    Cartilage is vulnerable to be damaged by internal and external environmental, but its self-repair ability is limited due to the lack of nerves, blood vessels and lymphatic tissues. In this process, longevity gene silencing information regulator 1 (SIRT1) plays an important role. After cartilage damage, cartilage anabolism and catabolism are disordered. SIRT1 gene can regulate the process of cartilage damage by influencing chondrocyte inflammation, oxidative stress and other factors. In the molecular mechanism, SIRT1 gene plays an important role in regulating cartilage homeostasis, which can be mediated by Notch, BMP, Wnt/ β-Catenin and other signal pathways that play a regulatory role in cartilage damage repair. In this review, we illustrate the protective effects of SIRT1 on cartilage under pathophysiological conditions and its possible molecular biological mechanisms, providing theoretical support for research related to restoring cartilage homeostasis and achieving cartilage metabolic balance.

    Objective To explore the clinical efficacy of Buzhong Yiqi Decoction plus acupoint intradermal needling combined with chemotherapy for advanced and driver gene test negative lung adenocarcinoma. Methods A total of 60 patients with advanced unresectable lung adenocarcinoma who were treated in Xu Yi People's Hospital from January 2020 to December 2021. They were randomly divided into two groups, the observation group and the control group. The control group was treated with standard western medicine chemotherapy and immunotherapy, and the treatment group was treated with the control group Combined with Buzhong Yiqi Decoction Modified Recipe and acupoint intradermal needling, improvement of clinical symptoms, quality of life score, tumor marker level and adverse reactions of the two groups were observed. Results The improvement rates of chest tightness, fatigue and anorexia in the treatment group were significantly higher than those in the control group (P<0.05); the KPS scores of the quality of life after treatment were improved compared with those before treatment (P<0.05), but the KPS of the treatment group after treatment was better than the control group (P<0.05); after treatment, the serum CEA and CYFRA21-1 values in the treament group decreased more than those in the control group (P<0.05); the incidence of dizziness, nausea, vomiting and bone marrow suppression in the control group was significantly higher than that in the treament group (P<0.05). Conclusion In the treatment of advanced unresectable and driver gene test negative lung adenocarcinoma patients, on the basis of conventional chemotherapy and immunotherapy, combined with Buzhong Yiqi Decoction plus acupoint intradermal needling can effectively relieve the clinical symptoms of patients, improve the quality of life and reduce the Toxic effects caused by chemotherapy, so have promotion value.

    Objective To investigate the effect of zopiclone combined with trazodone therapy on sleep structure in elderly patients with depression. Methods 96 elderly depression patients admitted to Beijing Changping District Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to December 2021 were selected, and divided into two groups according to the random number table method. The control group of 47 patients were treated with trazolone alone, and 49 patients in the experimental group were treated with zopicone and trazolone. The sleep structure of the elderly patients with depression and the effeetive rate in the two groups was detected, and the levels of serum dopamine (DA), norepinephrine (NE), cortisol and serotonin (5-HT) were detected in the two groups. Results The response rate of the experimental group was slightly higher than that of the control group, but the differences were not significant (P>0.05). Before treatment, there were no significant differences in sleep efficiency, total sleep time, REM sleep period, sleep onset latency, non-REM sleep period, number of awakenings and awakening time between the two groups (P>0.05). After treatment, the sleep efficiency, REM sleep period, total sleep time, and non-REM sleep period of the two groups were significantly increased (P<0.05), and the sleep-onset latency, number of awakenings and awakening time were significantly decreased (P<0.05). The sleep efficiency, REM sleep period, total sleep time, and non-REM sleep period of the observation group were significantly higher than those of the control group (P<0.05), and the sleep onset latency, number of awakenings and awakening time were significantly lower than those of the control group group (P<0.05). After treatment, the levels of serum DA, NE and 5-HT in the two groups were significantly higher than those before treatment (P<0.05), and the levels of cortisol were significantly lower than those before treatment (P<0.05). After treatment, there were no significant differences in the levels of serum DA, NE and 5-HT between two groups (P>0.05). After treatment, the cortisol level in the experimental group was significantly lower than that in the control group (P<0.05). Conclusion Zopiclone combined with trazodone has a significant effect on elderly patients with depression, and can effectively improve their sleep structure, which is worthy of application.

    Objective To observe the clinical effect of high and low frequency repetitive transcranial magnetic stimulation (rTMS) on patients with post stroke cognitive impairment (PSCI). Methods 60 PSCI patients hospitalized in Jiangsu Civil Affairs Rehabilitation Hospital and the Second Affiliated Hospital of Nanjing Medical University from March 2017 to March 2022 were selected and divided into treatment group and control group according to random number table.There were 30 patients in each group. The control group was treated with oral memantine hydrochloride tablets and conventional rehabilitation therapy. The treatment group was treated with high and low frequency rTMS on the basis of the control group. Before and after treatment, the patients were evaluated with Mini mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA), and modified Barthel index (MBI), and the data were recorded and analyzed statistically. Results After treatment, the MMSE scores of patients in the treatment group were higher than those in the control group, with a statistically significant difference (P<0.05); after treatment, the Moca score of patients in the treatment group was higher than that in the control group, with a statistically significant difference (P<0.05); after treatment, the MBI score of the treatment group was higher than that of the control group, with a statistically significant difference (P<0.05). Conclusion High and low frequency rTMS alternative treatment can improve the cognitive function of PSCI patients more quickly and improve the ability of daily living, which is worthy of further clinical application.

    Objective To investigate the effect of dexmedetomidine combined with regional nerve block of lumbar quadratus muscle on postoperative delirium in elderly patients with colon cancer. Methods Eighty elderly colon cancer patients admitted to the Affiliated Hospital of Xuzhou Medical University from January 2021 to June 2022 were selected for retrospective analysis and grouped according to whether dexmedetomidine was applied in the pre-induction of anesthesia. 40 patients in the observation group had dexmedetomidine applied prior to the induction of anesthesia, while 40 patients in the control group were not applied. All patients underwent radical colon cancer, the control group patients received general anesthesia combined with trapezius regional nerve block, and the observation group increased dexmedetomidine before induction of general anesthesia combined with trapezius regional nerve block, and compared the expression levels of diastolic blood pressure (SBP), systolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR) in different time [base value (T1), sedation (T2), lens(T3), end (T4)], and the scores of visual analogue scale (VAS) under rest and exercise at different times between the two groups, adverse conditions and false incidence incidence. Results There was no significant difference in SBP, DBP, MAP and HR at T1 between the two groups (P>0.05). There was no significant difference in SBP, DBP, MAP and HR at T1, T2, T3, T4 between the two groups (P>0.05). The levels of SBP, DBP, MAP and HR decreased from T1 to T2, and T4 increased from T3, SBP, DBP and MAP were higher than the control group, and SBP, DBP, and HR were lower than the control group (P<0.05); the resting state VAS score increased between 2 h and 12 h after surgery, and the observation group was lower (P<0.05); the exercise state VAS score gradually decreased from 12 h to 48 h after surgery, and the observation group was lower (P<0.05); the postoperative pain duration was significantly lower than the control group (P<0.05). There was no significant difference in the incidence of bradycardia, hypoxemia, hypotension and other adverse events between the two groups (P>0.05). The incidence of postoperative delirium of observation group was significantly lower than that of the control group (P<0.05). Conclusion The use of dexmedetomidine followed by general anesthesia combined with lumbar quadratus block in elderly patients with colon cancer can stabilize their blood pressure and heart rate, reduce the degree of pain in resting state and exercise state, reduce the incidence of postoperative delirium, and has a high safety.

    Objective To observe the effect of hyperbaric oxygen rehabilitation technology on traumatic brain injury and its effect on patients' stress response and cognitive function. Methods 112 patients with traumatic brain injury admitted to Heze municipal hospital from August 2020 to January 2022 were selected and divided into the comparison group (56 cases) and the study group (56 cases) by drawing lots. The two groups of patients received routine treatment first. The study group was treated with hyperbaric oxygen rehabilitation technology. Functional independence scale (FIM) was used to evaluate their independent living ability, neurobehavioral cognitive scale (NCSE) was used to evaluate their cognitive function, and malondialdehyde (MDA), superoxide dismutase (SOD), glutathione peroxidase (GSH-Px) and other indicators were detected by enzyme immunoassay to evaluate their stress response. The clinical cure rates of the two groups were statistically analyzed and compared. Results Before intervention, there was no difference between the two groups in FIM score, NCSE score and oxidative stress response (P>0.05). After intervention, the FIM and NCSE scores of the study group were significantly higher than those of the control group (P<0.05). After intervention, the level of SOD and GSH-PX of the two groups were increased and the level of MDA of the two groups were decreased (P<0.05); the level of SOD, GSH-PX and MDA of the study group were higher than those of the control group (P<0.05). The clinical cure rate of the study group was higher than the control group (P<0.05). Conclusion Hyperbaric oxygen rehabilitation is effective in the treatment of traumatic brain injury, which can effectively reduce stress response and improve cognitive function of patients.

    Objective To explore and analyze the diagnostic efficacy of Philips 64-slice spiral CT in pulmonary nodules of different sizes, so as to promote the application of Philips 64-slice spiral CT. Methods From August 2017 to March 2022, 86 patients with large and small pulmonary nodules were selected for diagnosis and treatment in Beijing Fengtai Hospital of Integrated Traditional Chinese and Western Medicine, included 40 cases of pulmonary nodules with a diameter of ≤ 1 cm (small nodule group), 46 cases of pulmonary nodules with a diameter of 1-3 cm (large nodule group). All patients were examined by Philips 64-slice spiral CT, and the routine CT features, CT values, CT perfusion parameters, nodule malignancy rate and diagnostic value of the two groups were recorded. Results The air bronchus sign, pleural stretch sign, vacuole sign, vascular aggregation sign, lobulation sign, burr sign and other CT features in the large nodule group accounted for 80.43%, 86.96%, 89.13%, 84.78%, 89.13%, 80.43%, which were significantly higher than 30.00%, 35.00%, 45.00%, 52.50%, 35.00%, and 50.00% in the small nodule group (P<0.05). The CT values of the large nodule group were significantly higher than those of the small nodule group (P<0.05). The bronchial artery perfusion value and pulmonary artery perfusion value in the large nodule group were significantly higher than those in the small nodule group (P<0.05), and there was no significant difference in the perfusion index between the two groups (P>0.05). The malignant rate of the large nodule group was 80.43% (37/46), included 20 adenocarcinomas, 13 squamous cell carcinomas, and 4 small cell carcinomas; the malignant rate of the small nodule group was 30.00% (12/40), of which there were 6 cases of adenocarcinoma, 4 cases of squamous cell carcinoma, and 2 cases of small cell carcinoma, there were significant differences in pairwise comparison (P<0.05). Conclusion The application of Philips 64-slice spiral CT in pulmonary nodules of different sizes can reflect the pathological status of the nodules through the imaging morphological features and perfusion characteristics, and has a good diagnostic performance, it can help the qualitative diagnosis of pulmonary nodules of different sizes.

    Objective To investigate the adverse reaction analysis and clinical evaluation of docetaxel combined with different antineoplastic drugs in breast cancer. Methods A retrospective study was conducted to select 78 breast cancer patients from March 2020 to March 2021 in the Affiliated Cancer Hospital of Shantou University Medical College. According to different treatment schemes, they were divided into three groups: TH (Docetaxel+Trastuzumab) group with 24 cases, TE (Docetaxel+Epirubicin) group with 25 cases, and TEC (Docetaxel+Epirubicin+Cyclophosphamide) group with 29 cases. The clinical efficacy, adverse reactions, serum biochemical index and 1-year survival rate of the three groups were compared. Results The therapeutic of TH group was significantly higher than that of TE group (P<0.05); the therapeutic of TEC group was significantly higher than that of TE group (P<0.05); the difference of curative effect between TH group and TEC group was not statistically significant (P>0.05). The adverse reactions of the three groups were mild, and there was no significant difference among the three groups (P>0.05). Compared with the 1-year survival rate of the three groups, the differences were statistically significant (P<0.05), among which the differences were statistically significant (P<0.05). The 1-year survival rate of TEC group was significantly higher than that of TE group (P<0.05), but there was no statistical difference between the other two groups (P>0.05). There was no significant difference in FSH and E2 between the three groups (P>0.05). Conclusion The efficacy and safety of Docetaxel combined with Trastuzumab is similar to that of Docetaxel combined with Epirubicin and Cyclophosphamide in the treatment of breast cancer, both of which are superior to those of Docetaxel and Epirubicin. Moreover, Docetaxel combined with Epirubicin and Cyclophosphamide has the best effect in inhibiting tumor cell proliferation, and the survival rate is higher than that of Docetaxel and Epirubicin, which can be selected according to the patient's condition.

    Objective To explore the effect of Ginkgo Damo Injection combined with Betahistine Mesylate Tablets on vertebrobasilar artery blood supply and blood flow indexes after the treatment of circulating ischemic vertigo. Methods 80 patients with posterior circulation ischemic vertigo who were admitted to Dongchangfu People's Hospital from September 2018 to September 2020 were randomly selected for research, and divided into control group and experimental group by random number table method, with 40 cases in each group. The control group was treated with Betahistine Mesylate Tablets, the experimental group was treated with Ginkgo Damo Injection combined with Betahistine Mesylate Tablets, the therapeutic effect was compared, and the blood flow velocity, hemorheology index, adverse reactions on both sides of the vertebral body and vertebral base rate and degree of vertigo disability were compared. Results The total effective rate of experimental group was higher than that of control group(P<0.05); after treatment, the blood flow velocity of left vertebral artery, right vertebral artery and basilar artery in the two groups were higher than before (P<0.05), and the experimental group was higher than the control group (P<0.05). After treatment, the whole blood high shear viscosity, whole blood low shear viscosity, plasma viscosity and fibrinogen in two groups were lower than before (P<0.05), and the experimental group was lower than the control group (P<0.05). The total incidence of adverse reactions such as rash, nausea and vomiting, headache and head distension in experimental group had no difference with that in control group (P>0.05). After treatment, somatic, emotionality and functional scores of DHI scale in 2 groups were lower than before (P<0.05), and inter-group comparison of experimental group was lower than control group (P<0.05). Conclusion In the treatment of patients with posterior circulation ischemic vertigo, Ginkgo Damole Injection combined with Betahistine Mesylate Tablets has outstanding therapeutic effect and few adverse reactions, and can effectively improve the vertebrobasilar artery blood flow velocity, hemorheology and vertigo disability degree, it is worth learning.

    This article reviews the focus of comprehensive management content of MAFLD, the mode of comprehensive management and the application of theoretical tools. It emphasizes the importance of screening and assessment of high-risk groups and treatment based on lifestyle intervention in health management, and expounds the management mode of “grading, stratification, whole process”, “multidisciplinary and individualized”. The article discussed the shortcomings in the comprehensive health management of MAFLD at the current stage, which may provide a basis for the prevention and management strategy, hierarchical diagnosis and treatment and clinical pathway establishment of MAFLD.

    Objective To investigate the current situation of ideological and political courses of professional teachers in colleges and universities, and to provide a theoretical basis for improving their ideological and political ability and quality of courses. Methods This research is a cross-sectional survey, using the convenience sampling method, using the “Factual Survey Questionnaire on the Ideological and Political Practice of Professional Course Teachers in Colleges and Universities”, through questionnaires and interviews, 671 students are surveyed and analyzed and summarized. Curriculum ideological and political problems to be solved. Results 83.61% of the students believed that the school combined ideological and political education to carry out professional courses, 80.33% of the students knew that the school offered general courses with the significance of ideological and political education, and 39.34% of the students said that most of the examinations in the professional courses had evaluated students' thoughts, attitudes and values.     Objective To analyze the efficacy of sacubitril/valsartan in patients with chronic heart failure and its effect on cardiac function, serum sirtuin-4 (Sirtuin-4) and Beclin-1 levels. Methods Sixty chronic heart failure patients treated from January 2019 to September 2021 in Inner Mongolia International Mongolian hospital were investigated. According to random number table, patients were divided into control group (n=30, benazepril group) and observation group (n=30, sacubitril/valsartan group). The levels of left ventricular end systolic volume (LVESV), left ventricular end diastolic volume (LVEDV), left ventricular ejection fraction (LVEF), serum sirtuin-4 and beclin-1 before and after treatment, the total effective rate, the number of hospitalizations, the average length of stay and the incidence of adverse reactions were compared between the two groups. Results The effective treatment rate in the observation group was higher than control group (P<0.05). Before treatment, the levels of LVEF, LVESV and LVEDV between the two groups were not significantly different (P>0.05), and after treatment, the levels of LVEF, LVESV and LVEDV in the two groups were higher than those before treatment, and the levels of LVEF, LVESV and LVEDV in the observation group were higher than those in the control group, and the differences were statistically significant (P<0.05). Before treatment, serum Sirtuin-4 and Beclin-1 levels between groups were not significantly different (P>0.05); after treatment, serum Sirtuin-4 and Beclin-1 levels in the observation group were lower than control group (P<0.05). The number of hospitalizations and the average hospitalization days in the observation group were less than those in the control group (P<0.05). There was no difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Sacubitril/valsartan has a definite curative effect in the treatment of chronic heart failure. It can improve the heart function of patients, reduce the serum levels of Sirtuin-4 and Beclin-1, and reduce the number of hospitalizations and the average length of hospitalization in patients, which is safe and worthy of clinical application.
