM) compared to the control group, and the difference was statistically significant (P<0.05); The total effective rate of intervention in the observation group was 96.67%, while in the control group it was 66.67%. The observation group had a higher effective rate, and the difference was statistically significant (P<0.05); The physiological, psychological, and mental state scores of the observation group were higher than those of the control group, and the difference was statistically significant (P<0.05); The incidence of complications in the observation group was 3.33%, which was statistically significant compared to 26.67% in the control group(χ2=6.405, P<0.05). Conclusion For patients with abdominal distension and constipation after long-term bed rest in orthopedics, the combination of bundled nursing and traditional Chinese medicine nursing intervention can accelerate the improvement of clinical symptoms and reduce the occurrence of other complications. The quality of life of patients is significantly improved, and the intervention effect is more ideal.

    Objective To explore the effect of multi perspective narrative nursing on happiness, psychological status, and social support in hospitalized SP patients. Methods A total of 92 hospitalized patients with SP from March 2022 to July 2023 who were admitted in the Fourth People's Hospital of Liaocheng City were selected and randomly divided into the intervention group (n=46, multi perspective narrative nursing intervention) and the control group (n=46, routine nursing intervention), intervention for 3 months on average. The happiness, psychological status, social support, and nursing satisfaction in both groups were compared. Results After intervention, GWB scores in both groups were increased, and in comparision of the control group, the intervention group was higher (P<0.05). After intervention, BAI and BDI-II scores in both groups were decreased, and in comparision of the control group, the intervention group was lower (P<0.05). After intervention, SSRS scores in both groups were increased, and in comparision of the control group, the intervention group was higher (P<0.05). In nursing satisfaction statistics, in comparision of the control group 80.43% (37/46), the intervention group 95.65% (44/46) was higher (P<0.05). Conclusion The application of multi perspective narrative nursing in hospitalized patients with SP can improve their sense of happiness, psychological state, social support, and nursing satisfaction.

    Objective To observe the changes of abdominal pain, physical discomfort symptoms, quality of life, anxiety and depression in women with missed miscarriage after postoperative treatment with auricular pressure combined with the fire dragon cupping therapy. Methods Select 60 cases of missed abortion patients who were hospitalized in the gynecology department of Shenzhen Hospital of Integrated Traditional and Western Medicine from September 2022 to September 2023 as the research subjects. They were evenly divided into an observation group and a control group according to the random number table method, with 30 cases in each group.The control group was given intramuscular oxytocin 20 U, qd, for 3 days. On this basis, auricular pressure combined with the fire dragon cupping therapy were added in the observation group.Symptoms of physical discomfort, Self-Rating Anxiety Scale (SAS) score, Self-Rating Depression Scale (SDS) score, and quality of life were assessed. Results Two weeks post-operation, the incidence of lower abdominal pain in the control group was higher than in the observation group, with a statistically significant difference (P<0.05).At 8 hours post-operation and 2 weeks post-operation, the median level of discomfort in the control group patients was higher than that in the observation group, with a statistically significant difference (P<0.05).Two weeks post-operation, the median SAS and SDS scores of the control group patients were higher than those of the observation group, with a statistically significant difference(P<0.05). The median quality of life of the control group patients was lower than that of the observation group,with a statistically significant difference(P<0.05). Conclusions Auricular pressure combined with the fire dragon cupping therapy has shown positive effects on postoperative pain, negative emotions and quality of life in patients with missed miscarriage and provides comprehensive rehabilitation support for patients with missed miscarriage.
