    Ultrasound-guided Type Ⅱ Thoracic Nerve Block In Breast Cancer Breast-conserving Surgery and Sentinel Iymph Node Biopsy

    Objective To evaluate the analgesic effect of ultrasound-guided type Ⅱ thoracic nerve block (PECS-Ⅱ) in breast cancer breast-conserving and sentinel lymph node biopsy. Methods Sixty patients undergoing elective unilateral breast-conserving surgery and sentinel lymph node biopsy in Tianjin Medical University Cancer Hospital from June to November 2022 were selected and divided into control group and PECS-Ⅱ group, 30 patients in each group using randomized number table. Control group was under general anesthesia with conventional laryngeal mask, while group P underwent ultrasound-guided PECS-Ⅱ block before awakening after surgery. Numerical rating scale (NRS) was used to assess the pain intensity in the breast and axilla of patients at 1 h, 6 h, 12 h and 24 h postoperatively. The 15-item Quality of Recovery (QoR-15) and the recording of total opioid consumption and adverse effects were assessed at 24 h postoperatively. Results There was no statistical difference between the two groups in terms of basic patient information. Compared with control group, the NRS scores were significantly lower in PECS-Ⅱ group at 1 h and 6 h after mastectomy (P<0.05), and in the axilla at 1 h, 6 h, 12 h, and 24 h after surgery (P<0.05). The 24 h postoperative QoR-15 score was significantly higher in PECS-Ⅱ group than in control group (P<0.05). Total opioid consumption and side effects were significantly lower in PECS-Ⅱ group than in control group 24 h postoperatively (P<0.05). Conclusion Ultrasound-guided type Ⅱ thoracic nerve block significantly reduced the intensity of breast and axillary pain 24 hours after breast cancer conservation and sentinel lymph node biopsy and improved the quality of patients' early postoperative recovery.

    Objective To understand the cognition and application of biological agents among dermatologists in Yunnan province. Methods From April to June, 2022, an electronic questionnaire was developed and distributed to dermatologists in Yunnan Province for anonymous investigation. Results 103 valid questionnaires were collected. The results showed that the doctor's awareness rate of biological agents was 99.03%, the utilization rate was 72.82%, and the remarkable efficiency rate of patients after use was 98.67%. Besides, the doctor's cognitive level and application ability were uneven, and it was easy to ignore the needs of patients' health, economy and care. The guarantee system of medical institutions and social policies needed to be improved, the concept of integrated traditional Chinese and western medicine treatment still needed to be strengthened, and large samples and multi-centers were needed in the future. Conclusion Yunnan dermatologists' cognition and application of biological agents have developed to some extent, but there are still some shortcomings. In the future, it is necessary to formulate comprehensive industry and regional standards, standardize the specialist training of dermatologists, and strive to make biological agents develop in the direction of precision medicine.

    Objective To analysis the hysteroscopy results of 2 123 female infertility patients in northern Shandong. Methods Retrospective analysis was performed on the case data of 2 123 female infertility patients in northern Shandong who underwent hysteroscopy in the Department of Reproductive Medicine of our hospital from January 2015 to December 2021 due to infertility (1 426 cases), adverse pregnancy history (448 cases), history of pregnancy failure assisted by IUI (61 cases) and history of pregnancy failure assisted by IVF (188 cases). The hysteroscopy results of patients with different types of infertility, age group, number of adverse pregnancies, and number of failed pregnancies assisted by IUI/IVF were compared. Results Among 1426 patients undergoing hysteroscopy due to infertility, 686 had abnormal results, with an abnormal rate of 48.11%. There were 428 cases of primary infertility, and 240 cases were abnormal, with an abnormal rate of 56.07%. There were 998 cases of secondary infertility, and 446 cases were abnormal, the abnormal rate was 44.69% (P<0.05). The incidence of endometrial polyp, chronic endometritis and abnormal uterine development in primary infertility group was 21.73%, 21.73% and 3.97%, respectively, which were higher than those in secondary infertility group (P<0.05). The incidence of intrauterine adhesions in secondary infertility group was 9.92% higher than that in primary infertility group (P<0.05). With the increase of age, the overall abnormal rate showed a downward trend (P>0.05), and the abnormal rate of uterine development gradually decreased (P<0.05). With the increase of infertility years, the overall abnormal rate gradually increased (P<0.05). The abnormal rate of hysteroscopy increased gradually with the increase of the number of adverse pregnancies (P<0.05). With the increase of IUI-assisted pregnancy failure, hysteroscopy abnormality rate showed an increasing trend (P>0.05). The abnormal rate of hysteroscopy decreased with the increase of transplant failure (P>0.05). Conclusion The uterine abnormality rate of female infertility patients in northern Shandong is high, the overall abnormal rate of hysteroscopy is related to infertility factors, and the abnormal rate of uterine development is related to age. The duration of infertility is greater than or equal to 3 years, and the number of adverse pregnancies is greater than or equal to 3 times. Hysteroscopy is recommended.

Exercise therapy is used a form of exercise to improve the disease state. This review focuses on the effects of exercise therapy on symptoms, neurological function, gait stability, balance ability, and quality of life in DPN. It is designed to provide a basis for the rehabilitation treatment of DPN.

    Objective To investigate the clinical effect and safety of scraping and scraping along the meridian in the treatment of chemotherapy-induced leukopenia. Methods The clinical data of 50 patients with leukopenia caused by chemotherapy admitted to Rizhao traditional Chinese medicine hospital from February 2022 to December 2022 were retrospectively analyzed. 25 patients who received recombinant human granulocyte stimulating factor treatment were set as the control group, and 25 patients who received recombinant human granulocyte stimulating factor combined with Meridian Scraping Therapy were set as the observation group. White blood cell division, blood routine index, immune function, liver and kidney function and adverse reactions were compared between the two groups. Results After treatment, the leukocyte division of the observation group was lower than that of the control group (P<0.05); the white blood cell count (WBC), neutrophil count (NE), hemoglobin (Hgb) and red blood cell count (RBC) in the observation group were higher than those in the control group after treatment, and the difference was statistically significant (P<0.05); after treatment, the level of CD3+, CD4+ and CD4+/CD8+ in the observation group was higher than that in the control group, and the level of CD8+ was lower than that in the control group (P<0.05); there was no significant difference in liver and kidney function between the two groups before and after treatment (P>0.05); there was no significant difference in adverse reactions between the two groups (P>0.05). Conclusion Scraping along the meridian can improve the therapeutic effect of chemotherapy-induced leukopenia, accelerate the normalization of WBC and NE, improve leukocyte differentiation, and enhance the immune function of the body. It has no obvious effect on liver and kidney functions, and is safe and reliable.

    Objective To investigate and analyze the correlation between the level of complement C3 in knee synovial fluid and knee arthritis. Methods From July 2018 to May 2021 in the hospital of traditional Chinese medicine of knee arthritis in 126 patients as 126 cases, in the same period, 126 cases of healthy physical examination of knee arthritis were selected as the control group, the level of knee synovial complement C3 and other serological indicators, the KL grade score of the patients with knee arthritis was evaluated, and the correlation between the KL grade score and complement C3 levels were analyzed. Results The level of complement C3 in the synovial fluid of the knee arthritis group were higher than that of the control group (P<0.05). The serum levels of antinuclear antibody (ANA), C4, cyclic citrullinated peptide (CCP) in the knee arthritis group were higher than those in the control group (P<0.05), and the immunoglobulin G levels compared were no statistically significant difference between the two groups (P>0.05). In the knee arthritis group, the KL grading score were (6.33±0.52) points. Pearson correlation analysis showed that the KL grading score were positively correlated with knee synovial fluid complement C3 and serum ANA, C4, CCP(P<0.05); in the knee arthritis group, unconditioned logistic regression analysis showed that knee synovial fluid complement C3, antinuclear antibodies, serum C4, and cyclocitrulline peptides served as important risk factors affecting the KL grade score (P<0.05). Conclusion Patients with knee arthritis are mostly accompanied by high expression of complement C3 in the synovial fluid of the knee joint, which are correlated with the patient's KL grading score and are also important factors affecting the KL grading score.

    Objective To explore the application value of modified Alvarado score and abdominal ultrasound in the diagnosis of acute appendicitis in children, so as to provide relevant reference standards for early diagnosis of acute appendicitis in children. Methods 100 children with suspected acute appendicitis who were treated in Zhenba County Hospital of TCM from May 2018 to May 2022 were selected as the research objects, and ultrasonic abdominal examination and modified Alvarado score evaluation were carried out respectively. This paper mainly evaluated the diagnostic efficacy of abdominal ultrasound and modified Alvarado score for acute appendicitis based on the surgical and pathological results, and calculated the difference between the diagnostic efficacy of combined examination and that of single examination. Results Operative pathology showed that 83 out of 100 children suspected of acute appendicitis were diagnosed as acute appendicitis and 17 as non-specific abdominal pain. The diagnostic accuracy of abdominal ultrasound was 83.00% (83/100), the sensitivity was 91.25% (73/80), and the specificity was 50.00% (10/20). The ROC curve of abdominal ultrasound diagnosis of acute appendicitis was drawn, and the AUC was 0.712 (95% CI=0.556~0.855,P=0.004). The improved Alvarado score in patients with acute appendicitis was significantly higher than that in children with nonspecific abdominal pain, with a statistically significant difference (P<0.05). The ROC curve of the improved Alvarado score for the diagnosis of acute appendicitis showed that its AUC was 0.718 (95% CI=0.581~0.855, SE=0.070, P=0.005). The diagnostic accuracy of the modified Alvarado score alone was 86.00%, sensitivity was 70.59%, specificity was 66.27%, the diagnostic accuracy of abdominal ultrasound alone was 87.00%, sensitivity was 91.25%, specificity was 50.00%, and the diagnostic accuracy of the modified Alvarado score combined with abdominal ultrasound was 96.00%, sensitivity was 93.56%, specificity was 92.16%. The differences between the three methods were statistically significant (P<0.05). The sensitivity and specificity of the combined diagnosis were significantly higher than those of the modified Alvarado score and abdominal ultrasound (P<0.05). There was no significant difference between the modified Alvarado score and abdominal ultrasound alone (P>0.05). Conclusion The improved Alvarado score is slightly better than abdominal ultrasound in the diagnosis of acute appendicitis, but the accuracy, specificity and sensitivity of the combined detection are significantly better than any single detection measure. It is suggested that the combined detection should be extended to the differential diagnosis of acute appendicitis in children.

    Objective To observe the effect of Jiawei Qingzhong Decoction combined with desensitization on allergic conjunctivitis. Methods A total of 100 patients with allergic conjunctivitis (from March 2020 to March 2022) admitted to Miyun Hospital, the Third Affiliated Hospital of Beijing University of Chinese Medicine were selected and divided into control group (50 cases) and observation group (50 cases). The control group was treated with desensitization, and the observation group was treated with Jiaweiqingzhong Decoction on the basis of desensitization treatment. The therapeutic effect, inflammatory factors, tear film function and clinical symptoms were observed. Results The effective rate of observation group was higher than that of control group (P<0.05). The levels of IL-5, IL-4, IL-8 and TNF-α were not significantly different between the two groups before treatment (P>0.05). The levels of IL-5, IL-4, IL-8 and TNF-α were all decreased between the two groups after treatment, and the above indexes in the observation group were reduced more than that in the control group after treatment. The differences were statistically significant (P<0.05). Before treatment, there was no significant difference in the levels of FL, SIt and BUT between the two groups (P>0.05). After treatment, the levels of FL decreased and SIt and BUT increased between the two groups, and the change range of the above indexes in the observation group was greater than that in the control group. The differences were statistically significant (P<0.05). Before treatment, there was no statistical significance in symptom scores of conjunctival congestion, increased secretions, ocular oxygen and tears between the two groups (P>0.05). After treatment, symptom scores of conjunctival congestion, increased secretions, ocular oxygen and tears between the two groups were decreased, and after treatment, the above index scores of the observation group were lower than that of the control group. The differences were statistically significant (P<0.05). Conclusion Jiawei Qingzhong Decoction combined with desensitization treatment for patients with allergic conjunctivitis can improve the therapeutic effect, reduce inflammation, promote the improvement of tear film function and clinical symptoms, which is worth reference.

    Objective To evaluate the efficacy and safety of tovaptan in the treatment of hyponatremia after aneurysmal subarachnoid hemorrhage. Methods 68 patients with aneurysmal subarachnoid hemorrhage admitted to our hospital from January 1, 2020 to June 30, 2022 were retrospectively analyzed. According to the treatment methods of patients, 35 patients who were treated with hypertonic saline were divided into hypertonic saline group, 33 patients who were treated with tolvaptan were divided into tolvaptan group. The clinical baseline data of the two groups, changes in blood sodium level and treatment, changes in NIHSS score at different times, and the comparison of adverse reactions in the course of treatment were analyzed. Results There was no significant difference in the blood sodium level between the two groups before treatment (P>0.05). The blood sodium level in the two groups increased significantly from 24 hours after treatment to discharge, but the level in the tolvaptan group was significantly higher than that in the hypertonic saline group. At the same time, the incidence of hyponatremia in the tolvaptan group at discharge and the need to receive treatment again 24 hours later was significantly lower than that in the hypertonic saline group (P<0.05); There was no difference in the NIHSS score between the two groups before treatment, and all time point after treatment. The 24-hour tolvarutan group was slightly lower than the hypertonic saline group (P>0.05), and 3 days was significantly lower than the tolvarutan group (P<0.05), but there was no difference between the two groups at discharge (P>0.05); one patient in the hypertonic saline group increased blood sodium too rapidly and improved after intervention. There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion The treatment of hyponatremia after aneurysmal subarachnoid hemorrhage has a significant effect, which significantly reduced the blood sodium level of patients, and reduced the nerve damage of patients, with high safety.

    Objective To explore the predictive value of angiopoietin like protein 3 (ANGPTL3) and serum myostatin (Myostatin) on hypoglycemia in diabetes neonates during pregnancy. Methods 80 patients with gestational diabetes admitted to Ya'an Traditional Chinese Medicine Hospital in Sichuan Province from January 2018 to October 2022 were selected for retrospective analysis, and they were divided into the observation group. In addition, 80 healthy pregnant women volunteers who came to Ya'an Traditional Chinese Medicine Hospital in Sichuan Province for physical examination in the same period were selected as the control group. The expression levels of ANGPTL3 and Myostatin in the two groups of pregnant women were compared, and the correlation between ANGPTL3, Myostatin and gestational diabetes was analyzed. Then we collected the postpartum data of 80 cases of pregnancy diabetes patients, excluded 3 cases of premature delivery and 1 case of stillbirth, and divided the remaining 76 cases of pregnancy diabetes patients who successfully delivered the fetus into two groups according to whether the newborn had hypoglycemia, divided 50 cases of neonatal hypoglycemia into neonatal hypoglycemia group, and divided 26 cases of patients who did not have neonatal hypoglycemia into non neonatal hypoglycemia group, and compared the two groups of patients with ANGPTL3. The expression level of Myostatin and the general clinical situation were analyzed by Logistic regression analysis. Results The levels of ANGPTL3 and Myostatin in the observation group during pregnancy were significantly higher than those in the control group (P<0.05); Pearson correlation analysis showed that ANGPTL3 and Myostatin were positively correlated with the severity of gestational diabetes (P<0.05); There was no significant difference between the neonatal hypoglycemia group and the non neonatal hypoglycemia group in the history of spontaneous abortion, age, BMI before pregnancy, and gestational weeks of delivery of diabetes patients during pregnancy (P>0.05), but there were significant differences in the family history of diabetes, empty blood glucose, blood glucose 1 hour after meal, blood glucose 2 hours after meal, ANGPTL3, and Myostatin levels (P<0.05); Logistic regression analysis showed that ANGPTL3 and Myostatin were independent risk factors for hypoglycemia in diabetes neonates during pregnancy (P<0.05). Conclusion ANGPTL3 and Myostatin have a certain correlation with the occurrence and development of diabetes in pregnancy. In addition, the level of ANGPTL3 and Myostatin in pregnancy can predict the occurrence of neonatal hypoglycemia. Therefore, it is necessary to take relevant measures to prevent neonatal hypoglycemia in time during production, or to supplement glucose in time after delivery of such newborns.

    Objective To understand the development status of traditional Chinese medicine (TCM) in primary medical institutions in Enshi Prefecture and the knowledge, attitude and practice of TCM among medical staff, and analyze its influencing factors. Methods A total of 1 842 medical staff from township hospitals and community health service centers in Enshi Prefecture were selected as the survey objects by convenient sampling method. A self-designed questionnaire was used to investigate the development status of traditional Chinese medicine in hospitals and the knowledge, attitude and practice of traditional Chinese medicine among medical staff. The development status, service cognition, utilization and influencing factors of traditional Chinese medicine were analyzed by single factor and multi-factor analysis. Results The top five rankings of TCM appropriate technologies in primary medical institutions in Enshi were acupuncture (1 547, 96.75%), massage (1 417, 88.62%), acupoint massage (1 300, 81.30%), cupping (1 294, 80.93%), and traditional Chinese medicine fumigation (1 209, 75.61%). The top three problems in the clinical application of TCM characteristic technology were Westernization of TCM, which did not give full play to the advantages of TCM characteristic technology (1 116, 69.79%), insufficient human resources (878, 54.91%), and failed to master the basic theoretical knowledge of TCM (839, 52.28%). The score of TCM knowledge, attitude and practice of medical staff (65.24±14.04) was at a medium level. The results of regression analysis showed that the factors affecting TCM knowledge, attitude and practice of medical staff were gender, age, major, hospital leadership's emphasis on the development of TCM and the investment in TCM diagnosis and treatment equipment. Conclusion The development of appropriate TCM technology is unbalanced in primary medical institutions in Enshi area, and there are many problems in the clinical application of TCM characteristic technology. The total score of TCM knowledge, attitude and practice of medical staff is at a medium level. The government should strengthen the construction and investment of TCM diagnosis and treatment service capacity in the region, strengthen the training of TCM professional knowledge and skills, improve the professional quality and working level of medical staff, and create good conditions for the development of TCM in basic medical institutions.

    Objective To investigate the effect of esketamine combined with dexmedetomidine anesthesia on stress response and resuscitation quality in children undergoing laparoscopic surgery. Methods From January 2021 to January 2022, 96 cases of children who underwent laparoscopic high ligation of inguinal hernia sac in Maternal and Child Health Hospital of Inner Mongolia Autonomous Region were selected as research objects. Accorded to the 1:1 random number table, the children were divided into the combined group and the control group with 48 cases in each groups. The control group was anesthetized with ketamine, the combined group was anesthetized with dexmedetomidine on the basis of the control group, and the anesthesia conditions of the combined group and the control group were recorded, the changes of serum epinephrine and Glucagon levels at different time points, the scores of restlessness in the awakening period, and the occurrence of adverse reactions on the 7th day after operation. Results There were no significant difference in the onset time of anesthesia compared between the two groups (P>0.05). The recovery time, ketamine dosage, and hospital stay after surgery in the combined group were less than those in the control group(P<0.05). The content of serum adrenaline and glucagon in the two groups when they were sent back to the ward were significantly lower than that when they entered the operating room (P<0.05), and the content of serum adrenaline and glucagon in the combined group were also significantly lower than that in the control group (P<0.05). The PAED score of restlessness in the combined group were significantly lower than that in the control group (P<0.05). The incidence of adverse reactions, such as nausea and vomiting, respiratory depression, tachycardia, and hypoxemia at 7 d after the combination group, was 4.17%, which was not significant compared with 8.33% in the control group (P>0.05). Conclusion The application of esketamine combined with dexmedetomidine anesthesia in pediatric laparoscopic surgery can alleviate stress reaction, promote the recovery and rehabilitation of children, reduce the dosage of ketamine, improve the restlessness of children during the recovery period, and will not increase the occurrence of adverse reactions.
    The Experience of Laparoscopic Surgery for Irreducible Infantile Intussusception in 10 Cases

    Objective To summarize the experience of laparoscopic surgery in the treatment of intussusception in children with failure of air enema reduction. Methods The clinical data of 10 pediatric intussusception patients admitted to Northern Jiangsu People's Hospital from June 2018 to March 2019 were retrospectively analyzed, and the feasibility of laparoscopic surgery for pediatric intussusception after failure of air enema reduction was discussed. Results All the 10 cases of intussusception failed to be reduced by air enema were treated with laparoscopic intussusception reduction. Nine cases were successfully reduced by endoscopic intussusception, one case failed to be reduced by recurrent knot intussusception due to intestinal polyps, and the other case was repaired by expanding umbilical incision to remove diverticulum. Conclusion Laparoscopic reduction of intussusception in children is safe and reliable, and has the advantages of laparoscopic minimally invasive surgery.
