    Study of the Effect of Andolin on the Degree of Bone Marrow Suppression during Radiotherapy for Esophageal Squamous Cell Carcinoma

    Objective To investigate the effect of Anduolin capsule on myelosuppression in patients with esophageal cancer during radiotherapy. Methods 96 patients with esophageal cancer were treated with radiotherapy alone in the first affiliated Hospital of Xinjiang Medical University, the radiation dose was 50~60 Gy, among them, 46 patients took Anduolin capsule 4 capsules per time, 3 times a day, continuously until the end of radiotherapy, and the other 50 patients did not take any drugs during radiotherapy. The blood routine was every 5 days, and the changes of hemogram of the patients were observed. The level of leukocyte, hemoglobin and platelet were used to monitor the myelosuppression during radiotherapy. Results Among the 96 patients, there were 68 cases of male and 28 cases of male and female. Most of the lesions were located in the middle part of the chest, accounting for 50.0%, and the PS scores of the patients were all 1, of which the Han nationality was the most, accounting for 46.9%, followed by the Kazakh nationality (32.3%), while patients with stage Ⅰ, Ⅱ, Ⅲ and Ⅳ accounted for 2.1%, 5.2%, 59.4% and 33.3%, respectively. Most of the patients who took Anduolin capsule during radiotherapy had first-degree myelosuppression, while those who did not use any drugs mostly had second-degree myelosuppression. The white blood cell count of the patients treated with Anduolin capsule was (4.02 ±1.67) ×109/L, while that of the unused group was (3.23 ±1.34) ×109/L, and the level of platelet count was (150.26±84.46) ×109/L in the use group, and(108.70±58.56) ×109/L in the non-use group. Conclusion The use of Anduolin could reduce the reaction of myelosuppression in patients with esophageal cancer treated with radiotherapy alone, especially at the level of white blood cell count.

    Objective Newborns with suspected neonatal hemolytic disease are screened for three hemolytic diseases. To understand the distribution of HDN detection rate in the blood type, gender, birth date age and number of pregnancies of the mother. Methods Blood samples from 402 suspected ABO-HDN newborns and their mothers were collected from our hospital. Both specimens were identified for blood type and the mother was screened for irregular antibodies. Neonatal specimens were tested for hemolytic blood (direct anti-human globulin test, free antibody test, and release test) and analyzed. Results There were 222 accounting for 55.22% confirmed cases of the disease in 402 neonates with suspected ABO-HDN. The positive rates of ABO-HDN in newborns with blood type A and blood type B were 62.86% and 49.34%, respectively. The positive rates of ABO-HDN in male and female newborns were 48.06% and 63.73%, respectively. The positive rate of newborns with a birth age of≤1 day was 77.23%. The positive rate of newborns at 2~3 days was 57.46%; the positive rate of newborns >3 days was 33.33%. The positive rates of ABO-HDN in mothers with first pregnancy and non-first pregnancy were 47.40% and 61.14%, respectively. Conclusion The positive detection rate of ABO-HDN was different in children with different blood types and genders. The positive detection rate of ABO-HDN was also different according to the age of birth and the number of pregnancies.

    Objective To investigate the effect of Dengzhan Shengmai Capsule combined with perindopril tert butylamine on chronic heart failure. Methods Selected 120 cases of patients with who chronic heart failure were treated in our hospital from January 2020 to December 2020, divided into two groups according to the treatment method. The control group was treated with perindopril tert butylamine alone, and the observation group was treated with Dengzhan Shengmai Capsule. Cardiac function and symptoms were evaluated in both groups, and high mobility group1 (HMGB1), β- Endorphin(β-EP), soluble glycation end product receptor (sRAGE), N-terminal precursor of B-type brain natriuretic peptide (NT-proBNP) and transformant factor-β1(TGF-β1). Results After treatment, LVESD, SV, LVEDD, 6 min walking distance and LVEF in the two groups were significantly improved (P<0.05), and LVESD, SV, LVEDD, 6-min walking distance and LVEF in the observation group were significantly better than those in the control group (P<0.05). After treatment, the scores of asthma, palpitations, shortness of breath, edema of lower limbs, chest tightness and chest pain, and chilly cold limbs in the two groups were significantly reduced (P<0.05), and the scores of asthma, palpitations, shortness of breath, edema of lower limbs, chest tightness and chest pain, and chilly cold limbs in the observation group were significantly lower than those in the control group (P<0.05). After treatment, serum levels of HMGB1, sRAGE, NT proBNP β- EP and TGF- β 1he were significantly decreased (P<0.05). Conclusion Dengzhan Shengmai Capsule combined with perindopril tert butylamine had a significant effect in chronic heart failure, which could improve the cardiac function and TCM syndrome, and reduce the serum levels of HMGB 1, sRAGE, NT-proBNP, β -EP, and TGF- β 1.

    The occurrence and development of myocardial ischemia-reperfusion injury (MIRI) is closely related to the relationship between qi, blood and body fluid. Physiologically, qi, blood, and body fluid are the basic substances for energy metabolism of myocardial cells, and pathologically, these substances are basic factors that damage the function of myocardial cells. With the in-depth study of autophagy, it was found that the metabolic role of autophagy in ischemia-reperfusion injury myocardial cells is similar to the function of qi, blood and body fluids. The imbalance of autophagy will lead to qi deficiency and stagnation, and the lack of autophagy will lead to the accumulation of blood stasis and phlegm. Therefore, this paper constructs the research idea of "autophagy-qi, blood and body fluid-myocardial ischemia-reperfusion injury", so as to promote greater innovation and breakthrough in the prevention and treatment of MIRI by traditional Chinese medicine.

    Objective To investigate and analysis the effect of arthroscopic frozen shoulder release on serum Bone-γ-Carboxyglutamic Acid-Containing protein (BGP) and insulin-like growth factor-1 (IGF-1) expression. Methods From June 2020 to April 2022, 76 cases of elderly patients with primary frozen shoulder who were diagnosed and treated in our hospital were selected as the study subjects. According to the random number table, the patients were divided into the arthroscopic group (38 cases) and the control group (38 cases). The control group were treated with conventional closed massage, and the arthroscopic group were treated with arthroscopic frozen shoulder release. The expression of serum osteocalcin and insulin-like growth factor-1 were detected. Results At one month after treatment, the excellent and good rate of the arthroscopic group were significantly higher than that of the control group (P<0.05). The degree of shoulder joint flexion and abduction in the arthroscopic group and the control group at one month after treatment were significantly higher than that before treatment (P<0.05), and the arthroscopic group were also significantly improved compared with the control group (P<0.05). The VAS score of shoulder joint pain in the arthroscopic group and the control group at one month after treatment were significantly lower than that before treatment (P<0.05), and the score of shoulder joint pain in the arthroscopic group were also significantly lower than that in the control group one month after treatment (P<0.05). The levels of serum Bone-γ-Carboxyglutamic Acid-Containing protein and insulin-like growth factor-1 in the arthroscopic group and the control group at one month after treatment were significantly higher than those before treatment (P<0.05), and the levels of serum Bone-γ-Carboxyglutamic Acid-Containing protein and insulin-like growth factor-1 in the arthroscopic group at one month after treatment were significantly higher than those in the control group (P<0.05). The incidence rates of joint swelling, joint infection, joint hematocele and other complications at 6 months after treatment in the arthroscopic group were significantly lower than the control group (P<0.05). Conclusion The application of arthroscopic frozen shoulder release in elderly patients can improve the degree of shoulder joint flexion and abduction, improve the overall treatment excellent rate of patients, promote pain relief, reduce the incidence of complications, and increase the expression level of serum Bone-γ-Carboxyglutamic Acid-Containing protein and insulin-like growth factor-1.

    Objective To investigate the diagnostic efficacy of surfactant protein D (SP-D) combined with soluble interleukin-2 receptor (sIL-2R) in patients with cerebral infarction complicated with pulmonary infection, and to analyze the prognostic value of SP-D and sIL-2R in patients with cerebral infarction complicated with pulmonary infection. Methods 80 patients with cerebral infarction complicated with pulmonary infection admitted in our hospital from June 2019 to May 2022 were selected as the study objects. All patients were evaluated with pulmonary infection score (CPIS). 49 patients with CPIS<10 were divided into general infection group, 31 patients with CPIS ≥10 were divided into severe infection group, and 40 patients with cerebral infarction without pulmonary infection who were treated in our hospital in the same period were selected as the control group, The expression levels of SP-D and sIL-2R were compared among the three groups of patients, and the ROC was established to analyze the diagnostic efficacy of SP-D combined with sIL-2R in patients with cerebral infarction complicated with pulmonary infection. All patients were followed up for 6 months by outpatient reexamination and telephone. The patients were divided into survival group (n=58) and death group (n=22) according to their survival conditions. The general clinical conditions of patients in survival group and death group were compared, and the prognostic value of SP-D and sIL-2R for cerebral infarction complicated with pulmonary infection was analyzed and applied. Results The expression levels of SP-D and sIL-2R in the three groups were significantly higher than those in the common infection group and the control group (P<0.05); The area under the diagnostic curve of SP-D for cerebral infarction complicated with pulmonary infection was 0.852, and the best diagnostic threshold was 23.52. The area under the curve of sIL-2R in the diagnosis of cerebral infarction complicated with pulmonary infection was 0.835, the best diagnostic threshold was 404.53, and the area under the curve of the combination of the two was 0.932. The sensitivity and specificity of combined diagnosis of SP-D and sIL-2R were significantly higher than that of single diagnosis (P<0.05); there was no significant difference between the death group and the survival group in terms of sex, age, concomitant basic diseases, smoking, BMI and WBC levels (P>0.05). There were significant differences between the death group and the survival group in terms of long-term bed rest, APACHE Ⅱ score, SP-D and sIL-2R levels (P<0.05); Logistic regression analysis showed that SP-D and sIL-2R were independent risk factors for the prognosis of cerebral infarction complicated with pulmonary infection (P<0.05). Conclusion SP-D combined with sIL-2R is of high diagnostic value for cerebral infarction complicated with pulmonary infection. The combination of SP-D and sIL-2R can be considered as an auxiliary diagnostic index for cerebral infarction complicated with pulmonary infection. At the same time, SP-D and sIL-2R have high predictive value for prognosis of cerebral infarction complicated with pulmonary infection. Therefore, it is necessary to take relevant measures for patients with elevated SP-D and sIL-2R to prevent poor prognosis.

    Objective To investigate and analysis the application values of cervical vascular ultrasound in the diagnosis of collateral circulation in patients with early internal carotid artery occlusion. Methods From February 2018 to May 2022, 88 cases of patients with early internal carotid artery occlusion diagnosed and treated in Zhenba County People's Hospital were selected as study subjects. All patients were given carotid artery ultrasound examination and recorded the ultrasonic characteristics. At the same time, all patients were given digital subtraction angiography, all the cases were to record the formation of collateral circulation and judge the diagnostic value. Results There were 38 (43.18%) cases were diagnosed as collateral circulation formation by digital subtraction angiography (DSA) in the 88 cases. There were no significant difference in sex, age, course of disease, body mass index, systolic blood pressure, diastolic blood pressure, fasting blood glucose compared between the formed group and the non formed group (P>0.05). The ultrasonic blood flow velocity of vertebral artery and basilar artery in the formed group were significantly higher than that in the non formed group (P<0.05). The score of National Institute of Health Stroke Scale (NIHSS) in the formed group were significantly lower than that in the non formed group at admission (P<0.05). Among the 88 patients, 36 of them were judged to have collateral circulation by ultrasound, Kappa=0.907 (P<0.001). The sensitivity and specificity of cervical vascular ultrasound in diagnosing collateral circulation formation in patients with early internal carotid artery occlusion were 92.11% (35/38) and 98.00% (49/50). Conclusion Most patients with early internal carotid artery occlusion are accompanied by collateral circulation formation, which can improve the patient's condition and increase the blood flow velocity of vertebral artery and basilar artery. Cervical vascular ultrasound has important value in judging collateral circulation in patients with early internal carotid artery occlusion.

    Objective To investigate and analysis the efficacy and safety of oral Breast No.1 combined with external application in the treatment of hyperplasia of mammary glands with liver depression and qi stagnation. Methods From May 2021 to May 2022, 70 cases of patients with mammary gland hyperplasia of liver depression and qi stagnation type diagnosed and treated in Beijing Miyun District Hospital of Traditional Chinese Medicine were selected as the research objects. The patients were randomly divided into the combined group and the control group with 35 cases each accorded to the numerical table. The control group were treated with oral Chinese medicine, and the combined group were treated with oral Breast No.1 combined with external application. Both groups were treated and observed for 4 weeks, and recorded the effectiveness and safety of treatment, determined the changes of breast lump and breast pain score, measure the changes of breast diameter, and detected the changes of serum prolactin and estradiol. Results All patients were completed the treatment, and there were no serious adverse reactions occurred during the treatment. The total response rates in the post-treatment combination group was higher than the control group, and it was not significant between the groups (P>0.05). The scores of breast lump and breast pain, diameter of breast masses in the two groups after treatment were significantly lower than those before treatment (P<0.05), and the scores of breast lump hardness and breast pain, diameter of breast masses in the combined group after treatment were significantly lower than those in the control group (P<0.05). The levels of serum prolactin and estradiol in the two groups after treatment were significantly lower than those before treatment (P<0.05), and the combined group after treatment were also significantly lower than those in the control group (P<0.05). Conclusion Oral Breast No.1 combined with external application can inhibit the release of prolactin and estradiol, promote the reduction of breast mass diameter, alleviate the symptoms of breast mass hardness and breast pain, and improve the overall therapeutic effect in the treatment of liver qi stagnation type of hyperplasia of mammary glands.

    Objective To investigate the therapeutic effect of Xuansu Zhike Decoction combined with glucocorticoid on bronchopneumonia and its influence on procalcitonin (PCT) and hypersensitive C-reactive protein (hsCRP). Methods 100 children with bronchopneumonia admitted to Dunhuang City Hospital from January 2020 to November 2022 were selected as study subjects and divided into observation group and control group by random lottery, with 50 cases in each group. The children in the control group were treated with conventional symptomatic treatment, while the children in the observation group were treated with Xuansu Zhike Decoction combined with glucocorticoids on the basis of conventional symptomatic treatment, and their clinical efficacy, symptom disappearance time and hospitalization time were compared, as well as the expression level of inflammatory factors before and after treatment in the two groups. Results The total response rate in the observation group was significantly higher than the control group (P<0.05); the disappearance time of lung rale/wheezing, cough, total days of fever and hospital stay in the observation group were significantly lower than those in the control group (P<0.05); there was no significant difference in the expression levels of WBC, PCT and hsCRP between the two groups before treatment (P>0.05). After treatment, the expression levels of WBC, PCT and hsCRP in the two groups decreased, and the observation group was lower than the control group (P<0.05). Conclusion Xuansu Zhike Decoction combined with glucocorticoid is effective in the treatment of bronchopneumonia, which can reduce the duration of symptoms, reduce the level of inflammatory factors such as PCT and hsCRP, and promote the early recovery of children.

    Objective To investigate the effect of Suzi Jiangqi Decoction combined with budesonide on patients with chronic obstructive pulmonary disease in stable stage. Methods Selected 80 cases of patients with chronic obstructive pulmonary disease from January 2019 to December 2021, divided into two groups randomly. The control group was treated with budesonide alone, and the observation group was treated with Suzi Jiangqi Decoction. The pulmonary function [peak expiratory flow (PEF), forced expiratory volume in the first second (FEV1), maximum mid respiratory flow (MMEF), and forced expiratory volume in the first second (FEV1/FVC)] of the two groups were measured to assess the changes in quality of life, and serum interleukin-17 (IL-17), intercellular adhesion molecule-1 (ICAM-1), C-reactive protein (CRP), chemokine 13 (CXCL13), and tumor necrosis factor (TNF-α) were detected. Results The effective rate of the observation group was significantly higher than control group (P<0.05). Before treatment, there was no significant difference in PEF, FEV1/FVC, MMEF and FEV1 between the two groups (P>0.05). After treatment, PEF, FEV1/FVC, MMEF and FEV1 in the two groups were significantly increased (P<0.05), and PEF, FEV1/FVC, MMEF and FEV1 in the observation group were significantly higher (P<0.05). Before treatment, there was no significant difference between the two groups in chest distress, cough, housework, expectoration, going out, sleep, energy, climbing score and CAT total score (P>0.05). After treatment, the chest distress, cough, housework, expectoration, going out, sleep, energy, climbing score and CAT total score of the two groups were significantly reduced (P<0.05), and the observation group were lower (P<0.05). Serum IL-17, ICAM-1, CRP, CXCL13 and TNF- α in both groups before treatment has no significant difference(P>0.05), after treatment, serum IL-17, ICAM-1, CRP, CXCL13 and TNF-α decreased significantly (P<0.05), and the levels of IL-17, ICAM-1, CRP, CXCL13 and TNF-α in the observation group were lower (P<0.05). Conclusion Suzi Jiangqi Decoction combined with budesonide has a significant effect on patients with chronic obstructive pulmonary disease in stable stage, which can significantly improve lung function, improve quality of life, and reduce inflammatory reaction, and is worthy of promotion.

    Objective To investigate the efficacy of Xiyanping combined with second-generation penicillin in the treatment of scarlet fever and the effect on the positive rate of group A streptococcus. Methods 100 children with scarlet fever admitted to Jilin Children's Hospital from January 2017 to December 2020 were selected as the research object, and were divided into two groups with 50 cases in each group by random number table method. The children in the control group received routine intravenous drip of the first generation penicillin, while the children in the observation group received intravenous drip of the second generation penicillin combined with Xiyanping to compare the clinical efficacy of the two groups, the sleep quality score, scarlatina symptom score, symptom improvement and recovery time of the children after treatment, and the positive rate of group A streptococci before and after treatment and adverse reactions. Results After treatment, the total effective rate in the observation group was higher than that in the control group, and the difference was statistically significant (P<0.05). After treatment, the positive expression of GAS strain, SpeC strain and SpeA strain of emm gene in the two groups decreased significantly, and the positive expression in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). There was no difference in sleep quality scores and scarlet fever symptoms between the two groups before treatment, the difference was not statistically significant (P>0.05). After treatment,the scores of the above two indicators in the two groups had a significant downward trend, and the scores in the observation group were lower than those in the control group, and the difference was statistically significant (P<0.05).     Objective To investigate the effects of percutaneous transhepatic gallbladder puncture and drainage on serum WBC and TBIL in the treatment of gallstones. Methods 204 cases of patients with gallstones diagnosed and treated in Zhengzhou Traditional Chinese Medicine Hospital From January 2020 to May 2022 were selected as the object of the study. Accorded to the 1:1 odd even grouping principle, the patients were divided into the experimental group and the control group with 102 cases in each group. The control group were treated with laparoscopic surgery, and the experimental group were treated with percutaneous puncture and drainage of the liver and gallbladder. Compare the postoperative recovery, incidence of postoperative complications, overall treatment efficacy, and changes in serum white blood cell count and total bilirubin expression between two groups of patients. Results The postoperative anal exhaust time, solid food intake time, postoperative defecation time, and postoperative hospitalization time of the experimental group were significantly shorter than those of the control group, and the diff erence was statistically significant (P<0.05). The incidence of complications such as poor wound healing, abdominal infection, biliary fistula, bile duct injury, and perforation in the experimental group 7 days after surgery was significantly lower than that in the control group, and the difference was statistically significant (P<0.05). The total effective rate of the experimental group at 7 days after surgery was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). The serum white blood cell count and total bilirubin in the experimental group and control group were significantly lower than those on the first day before surgery, and the difference was statistically significant (P<0.05). Moreover, the experimental group also showed a significant decrease on the seventh day after surgery compared to the control group,and the difference was statistically significant (P<0.05). Conclusion Percutaneous transhepatic gallbladder puncture and drainage for the treatment of gallstones can promote patient recovery, effectively reduce the occurrence of complications, inhibit the release of serum WBC and TBIL, and improve the overall treatment effect of patients.

    Objective Investigate the international exchange willingness and pre-travel training requirements of medical postgraduates to provide a reference for serving and offering information for postgraduates. Methods From December 7 to 16, 2020, we conducted a questionnaire survey to investigate the international exchange willingness and pre-travel training requirements of medical postgraduates of West China School of Medicine, Sichuan University. Results 125 medical postgraduates participated in the survey, of which 88 percent preferred participating in international exchanges during their studies. Regarding participation in international exchanges, 77.6% of postgraduates chose joint doctoral-student training, 61.6% preferred high-level international academic conferences, and 32% chose international online exchanges. The main factors hindering international exchanges included: the epidemic (60.8 percent), difficulty in applying (23.2 percent), financial problems (22.4 percent), and conflicts in scientific research (21.6 percent). In the survey on the importance of pre-trip training for international exchange, 96.8% of graduate students thought it was important. More than half of the medical postgraduates hoped that the international exchange training would include the following contents: study-abroad policy (91.2%), study abroad planning and self-cognition (88.8%), overseas safety knowledge and practical operation (80%), destination countries education (70.4%), international perspective and cross-cultural competence (56.8%). In addition, this survey compares the willingness of international exchange modes between postgraduates with professional degrees and postgraduates with academic degrees. Compared to master's students with academic degrees, more master's students with professional degrees want to study abroad for a doctoral degree (professional degree 83.3% vs. academic degrees 45.8%, P<0.05). Meanwhile, more master's students with academic degrees were willing to attend high-level international academic conferences than master's students with professional degrees (87.5% for academic degrees vs. Academic degrees 50.0%,P<0.05). Conclusion Medical postgraduates have a strong desire to participate in international exchanges. There are various ways to participate in international exchanges. On the whole, joint doctoral-student training is the most popular mode. Compared to master's students with academic degrees, more master's students with professional degrees want to study abroad for a doctoral degree, and fewer choose to attend high-level international academic conferences. However, there are still many obstacles to international communication for medical postgraduates.
    Effect of Auricular Point Pressing Combined with Chinese Medicine Package on Postoperative Pain Relief and Prevention of Urinary Retention in Patients with Mixed Hemorrhoids

    Objective To investigate the effect of auricular point pressing combined with Chinese medicine package on reducing postoperative pain and preventing urinary retention in patients with mixed hemorrhoids. Methods 112 patients with mixed hemorrhoids treated in the Third Hospital, Shenzhen University from Jul 2021 to Dec 2021 were selected as the research object. All patients were randomly separated to two groups by the name of control group (n=56) and treated group (n=56). The control group was given postoperative routine nursing. The treated group was given ear point pressing bean combined with traditional Chinese medicine package beyond postoperative routine nursing. The first voiding time, effective rate of preventing urinary retention, VAS score were recorded and compared between treated group and control group. Results The first self voiding time and remaining urine of bladder in the treated group was shorter and less than that in the control group (P<0.05). The incidence of postoperative urinary retention in the treated group was lower than that in the control group (P<0.05). All the two groups got lower VAS scores after nursing than themselves before nursing (P<0.05). The treated group got lower VAS score than the control group after nursing (P<0.05). Conclusion Ear point beans combined with Chinese medicine package can effectively reduce postoperative pain after mixed hemorrhoids, shorten the first time of urination after operation, prevent the occurrence of urinary retention after mixed hemorrhoids.
