    Clinical Study on Yiyun Materia Medica Yin in Auxiliary Improving Ovarian Function

    Objective To evaluate the auxiliary effect of Yiyun Materia Medica Yin in improving the ovarian reserve, so as to provide a way for early treatment of DOR. Methods A total of 64 subjects with diminished ovarian reserve who were recruited were enrolled in this study from August 2020 to April 2021.They were assigned to two groups: the experimental group and the control group, according to the sequence, with 32 cases in each group. The trail continued twelve weeks. After the trail,the sex hormone levels, number of bilateral sinus follicles, sexual life indicators, menstrual indicators and symptom and sign scores of the two groups of subjects were compared between the two groups before and after treatment. Result Before the trail, there was no significant difference in all indicators between the two groups(P>0.05). After the trail, the FSH in the experimental group was lower than before, and the scores of AFC, AMH, sexual life indicators, symptoms and signs were improved compared with at the begining of the trail. The FSH, sexual life indicators, symptoms and signs were significantly different from those in the control group after the trail (P<0.05). Conclusion Yiyun Materia Medica Yin can effectively improve the decreased ovarian reserve, and the safety is good.

    Objective To explore the effect of external diaphragm pacing combined with holistic nursing on patients with chronic obstructive pulmonary disease (COPD) and pulmonary function. Methods From January 2022 to December 2022, 90 patients with chronic obstructive pulmonary disease in the Heze Municipal Hospital were selected as the study subjects. They were divided into two groups by random number table method. The control group was given routine nursing mode, while the observation group was intervened by external diaphragm pacing combined with holistic nursing. The pulmonary function index value, incidence rate of complications, 6-minute walking distance observation index, respiratory condition and self-evaluation observation index, quality of life, oxygenation index and arterial blood gas index value, and clinical symptom regression time of the two groups were compared and analyzed. Results After nursing intervention, the indexes of lung function in the observation group improved significantly (P<0.05); in the comparison of 6-minute walking distance, the final walking distance of the observation group was much longer than that of the control group (P<0.05); from the comparison of quality of life, respiratory status, self-evaluation score, oxygenation index and arterial blood gas index, the observation group was better than the control group (P<0.05). Conclusion External diaphragm pacing combined with holistic nursing can significantly improve the pulmonary function of patients with chronic obstructive pulmonary disease, reduce the incidence of complications, improve the respiratory function of patients, and shorten the rehabilitation time.

    Objective To explore the effect of comprehensive diet nursing on nutritional status and quality of life of hemodialysis patients with diabetes nephropathy. Methods A total of 80 patients with diabetes nephropathy admitted in our hospital from February 2021 to February 2022 were randomly divided into the control group and the observation group. During hemodialysis treatment, routine nursing was given to the control group and comprehensive diet nursing was given to the observation group. The nutritional index value, renal function, calcium and phosphorus metabolism, quality of life index value and medical immunology index value of the two groups were compared. Results After the intervention of different nursing measures, the levels of hemoglobin (HB), total serum protein (STB) and serum protein (ALB) in the observation group were higher than those in the control group (P<0.05); The indexes of renal function (Scr, SUN), blood potassium and phosphorus in the observation group were better than those in the control group (P<0.05); In the observation group, the improvement of quality of life, immunological index A(IgA), immunoglobulin M(IgM) and immunoglobulin G(IgG) was more significant (P<0.05). Conclusion Comprehensive diet nursing can better improve the nutritional status of patients with diabetes nephropathy, better improve renal function and calcium and phosphorus metabolism, more conducive to the improvement of patients' quality of life and enhance immunity.

    Objective To explore and analyze the value of echocardiography parameter integration system in the risk assessment of prognosis of patients with heart failure at high altitude. Method From August 2020 to July 2022, 76 patients with heart failure in high-altitude areas who were treated in Maqin County People's Hospital were selected as research objects. All patients were given cardiac ultrasound examination, routine ultrasound indexes were recorded, and cardiac ultrasound parameter integrals were determined. Patients were divided into adverse and non-adverse cardiovascular events according to the follow-up results. Spearsman analysis and COX regression analysis were performed to follow up the prognosis of patients, and ROC curve was drawn for risk assessment and analysis. Results After six months of follow-up, major cardiovascular adverse events occurred in 16 of 76 patients (adverse group). There were no significant differences in gender, age, body mass index, systolic blood pressure, diastolic blood pressure, NYHA grade and primary disease in the adverse group compared with the non-adverse group (P>0.05). The score of cardiac ultrasound parameters in the adverse group which was significantly higher than that in the non-adverse group (P<0.05). The left atrial diameter, left ventricular end-diastolic diameter, left ventricular end-systolic diameter and early diastolic blood flow velocity in the poor group were higher than those in the non-adverse group (P<0.05), while the late diastolic blood flow velocity, left ventricular ejection fraction and stroke output in the poor group were lower than those in the non-adverse group (P<0.05). Among 76 patients, Spearsman analysis showed that early diastolic blood flow velocity and cardiac ultrasound parameter integral were positively correlated with prognostic major cardiovascular adverse events, while late diastolic blood flow velocity was negatively correlated with prognostic major cardiovascular adverse events (P<0.05). Multivariate COX regression analysis showed that early diastolic blood flow velocity, late diastolic blood flow velocity and cardiac ultrasound parameter integral were all important factors affecting the prognosis of patients with major cardiovascular adverse events (P<0.05). ROC curve analysis showed that the area under the curve of risk assessment of heart failure patients at high altitude was 0.889. Conclusion Major cardiovascular adverse events are likely to occur in patients with heart failure at high altitude during the prognosis process, which can lead to abnormal cardiac ultrasound parameter integration. Cardiac ultrasound parameter integration system has a good risk assessment value for the prognosis of patients with heart failure at high altitude, and has high clinical practical value.

    Objective To analyze the effect of thymus pentapeptide combined with conventional antituberculosis regimen in patients with pulmonary tuberculosis. Methods The medical records of 80 tuberculosis patients diagnosed and treated by Rizhao Tuberculosis Prevention and Control Institute from April 2021 to April 2022 were retrospectively analyzed and divided into two groups according to different treatment methods. Among them, 40 patients treated with conventional anti-tuberculosis regimen (2HRZE/4HR) were included in the control group, and 40 patients treated with thymus pentapeptide combined with conventional anti-tuberculosis regimen were included in the observation group. Sputum negative conversion rate, focus absorption rate, immune function, lung function and adverse reactions were compared between the two groups. Results After 2 months and 8 months of treatment, the sputum negative conversion rate and obvious focus absorption rate in the observation group were higher than those in the control group, with statistical differences (P<0.05). Before treatment, CD3+, CD4+ and CD8+ had no statistical differences in forced vital capacity (FVC), forced breathing volume at the first second (FEV1) and peak expiratory flow (PEF) between the two groups (P>0.05). After treatment, CD3+, CD4+, FVC, FEV1 and PEF in the observation group were higher than those in the control group, while CD8+ was lower than those in the control group, with statistical difference (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Thymus pentapeptide and conventional anti-tuberculosis regimen has an obvious effect in patients with pulmonary tuberculosis, which can accelerate sputum negative conversion, promote lesion absorption, and significantly improve immune function and lung function without serious adverse reactions. Therefore, it has high clinical application value and is worthy of vigorous implementation.

    Objective To investigate the mechanism of inhibition of NSCLC by combination of Breviscapine and Tetramethylpyrazine. Methods Non-small cell lung cancer A549 cells were divided into control group, Breviscapine group, Tetramethylpyrazine group,Breviscapine and Tetramethylpyrazine group. Detect cell survival rate by MTT, apoptosis by flow cytometry, cell migration ability by Transwell test. Related pathway changes were detected by Western Blot. Results The cell survival rate of A549 cells in the control group, Breviscapine group, Tetramethylpyrazine group, and Breviscapine and Tetramethylpyrazine group decreased after 24h (P<0.05). Compared with control group, A549 cells showed significant apoptosis and cells in Breviscapine and Tetramethylpyrazine group showed the most evident changes; Transwell showed that the number of trans-membranous cells decreased significantly compared with the control group and membrane penetration was most evidently inhibited in combination group. Western blot showed that the expression of p-JAK2 and p-STAT3 decreased after drug administration, and the two proteins were lowest in the Breviscapine and Tetramethylpyrazine group. Conclusion Breviscapine combined with Tetramethylpyrazine inhibited the proliferation, migration and invasion of the non-small cell lung cancer cell line A549, and the mechanism may be related to the inhibition of the JAK-STAT pathway.

    Objective To study the effect of regulating breathing and nourishing lung exercises on the quality of life of patients with chronic obstructive pulmonary disease in stable period. Methods Using convenient sampling method, 80 patients with chronic obstructive pulmonary disease in stable stage who were admitted from March 2022 to October 2022 were selected as the study subjects which were randomly divided into control group and observation group with 40 cases each. The control group was given routine treatment and nursing, while the observation group was given routine treatment and nursing plus breathing and lung exercises intervention. The quality of life scores and six-minute walking test distance of the two groups were compared after 8 and 16 weeks of intervention. Results After 8 and 16 weeks of intervention, the quality of life scores of the observation group decreased, and the six-minute walking test distance increased. The difference was statistically significant between the two groups (P<0.05). Conclusion Breath-regulating and lung exercises can improve the quality of life and exercise endurance of patients with chronic obstructive pulmonary disease in stable stage.

    Objective To investigate the effect of Jiawei Liujunzi Decoction in treating tic disorder of spleen deficiency and phlegm accumulation type. Methods A total of 76 children with tic disorder of spleen deficiency and phlegm accumulation type admitted to the pediatric clinic of Weifang Hospital of traditional Chinese medicine 10.2021~5.2022 were seperated into two groups by random numbers. 38 cases in the control group were treated with tiapride hydrochloride tablets; 38 cases in the observation group were treated with Jiawei Liujunzi Decoction. Duration of two groups’ treatment were 8 weeks. The score of YGTSS, TCM symptoms, the levels of neurotransmitters, T lymphocyte subsets, vitamin A and vitamin D were detected and compared. Results The YGTSS and TCM symptom score of the children treated with Jiawei Liujunzi Decoction were lower than those treated with tiapride after treatment (P<0.05). The levels of IL-6, 5-HT and dopamine in the two groups after treatment were lower than those before treatment, GABA was higher than that before treatment, the range of changes in these indicators in the observation group were more remarkable than those in the control group after treatment (P<0.05). After treatment, the levels of CD3+、CD4+、CD4+/CD8+ in the two groups were raised than 8 weeks before, and the levels of CD8+ were lower than 8 weeks before. The range of changes in these indicators in the observation group were more remarkable than those in the control group after treatment (P<0.05).The levels of Vit A and Vit D in the observation group were higher than those in the control group (P<0.05). Conclusion Jiawei Liujunzi Decoction has a more effective therapeutic effect on the tic symptoms of spleen deficiency and phlegm accumulation, and has a better effect on the level of neurotransmitters and immune function of children, which is worthy of clinical application.

    Objective To investigate the effect of Qingqi Huatan Decoction plus budesonide on patients with acute exacerbation of chronic obstructive pulmonary disease. Methods Selected 316 cases of patients with acute exacerbation of chronic obstructive pulmonary disease from September 2020 to December 2022, divided into two groups randomly. The control group was inhaled budesonide by atomization, and the observation group was treated with Qingqi Huatan Decoction. The 1s forced expiratory volume/forced vital capacity (FEV1/FVC), maximum vital capacity (VCmax), maximum voluntary ventilation volume (MVV) and FEV1% of the two groups were measured; Airway platform pressure (Pplat), airway resistance (Raw) and airway peak pressure (PIP); Serum interleukin-8 (IL-8), monocyte chemoattractant protein-1 (MCP-1) and tumor necrosis factor-α (TNF-α). Results After treatment, the effective rate of the observation group was significantly higher than control group (P<0.05). the expected values of FEV1/FVC, VCmax, MVV and FEV1% in the two groups were significantly higher (P<0.05), and the expected values in the observation group were significantly higher than those in the control group (P<0.05). The Pplat, Raw and PIP of the two groups were significantly improved (P<0.05), and the observation group were significantly better than those of the control group (P<0.05). The serum levels of IL-8, MCP-1 and TNF-α in both groups were significantly decreased (P<0.05), the observation group was lower (P<0.05). Conclusion Qingqi Huatan Decoction plus budesonide has a significant effect on acute exacerbation of chronic obstructive pulmonary disease, which can significantly improve the level of respiratory mechanics and lung function, and reduce inflammatory reaction.

    Objective To explore the efficacy and safety of fluticasone propionate nasal spray and budesonide nasal spray combined with fluconazole topical irrigation treatment after allergic fungal sinusitis. Methods A total of 80 patients with allergic fungal sinusitis who received treatment in Xing'an League People's Hospital from 2015 to 2022 were selected as research objects. According to coin toss method, the patients were randomly divided into control group and observation group, with 40 cases in each group. The two groups underwent nasal endoscopic surgery, and budesonide group received nasal spray with fluconazole and fluticasone propionate group received fluticasone nasal spray with propionate with fluconazole for 8 weeks. The levels of interleukin-2 (IL-2), interleukin-6 (IL-6), procalcitonin (PCT), visual analogue scale (VAS), serum total immunoglobulin E (TIgE) and eosinophil cationic protein (ECP), and the occurrence of adverse reactions were compared between the two groups. Results After treatment, the levels of IL-2, IL-6 and PCT in budesonide group were lower than those in fluticasone propionate group (P<0.05). The VAS score of budesonide group was significantly lower than that of fluticasone propionate group 3 days after surgery (P<0.05). After treatment, TIgE and ECP levels in budesonide group were lower than those in fluticasone propionate group (P<0.05). The incidence of adverse reactions in budesonide group was significantly lower than that in fluticasone propionate group (P<0.05). Conclusion Compared with fluticasone propionate nasal spray, Budesonide nasal spray can significantly inhibit inflammation, relieve pain, improve allergic reactions, and has higher safety, worthy of clinical use.

    Objective To systematically assess the efficacy and adverse reactions of pirfenidone in patients with idiopathic pulmonary fibrosis (IPF) by meta-analysis. Methods Randomized controlled trials using pirfenidone as an intervention to treat IPF published from 1999 to 2022 were searched in PubmedWeb Of Science, CNKI, WanFang database and VIP database. Methodological quality evaluation and statistical analysis were conducted using Review Manager 5.3 provided by the Cochrane Collaboration. Results 13 clinical trials,involving 2088 patients, met the inclusion. The improvement of forced vital capacity (FVC), the main index of lung function, was significantly higher in pirfenidone group than in control group[SMD=1.03, 95%CI (0.81, 1.24), P<0.001]. And most of the secondary indexes, such as the change of FEV1 [SMD=0.96,95%CI (0.77, 1.16),P<0.001], the change of DLCO[SMD=0.30, 95%CI (0.09, 0.51, P=0.005]and the change of PaO2[SMD=0.57, 95%CI (0.28,0.86), P<0.001], were statistically significant; the incidence of rash between pirfenidone group and control group was statistically significant[RR=2.81, 95%CI (2.17, 3.65), P<0.001]. The incidence of adverse reactions including dizziness [RR=1.59, 95%CI (1.21,2.09), P<0.001], gastrointestinal reaction [RR=1.87,95%CI (1.65,2.12), P<0.001] were compared with the control group. Conclusion Pirfenidone can delay the decline of pulmonary function in patients with idiopathic pulmonary interstitial fibrosis and slow the progression of the disease. Adverse reactions are common, and they are mild and can be tolerated by patients.

    Objective This paper analyzes the research hotspots and frontiers of Chinese medicine health tourism development, and provides the direction and basis for the development of Chinese medicine health tourism industry in the new era. Methods The literatures about Chinese medicine health tourism published from January 1, 2000 to September 30, 2022 in CNKI database of China were searched, and CiteSpace 6.1.R2 software was used for visual analysis. Results 307 articles were included after excluding irrelevant articles, and the number of published articles basically showed an upward trend, especially in 2020. The author's analysis shows that the close cooperation density is relatively low; lack of close cooperation among domestic institutions; keywords co-occurrence analysis shows that the keywords with high frequency are Chinese medicine, Chinese medicine culture, health tourism, SWOT analysis, industrial integration, Chinese medicine industry, Chinese medicine health preservation, etc. Cluster analysis focuses on nine clusters, such as tourism of traditional Chinese medicine, tourism of health care and research, traditional Chinese medicine,rural revitalization, development mode, traditional Chinese medicine industry, resource development and personnel training. At present, the keywords with high prominence are Chinese medicine health tourism (4.86), rural revitalization (3.17), Chinese medicine tourism (2.74), health tourism (2.74), development (2.51), health tourism (2.41) and so on. Conclusion In the new era, China's TCM health tourism industry should strengthen the cooperation between domestic and foreign institutions and experts, highlight the core competitiveness of TCM health service industry, strengthen the integration and innovative development with the health tourism industry,promote the construction of national TCM health tourism demonstration zones, speed up the training of relevant talents, dig and develop TCM health tourism resources and technologies in depth, and form a new format of health tourism industry with China characteristics.

    Objective To explore the effect of evidence-based cluster nursing in premature rupture of membranes at term.. Methods 60 cases of full-term premature rupture of membranes were selected from October 2019 to October 2020, the patients were divided into control group and observation group by odd and even numbers arrangement method, there were 30 cases in each group. The control group was given routine nursing intervention and the observation group was given evidence-based nursing intervention. The postpartum hemorrhage, pregnancy outcome and neonatal outcome were observed and compared between the two groups. Results After the implementation of evidence-based cluster nursing intervention, the amount of postpartum hemorrhage in the observation group was less than that in the control group at 0.5 h, 2 h and 24 h (P<0.05) , and the natural delivery rate in the observation group was higher than that in the control group, the incidence of complications was lower in the observation group than in the control group (P<0.05) , the APGAR score was higher in the observation group than in the control group, and the neonatal outcome was better in the observation group than in the control group (P<0.05) . Conclusion Evidence-based cluster nursing care can significantly reduce postpartum hemorrhage, reduce the incidence of complications, to ensure the safety of mothers and infants, good clinical results, promote the use.

    Objective To investigate the effect of short-term preoperative ureteral stent implantation on the outcome of retrograde nephrolithotomy. Methods 90 patients with retrograde renal calculi who underwent surgical treatment in Jinan Heavy Truck Hospital from August 2019 to May 2022 were divided into the study group (n=45, receiving short-term preoperative ureteral stent implantation) and the control group (n=45, receiving conventional surgical treatment) by random number table method combined with the patient's wishes. The difference of renal function indexes and inflammatory factors before and after operation, and the comparison of perioperative complications between the two groups. Results The operation time of patients in the study group was longer than that in the control group (P<0.05), the amount of intraoperative bleeding, the time to get out of bed after operation and the retention time of nephrostomy fistula in the study group were lower than those in the control group (P<0.05). The clearance rate of the study group was slightly lower than that of the control group, and the clearance rate of multiple renal calculi was slightly higher than that of the control group, with no difference (P>0.05). The levels of serum cortisol (Cor), epinephrine (AD) and norepinephrine (NE) in the study group were lower than those in the control group on the 1st day after operation (P<0.05). At 1 and 7 days after operation, the levels of IL-4 and IL-10 in the study group were lower than those in the control group (P<0.05). The incidence of complications in the study group was, ower than the control group (P<0.05). Conclusion The short-term preoperative ureteral stent implantation for patients with retrograde renal calculi can accelerate the recovery process of patients after surgery, improve the clearance rate of multiple stones, and reduce the incidence of various complications of patients during the perioperative period.

    This study summarizes the nursing experience of one patient with spinal cord injury in our hospital who developed lung infection after cervical bone graft fusion, and then experienced ectopic ossification of the hip after pelvic fracture surgery 6 days later. The main nursing experience of patients with ectopic ossification complicated by lung infection after spinal cord injury includes: staged breathing training to improve respiratory function, glossopharyngeal breathing combined with abdominal shock to promote sputum production, early recognition and nursing intervention of ectopic ossification, empathic nursing combined with relaxation therapy to promote psychological recovery. After active treatment and nursing, the patient's condition was stable, and he recovered smoothly and was discharged from the hospital.
