    Predictive Value of Homocysteine Combined with Serum miR-210-3p on Cognitive Dysfunction after Acute Ischemic Stroke

    Objective Analyze the predictive value of homocysteine (Hcy) combined with serum miR-210-3p in cognitive dysfunction of patients with acute ischemic stroke. Methods A total of 97 patients with acute ischemic stroke who received treatment in Shouguang People's Hospital from July 2020 to July 2023 were selected as the study group, and the patients were divided into cognitive dysfunction (PSCI) group (n=38) and non-PSCI group (n=59) according to the results of the Montreal Cognitive Assessment Scale (MoCA). In addition, 100 healthy people who came to our hospital for physical examination at the same phase were regarded as the control group. Results The Hcy of the study group was higher than that of the control group, while the miR-210-3p was lower than that of the control group, (P<0.05);the Hcy of the SCI group was higher than that of the non-PSCI group, and the miR-210-3p was lower than that of the non-PSCI group(P<0.05). miR-210-3p (OR=0.469, P<0.05) may be an independent protective factor for cognitive dysfunction following acute ischemic stroke; whereas Hcy (OR=25.842, P<0.05) may be a risk factor contributing to cognitive dysfunction following acute ischemic stroke. The analysis of the ROC curve shows that the areas under the curve (AUC) for Hcy, miR-210-3p, and their combined detection are 0.791, 0.744, and 0.894, respectively. The ranking from highest to lowest is: combined detection>Hcy>miR-210-3p. The sensitivity and specificity of the combined detection were 87.80% and 94.10%. Conclusion Hcy combined with miR-210-3p can predict the occurrence of PSCI after acute ischemic stroke, and provide effective risk prediction for clinical treatment.

    Objective To investigate and evaluate the nutritional status of hospitalized elderly patients, and explore the risk factors of malnutrition in hospitalized elderly patients, so as to provide basis for nutritional support for elderly patients. Methods A random sampling method was adopted to select the elderly patients who participated in nutrition consultation in the nutrition Department of Sanming Hospital of Integrated Chinese and Western Medicine from January 2023 to December 2023. The 262 elderly patients were enrolled by nutritional risk screening (NRS2002) and subjective overall assessment (SGA) for comprehensive nutritional status assessment. 262 questionnaires were distributed and 262 valid questionnaires were recovered, with an effective rate of 100%. The data of laboratory examination indicators and clinical data of patients were analyzed, and the influencing factors of malnutrition were analyzed by logistic regression. Results In this study, the incidence of nutritional risk (NRS≥3 points) and malnutrition was 81.3% and 72.9% in 262 elderly patients consulted by the Department of Nutrition. According to whether they had malnutrition, they were divided into malnutrition group (n=191) and good nutrition group (n=71). logistic multivariate analysis found that, Infection, activity, disease type and sufficient diet were the influencing factors of malnutrition in elderly patients (P <0.05). Conclusion The incidence of nutritional risk and malnutrition in hospitalized patients aged 65 and above is high. The occurrence of malnutrition in hospitalized elderly patients is related to many factors. Clinical attention should be paid to malnutrition identification and nutrition intervention as soon as possible.

    Objective To explore the pharmacological effects of Suanzaoren decoction with differential diagnosis and modification in the treatment of hypertension combined with sleep disorders. Methods From January 2022 to December 2023, 98 hypertensive patients with sleep disorders were selected from Tai'an Traditional Chinese Medicine Hospital. They were randomly divided into two groups of 49 each using a random number table. The control group received conventional Western medicine treatment during treatment, while the study group used a traditional Chinese medicine decoction called Suanzaoren decoction on the basis of Western medicine treatment, with adjustments made according to the syndrome. Compare the total effective rate and incidence of adverse reactions between the two groups, and compare blood pressure indicators, sleep indicators, psychological scores, quality of life scores, and liver and kidney function indicators before and after treatment in both groups. Results Regarding the total effective rate, the values in the study group were higher than those in the control group (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the study group and the control group (P>0.05). After treatment, systolic and diastolic blood pressure were measured, and both blood pressure were lower than before treatment, while the study group was lower than the control group(P<0.05). After treatment, various sleep data monitored at night in both groups improved compared to before treatment, and in the inter group comparison, the sleep latency of the study group was shorter than that of the control group, the actual sleep duration of the study group was longer than that of the control group, and the sleep quality score of the study group was lower than that of the control group, (P<0.05). After treatment, the anxiety score and depression score of both groups of patients decreased compared to before treatment, while the two psychological scores of the study group were lower than those of the control group(P<0.05). After treatment, quality of life in both groups, on four dimensions, was higher in both groups than in the same group before treatmentThe study group was higher than the control group (P<0.05). Regarding liver and kidney function indicators, there was no statistically significant difference between the two groups before and after treatment (P>0.05), and there was no statistically significant difference between the groups (P>0.05). Conclusion When treating patients with hypertension and sleep disorders, the use of Suanzaoren Tang has significant therapeutic effects. It can not only control blood pressure levels, but also improve patients' sleep conditions, alleviate sleep disorders, and help improve their psychological and quality of life. Moreover, the combination of Western medicine and Suanzaoren Tang does not increase the risk of adverse reactions and has good safety.

    Objective To explore the application effect of straight leg elevation training and early lumbar and dorsal core muscle strength training in patients undergoing vertebroplasty. Methods A total of 116 patients who were underwent vertebroplasty at the Third People's Hospital of Jinan City, Shandong Province from January 2023 to December 2023 were selected, and randomly divided into the study group (n=58) and the control group (n=58). The control group was received routine rehabilitation training, while the study group was received straight leg elevation training and early lumbar and dorsal muscle nucleus myocardial strength training. The spinal function, pain level, balance and exercise ability, and daily living ability in both groups were compared. Results Before intervention, in the Oswestry Disability Index (ODI) and Japanese Orthopaedic Association (JOA) scores in both groups, there was no statistically significant difference (P>0.05). After intervention, in comparison of the control group, the ODI scores of the study group were lower, and JOA scores were higher, with statistically significant differences (P<0.05). Before intervention, there was no statistically significant difference in the visual analog scale (VAS) scores in both groups (P>0.05). After intervention, in comparison of the control group, the VAS score of the study group was lower, and the difference between the groups was statistically significant (P<0.05). Before intervention, there was no statistically significant difference in the scores of the Berg balance scale (BBS) and the lower section of the Fugl Meyer assessment (FMA-LE) in both groups (P>0.05). After intervention, in comparison of the control group, the BBS and FMA scores of the study group were higher, with statistically significant differences (P<0.05). Before intervention, there was no statistically significant difference in the Barthel index (BI) in both groups (P>0.05). After intervention, in comparison of the control group, the study group was higher, and the difference was statistically significant (P<0.05). Conclusion The combination of straight leg elevation training and early lumbar and dorsal core muscle strength training can improve spinal function, reduce pain, enhance balance and mobility, and improve daily living abilities in patients undergoing vertebroplasty.

    Objective Based on the theory of Emergent Diarrhea and Tenesmus,All Are Fever, to explore the clinical efficacy and safety of acupressure combined with traditional Chinese medicine umbilical cord compresses in the treatment of pediatric rotavirus enteritis with damp-heat diarrhea. Methods The 80 infants with wet hot diarrhoea who were confirmed as RV enteritis in Pediatrics Clinic and In-Patient Section of Wuhu City Hospital of Traditional Chinese Medicine between Sept 2023 and Feb 2024 were classified into two groups:1 and 2,respectively, and 40 patients.The control group was treated with pediatric diarrhea stop granules orally, and the observation group was treated with finger acupoint massage combined with traditional Chinese medicine umbilical cord compresses, with the same basic treatment for both groups. Both groups were treated with the same basic treatment.5 consecutive days of treatment was one course of treatment. The total effective rate of Chinese medicine symptoms,the improvement level of Chinese medicine symptoms points before and after the treatment, the recovery time of clinical symptoms and signs, the conversion rate of rotavirus, the improvement of quality of life score and the occurrence of adverse reactions were compared between the two groups. Results Compared with the control group, the effective rate of 95.00% was higher than 87.50%, and the difference was significant(P<0.05); the improvement of TCM symptom score, the recovery time of symptoms and signs, rotavirus turnover rate, QOL and QOL were significantly different(P<0.05 or P<0.001).In addition to 3 patients with vomiting in the control group, there were no side effects in the two groups, but only 3 patients with nausea. Conclusion Based on the theory of Emergent Diarrhea and Tenesmus, All Are Fever, applying acupoint massage combined with traditional Chinese medicine rotavirus enteritis with damp-heat diarrhea on the basis of conventional treatment in western medicine has remarkable efficacy, with faster symptomatic relief, accelerated conversion of rotavirus antibody to negative, good adherence, significant improvement in the quality of life and safety.

    Objective To explore the effect of Cathepsin B on the proliferation, migration and invasion of papillary thyroid carcinoma TPC-1 cells. Methods The siRNA of targeting Cathepsin B was transiently transfected into TPC-1 cells and Western blot was used to detect the expression of Cathepsin B. Cell counting kit-8, colony formation assay, Wound-healing migration assay and transwell assay were used to detect cell proliferation, migration and invasion. Results Western blot results showed that the expression of Cathepsin B in the experimental group was significantly lower than that in the blank group and the negative control group, and the difference was statistically significant. After silencing Cathepsin B, the ability of cell proliferation, migration, and invasion were significantly weakened, and the difference was statistically significant. Conclusion This study confirmed that silencing Cathepsin B with SiRNA significantly decreased the proliferation, migration, and invasion of TPC-1 cells, indicating that Cathepsin B may be an effective target for papillary thyroid carcinoma gene therapy.

    Objective To analyze the modeling elements of experimental animal models of vascular dementia, and to provide reference for the establishment of stable and reliable animal models of vascular dementia experimental research. Methods With "vascular dementia + animal model" as the theme, the experimental studies on vascular dementia were searched from major databases from January 1, 2011 to October 1, 2021, respectively. Statistical analysis was carried out on related modeling methods and index detection in literature. Results A total of 148 valid literatures were screened. Male Wistar rats and male SD rats were mostly selected for VD model animals, and their body weight ranged from 200g to 300g. Permanent ligation of bilateral common carotid arteries was usually used for modeling. Morris water maze was used to detect animal behavior changes, oxidative stress and cholinergic nerve indexes, and pathological state of brain and hippocampal tissues, which reflected the pathophysiological state of VD animal models. Conclusion In the future, we should focus on optimizing the modeling method to improve the success rate of modeling and reduce the mortality rate.To establish the animal model of vascular dementia with Chinese and western medicine.

    Yi medicine is one of China's intangible cultural heritages, which has a similar philosophical basis and treatment concepts with Chinese medicine, and there are many similarities between the two in clinical practice. This paper explores the commonalities and differences between Yi medicine and Chinese medicine through a comparative analysis of the two, and analyzes the advantages and problems in the development of the intermingling of Yi medicine and Chinese medicine in Yuxi. It proposes strategies to strengthen the intermingling development of Yi medicine and traditional Chinese medicine(TCM), aiming to promote the inheritance and development of Yi medicine in Yuxi, as well as the exchange and cooperation between TCM and local medicine.

    Objective To explore the practice and effect of ideological and political teaching in the course of nursing etiquette. Methods A total of 116 freshmen of the nursing major in our college were randomly selected as the research objects. The ideological and political education was carried out in the teaching of nursing etiquette course. The “two-main-lines and four-dimensions” teaching idea of ideological and political course, the “three-links and six-steps” implementation plan, and the online and offline mixed teaching and the integration of theory and practice were adopted in the teaching. The ideological and political elements and cases were appropriately integrated into the teaching according to the content of the teaching knowledge points, and the handshake etiquette teaching was taken as an example. Finally, the comprehensive evaluation of the students’ grades was carried out through the process evaluation, the summative evaluation, and the educational evaluation. After one semester, a self-made questionnaire was used to investigate the teaching effect of ideological and political education. Results After one semester of teaching practice, 114 of the 116 students had a comprehensive score of 80 or above, accounting for 98.28%; the overall evaluation of the teaching effect of ideological and political education was excellent in 98 students, accounting for 84.48%. Conclusion The organic integration of professional course teaching and ideological and political education has improved the ideological and political teaching ability of professional course teachers, established a new path for digital ideological and political education, formed an evaluation index system for the teaching quality of ideological and political education, and at the same time, improved students’ learning confidence and sense of professional responsibility in the nursing profession, laying a foundation for the formation of good nursing professional ethics.

    Objective The purpose of this study is to evaluate simulation based courses to enhance the technical performance and interest level of clinical medical undergraduate students in vascular surgery. Method A total of 41 undergraduate clinical medical students who interned in our department from October 2020 to March 2022 were included in the interest survey and pre tested on an intravascular simulator. During the internship, I guided simulator courses on a weekly basis and completed the final testing on the simulator. The simulator determines Objective procedures before and after testing, and vascular surgery teachers use a structured global evaluation scale to score subjective performance. After the course, the students received a survey of opinions on vascular surgery as a career choice. Finally, six months after the course ends, a survey will be conducted again among all students to determine their interest in vascular surgery. Results In terms of diagnostic time, stent deployment time, and fluoroscopy time, there was a significant statistical difference (P<0.05). At the same time, there was also a significant statistical difference in stent release effect indicators (P<0.05). More than 95% of students reported that intravascular simulators have increased their knowledge and interest in vascular surgery. Conclusion Receiving guidance on endovascular surgery on a simulator can have a positive impact on the attitude of clinical medical undergraduate students towards vascular surgery. Simulator based courses may be an important component of vascular surgery training programs.
    A Survey on Cognition, Attitudes and Demands Status of Birth Plan among Pregnant Women

    Objective To explore the cognition, attitudes and demands status of birth plan, and to provide the basis for making scientific birth plan. Methods Based on simple random sampling sampling, a total of 310 pregnant women were selected to explore the status of the cognition, attitudes and demands of birth plan and analysis the influencing factors and correlation. Results A total of 289 valid questionnaires were collected. 73.7% of pregnant women had never heard of birth plan before , and all pregnant women had not made birth plan. 54.7% of pregnant women hoped to make a delivery plan. The influencing factors of pregnant women’s attitude towards birth plan were age, education level, occupation, planned pregnancy and previous knowledge of birth plan (P<0.05). There were 20 higher plan demands and 6 lower plan demands. Conclusion The cognition of pregnant women to birth plan is generally low. Nursing managers should pay attention to the health education training of midwives and antenatal care personnel, to improve the childbirth experience and satisfaction of pregnant women.

    Objective To observe the preventive effect of Xinhuang tablets mixed with vinegar on Oxaliplatin-induced peripheral neurotoxicity. Methods A total of 122 patients with gastrointestinal malignancy treated with oxaliplatin chemotherapy admitted to the Department of Traditional Chinese Medicine of a Class III Grade A hospital were randomly divided into two groups: control group (61 cases) and observer group (61 cases). The control group was treated with chemotherapy nursing routine, while the intervention group was treated with Xinhuang Tablet dissolved in vinegar on the local skin around the infusion site on the basis of chemotherapy nursing routine. The incidence of peripheral neurotoxicity, toxicity grade and TCM syndrome score of the two groups were observed. Results Before intervention, there was no significant difference in the incidence of peripheral neurotoxicity between the two groups (P>0.05). After intervention, there was significant difference in the incidence of peripheral neurotoxicity between the two groups (P<0.05). Before intervention, there was no significant difference in the peripheral neurotoxicity grade between the two groups (P>0.05). After intervention, there was significant difference in the peripheral neurotoxicity grade between the two groups after intervention (P<0.05). Before intervention, there was no significant difference in the total score of TCM symptoms, numbness of limbs, cold of hands and feet, mental fatigue, dizziness and adverse flexion and extension scores between the two groups (P>0.05). After intervention, there were statistically significant differences in the total score of TCM symptoms, numbness of limbs, cold of hands and feet, mental fatigue, dizziness and adverse flexion and extension scores between the two groups (P<0.05). Conclusion External application of Xinhuang Tablets mixed with vinegar can reduce the incidence of Oxaliplatin-induced peripheral neurotoxicity in patients with gastrointestinal malignant tumor chemotherapy, reduce its severity, and obviously improve the TCM syndrome of "cold coagulation blood stasis type " blood arthralgia, which is convenient and feasible.

    Objective To provide hierarchical chain care joint intervention for children with neonatal pneumonia and explore its application effect. Methods A total of 66 children with neonatal pneumonia treated in the neonatology department of Chifeng Hospital from June 2022 to December 2023 were selected and randomly divided into control group (33 cases, conventional care) and experimental group (33 cases, hierarchical chain nursing combined intervention) by envelope drawing method. T-test was used to compare the time of symptom disappearance, length of hospital stay, physical stress and oxygenation indexes of the two groups, and χ2 test was used to compare nursing satisfaction. Results The disappearance time of fever, cough, lung rales, chest X and shadow were shorter than those of the control group (P<0.05); Oxyoxygen saturation (SpO2), heart rate (HR) and respiration (R) were higher than / lower than the control group (P<0.05), The oxygen partial pressure (PaO2) and oxygenation index (PaO2/ FiO2) were higher than the control group (P<0.05), The difference between alveolar-artery oxygen (A-aDO2) was lower than that of the control group (P<0.05); The nursing satisfaction of the children was higher than that of the control group (P<0.05). Conclusion The hierarchical chain nursing consortium intervention has important value in neonatal pneumonia, which can accelerate the resolution of pneumonia symptoms in children, and promote the improvement of pulmonary oxygenation function to a certain extent, shorten the length of hospital stay, and improve nursing satisfaction.

    Objective To investigate the effects of family simultaneous empowerment education combined with food diary on dietary behavior and nutritional status of maintenance hemodialysis patients. Methods A total of 102 maintenance hemodialysis patients treated in Yantai Laiyang Central Hospital from January 2022 to January 2024 were selected and divided into control group and experimental group by random number table method, with 51 cases in each group. The control group was given routine diet management, and the experimental group was given family members simultaneous empowerment education combined with food diary intervention for 3 months. The scores of dietary compliance behavior, nutritional status and quality of life of the two groups were compared. Results The total score of dietary compliance behavior in experimental group was higher than that in control group (P<0.05). Muscle circumference and skin fold thickness of upper arm were higher than those of control group (P<0.05). Hemoglobin (Hb), albumin (ALB) and prealbumin (PA) were higher than those in control group (P<0.05). The quality of life score was higher than that of control group (P<0.05). Conclusion Simultaneous empowerment education combined with dietary diary intervention can promote the development of dietary compliance behavior, improve nutritional status and quality of life of patients with maintenance hemodialysis.

    Objective To explore the influencing factors of venous thromboembolism (VTE) in patients with cerebrovascular disease. Methods A retrospective analysis was conducted to collect data from 100 patients with cerebrovascular diseases who visited Chifeng Municipal Hospital from January 2021 to December 2023. The incidence of VTE during hospitalization was organized and recorded, and the patients were divided into VTE group and non VTE group. The clinical data and laboratory test results of the patients were sorted out and recorded, and the influencing factors of VTE in patients with cerebrovascular disease were analyzed. Results 24 out of 100 patients with cerebrovascular disease developed VTE, with an incidence rate of 24% (24/100). Preliminary comparison of data between VTE group and non VTE group patients revealed that age, previous VTE history, bed rest time may be influencing factors for VTA in cerebrovascular disease patients (P<0.05). Through logistic regression analysis, the results showed that age≥60 years old, previous VTE history, bed duratio ≥72 h were the risk factors for VTE in cerebrovascular disease patients (P<0.05), and all were risk factors (OR>1). Conclusion Cerebrovascular disease patients with age≥60 years old,previous VTE history, and bed time≥72 h may have the risk of VTE, which should be paid attention to in clinical practice.
