Diabetic nephropathy (DN), one of the microvascular complications of diabetes, is clinically characterized by progressive decrease of albuminuria and glomerular filtration rate. In recent years, the role of podocyte injury in the pathogenesis of DN has become a focus of research. Hemodynamic abnormalities, oxidative stress, autophagy, apoptosis, mitochondrial dysfunction and lipid metabolism disorders all lead to podocyte injury. This paper discusses the relationship between podocyte injury and mitochondrial dysfunction and lipid metabolism disorder in diabetic nephropathy, and summarizes the existing Chinese medicine monomers and compound therapies targeting this pathway and protecting podocyte. The findings summarized here may provide potential therapeutic targets and strategies for the management of DN associated with podocyte injury.

Objective To investigate the expression level of long non-coding RNA MEG3 in patients with papillary thyroid carcinoma (PTC) and its relationship with clinicopathological features and prognosis. Methods A total of 78 patients who were treated for papillary thyroid cancer in my Hospital from April 2020 to March 2022 were selected as the study objects. Cancer tissue and para-cancer tissue (distance from the edge of cancer tissue>1cm) were taken from patients during operation; lncRNA MEG3 expression levels were detected by fluorescence quantitative PCR. The PTC patients were followed up for 24 months. Clinical data and clinicopathological features of patients were collected and analyzed. Receiver operating characteristic (ROC) curve was used to analyze the predictive value of MEG3 on PTC patients Kaplan-Meier and multi-factor Logistics regression analysis were used to analyze the 3-year survival of patients and the factors affecting the poor prognosis of PTC. Results MEG3 expression level in patients with PTC was lower than that in control group , and the difference was statistically significant (P<0.05); The expression level of MEG3 was correlated with tumor differentiation, TNM stage and lymph node metastasis in PTC patients (P<0.05), was not related to patient gender, age, tumor diameter, number of lesions (P>0.05), the MEG3 expression level in the poor prognosis group was lower than that in the favourable prognosis group , and the difference was statistically significant (P<0.05). The area under ROC curve of MEG3 for the diagnosis of thyroid papillary carcinoma was 0.882, the diagnostic sensitivity was 82.1%, and the specificity was 79.5%. The survival rate of patients with high MEG3 expression was significantly higher than that of patients with low MEG3 expression. Low MEG3 expression, low tumor differentiation, TNM stage Ⅲ-Ⅳ, and lymph node metastasis were independent risk factors for poor prognosis in patients with papillary thyroid carcinoma (P<0.05). Conclusion MEG3 is low expression in patients with PTC, which may be involved in the occurrence and development of PTC. MEG3 is an independent risk factor affecting the prognosis of patients with PTC, and can be used as a potential marker for the prognosis evaluation of PTC.

The theory of Three Elements in Traditional Chinese Medicine refers to the interrelationship and unity among "Heaven, Earth and Man". In Traditional Chinese Medicine theory, Heaven represents the natural environment and the laws of the universe, Earth represents the growth environment and the material basis, and Man is the subject in this system and also the core of medical attention. The three interact with each other to jointly maintain the healthy state of the human body. Meanwhile, as a theory, it also guides the treatment of human diseases in Traditional Chinese Medicine. Chronic Laryngeal Obstruction, also known as chronic pharyngitis, is a common and difficult-to-cure laryngeal disease. Its symptoms are mostly manifested as dry throat, sore throat, itchy throat, foreign body sensation in the throat, etc., which seriously affects the quality of life of patients. This article will explore the experience in treating Chronic Laryngeal Obstruction based on the "Three Elements" theory.
