    Objective To investigate the clinical effectiveness of Fenugreek in patients with pre-diabetes. Methods The selected 100 patients with pre-diabetes who get medical treatment in the 5 hospitals were randomly divided into 3 groups used the random number table method: comparison group (20 cases), the treatment group with placebo (40 cases),the treatment group with fenugreek (40 cases). They all were applied with diabetes life intervention education. The other two treatment groups with placebo and fenugreek additionally took placebo or fenugreek one-to-one correspondence. Observe records and compare waist circumference (WC), body mass index (BMI), glycosylated hemoglobin A1c (HbA1c), fasting plasma glucose (FPG), oral glucose after 2 hours blood glucose (2 hPG), triglyceride (TG) of pertherapy and post-treatment of the 3 groups. Result Compared with those of pertherapy,the observation indexes were decreased in all groups. Compared with the other two groups, the levels of WC, BMI, HbA1c, FPG, 2 hPG, TG of the treatment group with fenugreek decreased more obviously (P<0.05). Conclusions Fenugreek granule is effective to pre-diabetes and it is worth being used widely in clinical practice.

    Objective To observe and analyze the safety and effectiveness of transurethral holmium laser enucleation of the prostate (HoLEP) in patients with BPH after transrectal or transperineal prostate biopsy. Methods The data of 60 BPH patients who underwent HoLEP in the Department of Urology, First Affiliated Hospital of USTC(Anhui Provincial Hospital) from January 2018 to June 2020 were collected. All the patients are diagnosed by prostate biopsy and divided into transperineal group and transrectal group according to the operational approach.There were 29 cases in the transperineal group and 31 cases in the transrectal group. The basic information, perioperative indicators and follow-up data of 3 and 6 months after surgery were collected and analyzed. Results The length of surgery and postoperative catheterization time in the transrectal group was longer than that in the transperineal group, and the difference was statistically significant (P<0.05). The overall postoperative complication rate was lower in the transperineal group than in the transrectal groupthe transperineal group, the difference between the two groups was statistically significant (P<0.05). Comparison of the International Prostate Symptom Score (IPSS), Quality of Life Score (QOL) for prostate symptoms and maximal urinary flow rate (Qmax) of patients in the two groups at 3 and 6 months postoperatively, the difference was not statistically significant (P>0.05). Conclusion HoLEP treatment for BPH patients at 3 weeks after transperineal or transrectal prostate biopsy is effective and safe. In the patients who underwent HoLEP after transperineal biopsy, the operation time and indwelling catheterization time are shorter and has fewer complications postoperatively.

    Objective To investigate the effects of bronchoscopic alveolar lavage combined with inhalation of acetylcysteine solution on blood gas indexes and pulmonary ventilation function in patients with severe pneumonia. Methods 80 patients with severe pneumonia admitted to First People's Hospital of Pingyuan County from January to December 2021 were selected and randomly divided into groups. The control group (n=40) received bronchoscopic alveolar lavage, while the other (n=40) received inhalation of acetylcysteine solution for intervention. The total effective rate and clinical indicators of the two groups were compared. Results The total effective rate of treatment in the observation group was higher than that in the others, with a statistically significant difference (P<0.05). After treatment, the partial pressure of oxygen in the observation group was higher than those, and the partial pressure of carbon dioxide was lower than the others. The levels of FEV1, FEV1/FVC, and PEF in the first second were significantly higher than those in the others (P<0.05). After treatment, the PCT and interleukin IL-6 in the observation group were lower on average than those in the control group, and the levels of IL-10 were higher than those in the control group, with a statistically significant difference (P<0.05). Conclusion The treatment of severe pneumonia patients with bronchoscopic alveolar lavage combined with inhalation of acetylcysteine solution has a definite effect, and can alleviate the body's inflammatory reaction.

    Objective ToMethodsexplore and analysis the correlation between humoral immune function and dehydration acidosis in children with rotavirus group A infectious diarrhea. Methods From July 2021 to December 2022, 80 cases of children with group A rotavirus infectious diarrhea diagnosed and treated in the 924 Hospital of the Joint Service Support Force of the People's Liberation Army of China were selected as the rotavirus group, and the other 80 cases of children with non-rotavirus infectious diarrhea diagnosed and treated in our hospital were selected as the control group. After admission, all children were judged the occurrence of dehydrated acidosis, and their humoral immune function were tested and correlation analysis were performed. Results The incidence rates of dehydration acidosis in rotavirus group were 13.75%, and that in control group were 2.50%, the incidence rates of dehydration acidosis in rotavirus group were significantly higher than that in control group (P<0.05). The contents of serum IgA, IgM and IgG in rotavirus group were significantly lower than those in control group (P<0.05). In the rotavirus group, Spearman analysis showed that there were negative correlation between dehydrated acidosis and serum IgA, IgM and IgG levels (P<0.05). Multiple dichotomy logistic regression analysis showed that the contents of serum IgA, IgM and IgG were important risk factors for dehydration acidosis in children with rotavirus group A infectious diarrhea (P<0.05). Conclusion Children with group A rotavirus infectious diarrhea are often accompanied by dehydration acidosis, and humoral immune function will also decline. There is a correlation between humoral immune function and dehydration acidosis in children with group A rotavirus infectious diarrhea.

    Objective To explore and analysis the application of genome sequencing (The mNGS technology guides the clinical effects of bronchiectasis. Methods From August 2019 to November 2022, 100 patients with bronchiectasis treated in the Fourth People's Hospital of Nanhai District, Foshan City, Guangdong Province were selected as the study subjects, 50 patients in mNGS group and 50 in control group according to random 1:1 numerical table. The control group were given bronchoscopy and alveolar lavage treatment. The mNGS group were given genome sequencing analysis and guided clinical treatment on the basis of the treatment of the control group. The treatment observation time of the two groups were 1 month, and the efficacy, changes in blood gas indicators, improvement time of clinical symptoms, adverse reactions and Changes in lung functionof the two groups were analyzed. Results The total effective rates of the mNGS group after 1 month of treatment were significantly higher than the control group (P<0.05). The arterial oxygen partial pressure and blood oxygen saturation of the two groups after 1 month of treatment were significantly higher than those before treatment (P<0.05), and the mNGS group were also significantly higher than the control group (P<0.05). The disappearance time of lung rale, cough and sputum, and the time of body temperature returning to normal in the mNGS group were significantly reduced compared with the control group (P<0.05). The incidence of adverse reactions such as headache, rash, insomnia and gastrointestinal discomfort after treatment in the mNGS group were significantly lower than that in the control group (P<0.05). After 1 month of treatment, forced expiratory volume (FEV1) and forced vital capacity (FVC) of 1s in two groups were significantly increased compared with before treatment (P<0.05), and mNGS group was also significantly increased compared with control group (P<0.05). Conclusion Using mNGS technology to guide the clinical treatment of bronchiectasis can reduce the time for the disappearance of lung rale, cough and sputum, and the time for the temperature to return to normal, improve the treatment effect of patients, improve the arterial oxygen partial pressure and blood oxygen saturation of patients, reduce the incidence of adverse reactions, and also improve the lung function of patients.

    Objective To explore the effects of respiratory exercises combined with aerobic exercise on blood gas analysis and lung function in patients with stable COPD. Methods Clinical data of 100 patients with COPD stable stage admitted to Rizhao Central Hospital from January 2020 to May 2022 were retrospectively analyzed and divided into two groups according to different intervention methods. The control group (n=50) received aerobic exercise, and the observation group (n=50) received respiratory exercises combined with aerobic exercise, continuous training for 2 months. Blood gas analysis index, lung function index, exercise endurance and quality of life were compared between the two groups. Results There was no significant difference in blood gas analysis indexes between the two groups before intervention (P>0.05). After intervention, the partial oxygen pressure (PaO2) and oxygen saturation (SaO2) of the observation group were higher than those of the control group, while the partial carbon dioxide pressure (PaCO2) of the observation group was lower than that of the control group, and the difference was statistically significant (P<0.05). Before intervention, there was no significant difference in lung function index and exercise endurance between the two groups (P>0.05). After intervention, forced vital capacity (FVC), forced expiratory volume (FEV1) at the first second, FEV1/FVC and etc. in the observation group were higher than those in the control group; The walking distance of 6 min was longer than that of control group,and the difference was statistically significant (P<0.05). Before intervention, there was no significant difference in quality of life between the two groups (P>0.05). After intervention, the scores of psychological dimension, spiritual dimension,environmental dimension, physiological dimension, social relation dimension, independence dimension, general health and quality of life in the World Health Organization Quality of Life Scale (WHOQOL-100) in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). Conclusion Combined aerobics with aerobic exercise can improve the blood gas index of patients with COPD,restore lung function, and improve exercise endurance and quality of life of patients.

    Objective To analyze the efficacy of Huangqi Guizhi Wuwu Decoction in the treatment of peripheral neuritis after chemotherapy. Methods From January 2021 to May 2022, 60 patients with peripheral neuritis after chemotherapy who were treated in Heilongjiang Provincial Hospital were selected as the study subjects. They were randomly divided into two groups according to the digital grouping method: the external washing group with traditional Chinese medicine (30 cases) and the control group (30 cases). The control group patients were given routine treatment and vitamin treatment, and the external washing group patients were treated with Huangqi Guizhi Wuwu Decoction on the basis of the control group. Compare the overall efficacy, visual analog scale (VAS), and serum interleukin-1 of the two groups of patients before and after treatment β (IL-1β), Tumor necrosis factor-α(TNF-α) Level improvement. Results The total effective rate of patients in the external washing group was significantly higher than that in the control group (P<0.05). Before treatment, there was no significant difference in serum IL-1β, TNF-α levels and pain scores between the two groups (P>0.05). After intervention, serum IL-1β、 TNF-α and pain scores were significantly decreased (P<0.05), with a significant difference compared to before intervention (P<0.05), and the serum IL-1β, TNF-α of patients in the external washing group of traditional Chinese medicine was also significantly decreased (P<0.05). The decrease in pain scores was significantly greater in the control group than in the control group (P<0.05). Conclusion Huangqi Guizhi Wuwu Decoction has a good therapeutic effect on peripheral neuritis after chemotherapy, which can effectively improve the inflammatory status of the body, reduce the degree of pain, and improve the overall treatment effect of patients. It is a safe and efficient treatment method.

    Objective To investigate the therapeutic effect of modified Sturmdorf suture after cervical conization on patients with cervical intraepithelial neoplasia (CIN) confirmed by cytological examination under colposcopy. Methods 64 patients with CIN confirmed by colposcopy cytology in the First People's Hospital of Aksu District from January 2020 to January 2022 were selected as the study subjects. They were divided into observation group and control group by random number table method, with 32 patients in each group. The patients in the control group were treated with traditional surgery and suture, while the patients in the observation group were treated with modified Sturmdorf suture after cervical conization. The relevant perioperative indexes of the two groups were compared, and their clinical efficacy, postoperative complications, bleeding and satisfaction with cervical appearance were compared. Results The intraoperative blood loss in the observation group was less than that in the control group, the operation time was longer than that of the control group, and the incision healing time and hospital stay were shorter than those in the control group (P<0.05). There was no significant difference in the total effective rate between the observation group and the control group (P>0.05). The incidence of postoperative complications such as cervical mucosa hyperplasia, cervical canal stenosis, pelvic inflammatory disease and postoperative bleeding in the observation group was significantly lower than that in the control group, and the satisfaction degree of cervical appearance in the observation group was significantly higher than that in the control group (P<0.05). Conclusion The modified Sturmdorf suture after cervical conization is effective for the patients with CIN disease confirmed by cytological examination under colposcopy. Compared with traditional surgery and suture, it can reduce the amount of bleeding and operation time of the patients during operation, promote the early recovery of the patients, and reduce the postoperative complications and bleeding of the patients. Moreover, the satisfaction of cervical appearance is good, which is worthy of clinical application and promotion.

    Objective To discuss the changes of infectious pathogens and lymphocyte subsets in patients with perianal necrotizing fasciitis. Methods From January 2022 to December 2022, 98 cases of patients with perianal necrotizing fasciitis diagnosed in a certain hospital were selected as the study subjects. All patients were given infection related microbial detection and surgical treatment, and the prognosis were observed. Investigated the general information of all patients, Detected and correlation analyzed of changes in lymphocyte subsets. Results Infection-related microorganisms were detected in all patients, included 54 cases of gram-positive bacteria, 39 cases of gram-negative bacteria and 5 cases of fungi. The main microorganisms were Staphylococcus haemolyticus, Escherichia coli, Enterococcus faecium, Klebsiella pneumoniae, Staphylococcus aureus, Pseudomonas aeruginosa. All patients were successfully completed the treatment, with 12 cases of complications and 9 cases of deaths (death group). The incidence rates of complications and mortality were 12.24% and 9.18% respectively. The proportion of CD3+T cells, CD8+T cells and NK cells in dead patients were significantly higher than that in living patients, while the proportion of B lymphocytes were significantly lower than that in living patients (P<0.05). Spearman correlation analysis showed that the death of patients with perianal necrotizing fasciitis were positively correlated with the proportion of CD3+T cells, CD8+T cells and NK cells, and were negatively correlated with the proportion of B lymphocytes (P<0.05). Logistic regression analysis showed that the proportion of CD3+T cells, CD8+T cells and NK cells were the main factors leading to the death of patients (P<0.05). Conclusion The infection microorganism of patients with perianal necrotizing fasciitis are mainly Staphylococcus haemolyticus, Escherichia coli and Enterococcus faecium. The incidence of complications and mortality after treatment are still relatively high. The changes of lymphocyte subsets are correlated with the prognosis of patients.

    Objective To investigate the application value of transthoracic echocardiography and double-source spiral CT in preoperative diagnosis of congenital heart disease in children. Methods A total of 66 cases of suspected congenital heart disease who were hospitalized and treated in Lanzhou First People's Hospital from February 2019 to December 2022 were collected as the study subjects. Before these patients, transthoracic echocardiography and double-source spiral CT were performed, and the surgical. Results were taken as the gold standard, comparing the subjective scores of image quality between ultrasound and CT groups, radiation dose and noise and signal-to-noise ratio of preoperative dual source spiral CT, surgical diagnosis, and the diagnostic value of transthoracic echocardiography and dual source spiral CT in children with congenital heart disease. Results The subjective score of the image quality of the structure, the near coronary artery between the ultrasound and CT were>4 points, and the contrast showed no significant difference (P>0.05). At the CT test, the dose length product was (13.20±2.18) mGy, the effective dose was (0.27±0.03) mSv, the cardiac macro-vessel CNR value was (8.22±0.47). The surgical diagnosis was 38 cases of congenital heart disease, included 15 atrial septal defects, 9 single atrium, 6 ventricular septal defects, 3 valvular lesions, 2 triplicate hearts, 2 single ventricles and 1 endocardial pad defect. In ultrasound examination, 36 cases of congenital heart disease and 2 cases of atrial septal defect were missed, 38 cases of CT with no missed misdiagnosis. Transthoracic echocardiography with bisource spiral CT had 94.74% and 100.00% sensitivity and 100.00% and 100.00% for preoperative diagnosis, and the ROC curve showed that the area under the curve predicted by transthoracic echocardiography was 0.963 and 1.000, respectively. Conclusion The application of transthoracic echocardiography and double-source spiral CT in preoperative diagnosis of congenital heart disease in children has high sensitivity and specificity, and both can provide a certain basis for clinical diagnosis and treatment of congenital heart disease.

    Objective To explore and analysis the diagnostic values of contrast-enhanced ultrasound combined with vaginal color Doppler ultrasound in endometrial lesions of postmenopausal vaginal bleeding. Methods From December 2019 to December 2022, 89 cases of patients with postmenopausal vaginal bleeding and endometrial lesions who were diagnosed and treated in our hospital were selected as the study subjects. All patients were examined with contrast-enhanced ultrasound combined with vaginal color Doppler ultrasound, and the ultrasonic characteristics were recorded. Pathological examination was taken as the gold standard, and the diagnostic value was judged. Results In the 89 patients, there were 69 cases were diagnosed as benign lesions (23 cases of endometrial hyperplasia, 17 cases of hysteromyoma, 29 cases of endometrial polyps, benign group) and 20 cases as malignant lesions (endometrial cancer, malignant group). The blood flow resistance index, peak systolic velocity and pulsation index in benign group were significantly higher than those in malignant group (P<0.05). The average transit time of contrast agent in the benign group was significantly shorter than that in the malignant group (P<0.05), and there was no significant difference compared between the two groups in terms of development time, peak time and peak intensity (P>0.05). In the 89 patients, the diagnostic coincidence rate of color Doppler ultrasound was 87.64%, the diagnostic coincidence rate of contrast-enhanced ultrasound was 86.52%, the diagnostic coincidence rate of contrast-enhanced ultrasound combined with vaginal color Doppler ultrasound was 98.88%, and the diagnostic coincidence rate of contrast-enhanced ultrasound combined with vaginal color Doppler ultrasound were significantly higher than that of single method (P<0.05). Conclusion The combination of contrast-enhanced ultrasound and vaginal color Doppler ultrasound has high diagnostic value for endometrial lesions of postmenopausal vaginal bleeding, which is worthy of clinical application.

    Objective To search the differentially expressed genes between chronic atrophic gastritis and gastric cancer, explore the molecular mechanism of gastric cancer transformation, and explore the potential therapeutic effects of traditional Chinese medicine and active components of traditional Chinese medicine. Methods The GSE116312 gene chip dataset was downloaded from the GEO database, and the differentially expressed genes were obtained. The GO and KEGG pathways were enriched through the Metascape database. The String database was used to construct the differentially expressed gene-protein interaction network to identify key genes, and to analyze the prognosis and immunity of key genes. The Coremine database and the CTD database were used to obtain Chinese herbal medicines and active ingredients with potential therapeutic effects, respectively. Results A total of 129 significantly differentially expressed genes were obtained, of which 105 were up-regulated and 29 were down-regulated. Biological processes mainly include digestion, hormone regulation, response to bacteria, organ or tissue specific immune response, response to heterogeneous stimuli, etc. They are enriched in protein digestion and absorption, cancer transcriptional disorder, gastric acid secretion, fructose and mannose metabolism, fat digestion and absorption and other pathways. MMP3, CTGF, THBS1, SPP1, IL6, CXCL8, END1, BGN, THBS2, and MUC6 are key genes in the transformation of chronic atrophic gastritis to gastric cancer. There is a high correlation between key genes and immune cell infiltration. The survival rate of patients with high expression of MUC6 and THBS1 is significantly lower than that of patients with low expression, and the survival rate of patients with high expression of MMP3 and CTGF is higher than that of patients with low expression. The potential therapeutic traditional Chinese medicines such as ginseng, panax notoginseng, herba artemisiae, magnolia officinalis, poria cocos, salvia miltiorrhiza, hedyotis diffusa, lycium barbarum were discovered, and the potential targeted active ingredients of traditional Chinese medicines such as resveratrol, quercetin, paclitaxel, andrographolide for treating gastric cancer were screened. Conclusion The core differentially expressed genes and pathways of chronic atrophic gastritis and gastric cancer discovered in this study provide a reference for subsequent basic experiments and clinical research, and provide ideas for the use of traditional Chinese medicine to block the transformation of inflammation and cancer.

    Objective To use the thinking of hexagrams to explain the traditional theory of Tibetan images. Methods Nearly 100 cases of diarrhea-predominant irritable bowel syndrome (IBS-D) were collected and reviewed. The relationship between liver and large intestine was interpreted according to the thinking of hexagrams and image-reading in Yi Medical College. Results Compared with the traditional acupuncture therapy, Yiyi navel acupuncture has the characteristics of less needling, surprising effect and rapid onset of action. Conclusion The umbilical-needle-tract method of Yiyi Medicine is natural, simple in usage, extraordinary in use and miraculous in effect, and provides a new perspective for the establishment basis and internal connection of the special relationship between zangfu organs by using the thinking of hexagrams and images, and Guide the daily health preservation.

    Objective To investigate the effect of action-oriented continuous nursing in the management of elderly patients with Mild Cognitive Impairment (MCI). Methods A total of 60 elderly patients with MCI admitted to The Third Affiliated Hospital of Qiqihar Medical University from January to December 2022 were selected as research objects and divided into observation group and control group by random number table method. The control group received routine nursing, and the observation group received action-oriented continuous nursing. The data of cognitive function, self-management ability, quality of life, falls and so on were compared between the two groups. Results There was no statistical significance in all indexes before nursing (P>0.05). After nursing, the cognitive function score of observation group was higher than that of control group (P<0.05); The self-management ability of observation group was higher than that of control group (P<0.05). The life quality score of observation group was higher than that of control group (P<0.05). The number of falls in the observation group was lower than that in the control group (P<0.05). Conclusion When nursing elderly patients with MCI, action-oriented continuous care intervention can effectively improve patients' cognitive function, enhance patients' self-management ability, improve patients' quality of life, and reduce the possibility of patients falling.

    Perimenopausal syndrome (PMS) is refered to a clinical syndrome in women with a lot of speculations that appear before, during and after the onset of menopause, caused by estrogen level fluctuations or lower a series of physical. Theory in traditional Chinese medicine for PMS believes that kidney deficiency is the basic pathogenesis of this disease, and can accumulate the heart, liver and spleen. Professor Fang Zhaohui, who has dealed with PMS for many years, believes that the deficiency of kidney Yin is the main pathogenesis of perimenopausal hyperhidrosis. The deficiency of kidney Yin causes blood deficiency to generate heat, which causes disease through internal disturbance. Therefore, clinical treatment should be based on nourishing Yin and lowering fire, taking nourishing the heart and calming the mind as therapeutic principles. The dialectical emphasis is on distinguishing Yin and Yang as well as deficiency and excess. The deficiency patients should be treated by invigorating Qi, nourishing Yin, replenishing blood and harmonizing Yingwei according to the different syndroms. Those who are real should clear the liver, diarrhoea and heat, dehumidify and camp; those who are mixed with virtual and real should distinguish the primary and secondary, and take appropriate attention to it. Use the medicine with the certificate, add astringent and astringent products as appropriate, enhance the antiperspirant effect, and achieve a good curative effect.

    Objective To investigate the effect of clostridium butyricum capsule combined with compound glutamine enteric coated capsule on patients with ulcerative colitis. Methods A total of 120 patients with ulcerative colitis admitted to Dongchangfu People's Hospital of Liaocheng city from January 2019 to December 2021 was selected. The lottery method was divided into two groups. The control group was given Clostridium butyricum capsules, and the observation group was given compound glutamine enteric coated capsules. To compare the efficacy of the two groups, the serum tumor necrosis factor-α (TNF-α), interleukin-2 (IL-2), hypersensitive C-reactive protein (hs-CRP), IL-6, intestinal barrier function (D-lactic acid, diamine oxidase, and bacterial endotoxin), and stool, abdominal pain, mucosal diarrhea were scores. Results The effective rate of the observation group was significantly higher than control group(P<0.05). Before treatment, there was no significant difference in the levels of TNF-α, IL-2, hs-CRP and IL-6(P>0.05). After treatment, serum TNF-α, IL-2, hs-CRP and IL-6 decreased significantly(P<0.05), and serum TNF -α, IL-2, hs-CRP and IL-6 in the observation group were significantly lower(P<0.05). Before treatment, the levels of D-lactic acid, diamine oxidase and bacterial endotoxin in the two groups had no significant difference(P>0.05). After treatment, the levels of D-lactic acid, diamine oxidase and bacterial endotoxin in the two groups were significantly reduced(P<0.05), and the levels of D-lactic acid, diamine oxidase and bacterial endotoxin in the observation group were significantly lower than those in the control group(P<0.05). Before treatment, there was no significant difference in the scores of purulent stool, abdominal pain, mucosal lesions and diarrhea symptoms between the two groups(P>0.05). After treatment, the scores of purulent stool, abdominal pain, mucosal lesions and diarrhea symptoms in the two groups were significantly reduced(P<0.05), and the scores of purulent stool, abdominal pain, mucosal lesions and diarrhea in the observation group were significantly lower than those in the control group(P<0.05). Conclusion Clostridium butyricum capsule combined with compound glutamine enteric-coated capsule has significant efficacy in patients with ulcerative colitis,
