    As a common endocrine disease in gynecology, primary dysmenorrhea seriously affects the quality of life of contemporary women with its high incidence and periodicity. Acupuncture is one of the essential treatment methods of Chinese medicine, and it is widely used in the treatment of modern primary dysmenorrhea. However, how to improve the efficacy of acupuncture in the treatment of primary dysmenorrhea and whether it is necessary to get Deqi is worthy of further discussion. In this article, the author of this article has made a concise description of the effect of acupuncture to Deqi and its effect on the treatment of primary dysmenorrhea by consulting a large number of ancient and modern literatures.

    Objective To investigate the effects of ICP and cerebral oxygen saturation of severe traumatic brain injury (sTBI) patients on different head position. Methods The clinical data of 40 patients with severe craniocerebral injury admitted to Neurosurgical Intensive Care Unit (NSICU) of The People's Hospital of Inner Mongolia Autonomous Region from April 2019 to August 2021 were selected. Intracranial pressure monitoring and cerebral oxygen saturation monitoring were given to observe the changes of intracranial pressure and cerebral oxygen saturation when the head position was raised 0°, 30° and 60°. Results In our group of 40 patients who completed ICP and rScO2 monitoring, intracranial pressure and cerebral oxygen saturation changed when the head of the bed was elevated at different degrees, with the highest intracranial pressure and lowest cerebral oxygen saturation when the head was elevated at 0°. When the patient's head was elevated at 30° and 60°, the intracranial pressure decreased significantly and the cerebral oxygen saturation increased significantly. The difference between the three groups was statistically significant(P<0.05). Conclusion The application of ICP combined with rScO2 monitoring in patients with severe traumatic brain injury can detect the changes of intracranial pressure and brain oxygen saturation in time, and head position elevation can reduce intracranial pressure and improve brain oxygen saturation.

    Objective To investigate the effects of valsartan combined with spironolactone on the treatment of intractable heart failure, coagulation function and cardiac function. Methods Eighty patients with intractable heart failure at the Department of Cardiology, Chifeng Cancer Hospital from January 2019 to December 2020 were involved and randomly divided into control group and experimental group, and each group contained 40 patients. The control group were treated with valsartan, while experimental group was also administrated with spironolactone in addition to valsartan. Response rated of different therapeutic schemes, and the improvement of coagulation indexes, cardiac function, ET-1 and NO levels and improvement of life quality from patients were compared herein. Results Response rate of experimental group was higher than control group,the difference was statistically significant (P<0.05). At the same time, after treatment the levels of D-dimer (D-D) and von Willebrand factor (vWF) in the control group were significantly lower than those in the control group. But the level of activated partial thromboplastin time (APTT) was significantly higher than that of the control group, the levels of left ventricular ejection fraction (LVEF) and output per stroke (SV) were significantly higher than those of the control group, and the level of heart rate (HR) was significantly lower than that of the control group. There was statistically significant difference between the two groups (P<0.05). After treatment, the level of ET-1 in the experimental group was significantly lower than that in the control group, while the level of NO in the experimental group was statistically significantly higher than that in the control group (P<0.05). The scores of physical function, psychological state, material life and life quality of social relations in the experimental group were higher than those in the control group after clinical treatment, and the difference was statistically significant (P<0.05). Conclusion Valsartan combined with spironolactone in the treatment of patients with intractable heart failure possessed high therapeutic efficacy, and the indexes of coagulation function, cardiac function and the levels of ET-1 and NO were significantly improved after treatment, which showed the potential to improve the life quality of patients and afforded high value of popularization and application.

    Objective To evaluate and systematically analyze the practical effect of self-made Gushang No.1 ointment combined with small splint external fixation in the treatment of elderly patients with distal radius fractures, so as to guide clinical practice. Methods From January 2019 to January 2020, there were 84 patients with distal radius fractures in Zhanjiang Second Hospital of Traditional Chinese Medicine. The patients were divided into two groups with 42 cases in each group by random odd and even number method. The treatment mode of the control group was routine bonesetting reduction, while the observation group was treated with the combination of self-made Gushang No.1 ointment and small splint external fixation. The curative effect, complications, fracture reduction and other related indexes were compared between the two groups. Results The total effective rate of treatment in the observation group was higher than that in the control group, with significant difference (P<0.05). The total complication rate of the observation group was lower than that of the control group (P<0.05). After 3 months of treatment, the difference of ulnar and radial height and the angle of radius ulnar deviation in the observation group were higher than those in the control group, but the angle of radial palm inclination was lower than that in the control group, with significant difference (P<0.05). After 3 months of treatment, the quality of life score in the observation group was higher than that in the control group, and the data difference was significant(P<0.05). After 3 months of treatment, platelet count, thrombin time, prothrombin time, bone mineral density of radius, bone-derived alkaline phosphatase, type Ⅰ procollagen hydroxyl end anterior fetus and osteocalcin in the observation group were significantly higher than those in the control group (P<0.05). Conclusion For the treatment of distal radius fractures in the elderly, the self-made Gushang No.1 ointment combined with small splint external fixation in our hospital can improve the clinical treatment effect, reduce complications, improve the quality of life, improve the height difference between ulna and radius, the ulnar deviation angle of radius, the inclination angle of radius palm, platelet count, thrombin time and other related treatment indexes. It is worthy of clinical application.

    Objective To observe the clinical efficacy of intra-arterial thrombolysis combined with intravascular stenting applied to patients with acute cerebral infarction. Methods Sixty-eight patients with acute cerebral infarction in Pingdu Hospital of Chinese Medicine from October 2018 to January 2021 were divided into control group and observation group equally. The patients in control group were treated with intra-arterial thrombolysis, whereas the patients in observation group were subject to intra-arterial thrombolysis combined with intravascular stenting. The incidence of postoperative side effects and the total response rate were compared between the two groups. The hemodynamic indexes, serum-related indexes and life quality of the two groups before and after treatment were compared. The neural function deficiency of the two groups at different stages during treatment was compared. Results The incidence of side effects in observation group was lower than control group, while the total response rate of observation group was higher than control group (P<0.05). Before treatment, there was no significant difference in hemodynamic indexes, serum-related indexes, life quality and neural function deficiency between the two groups (P>0.05). However, after treatment, the hemodynamic indexes, serum-related indexes and neural function deficiency scores in the observation group were lower than those in the control group, and the quality of life was better than that in the control group (P<0.05). Conclusion The application of intra-arterial thrombolysis combined with intravascular stenting to patients with acute cerebral infarction could significantly improve the hemodynamics and serum-related indexes of patients, attenuate neural function deficiency, possess high therapeutic safety and efficacy, and enhance the overall therapeutic effect and life quality of patients.

    Objective The purpose of this study is to investigate the distribution of pathogens, clinical characteristics and risk factors of polymicrobial bloodstream infections(PBSIs)in adult patients. Methods In a retrospctive matched case-control study design, all hospitalised adults with PBSIs from November 2018 to August 2021 were recruited. For each PBSI case, two patients with monomicrobial bloodstream infections(MBSIs)by pathogens and period were matched. All MBSIs cases matched composed the control group. The distribution of pathogens of the case group was analyzed. The clinical data of the two groups were compared. The multivariable analysis was performed using conditional logistic regression to determine the independent risk factors associated with PBSIs. Results Thirty-eight episodes of PBSI(2.9% of all adult bloodstream infections)and 76 pathogens were identified. The first two strains of fifty-four gram-negative bacill were escherichia coli and klebsiella pneumoniae. The majority of gram-positive bacteria were Enterococcus. Half of the pathogen combinations were the combination of gram-negative bacilli. Solid malignant tumor[P=0.016; odds ratio (OR)= 4.862]and biliary tract infections(P=0.032;OR=3.704)were independent risk factors for PBSIs. Patients with PBSIs had higher percentage of antibiotics combination therapy than patients with MBSIs(P<0.05). Conclusion Solid malignant tumor and biliary tract infections are independent risk factors for PBSIs. These provide a basis for clinicians to identify patients at risk of PBSIs.

    Objective To investigate the effects of intravenous thrombolysis with recombinant human urokinase on coronary blood flow and myocardial perfusion in patients with ST segment elevation myocardial infarction. Methods From January 2019 to January 2022, 120 patients with high thrombotic load acute ST segment elevation myocardial infarction (STEMI) treated in Xinjiang production and Construction Corps Hospital were randomly divided into study group and control group, with 60 cases in each group. Patients in the study group were treated with recombinant human urokinase and patients in the control group were treated with urokinase. The cardiac function index, coronary blood flow, myocardial perfusion and the incidence of cardiovascular adverse events were compared between the two groups. Results There was no statistical significant difference in left ventricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDd) and left ventricular ejection fraction (LVEF) between the two groups before and after treatment (P>0.05). After treatment, LVESD, LVEDd and LVEF were significantly improved (P<0.05), and there was no statistical significant difference between the two groups (P>0.05); There was no statistical significant difference in grade 0-1(P>0.05), grade 2 and grade 3 coronary artery blood flow between the two groups before treatment (P<0.05), and there was no significant difference in grade 0-1 coronary artery blood flow between the observation group after treatment(P>0.05). The number of grade 2 coronary artery blood flow in the observation group was lower than that in the control group, and the number of grade 3 coronary artery blood flow in the observation group was higher than that in the control group (P<0.05); The rate of ST treatment decline and myocardial staining grade (MBG) 3 in the two groups were significantly higher than those in the control group (P<0.05), CTFC (corrected TIMI frame number) and NR (no reflow) incidence were significantly lower than those in the control group (P<0.05); There was no statistical significant difference in the total incidence of adverse cardiovascular events between the two groups (P>0.05). Conclusion Intravenous thrombolysis with recombinant human urokinase can improve vascular recanalization, coronary blood flow and myocardial perfusion. The effect is positive and the safety is good. It is worthy of clinical application.

    Objective To explore the effect of meridian massage combined with fire dragon pot comprehensive therapy on cervical spondylosis of wind-cold obstruction type. Methods From January 2021 to December 2021, 74 cases of cervical spondylosis patients diagnosed and treated in Mudan District Hospital of Traditional Chinese Medicine, Heze City were prospectively selected. The patients were divided into two groups, with 37 cases in the fire dragon can group and 37 cases in the traditional group according to randomized numbers. The traditional group were given meridian massage therapy, and the fire dragon pot group were given fire dragon cupping treatment on the basis of the treatment of the traditional group. The two groups were treated and observed for 14 days, and the curative effects of the two groups were compared, before and after treatment, the pain severity score, the disease score, and the change of inflammatory factors IL-6 and TNF-α. Results The total effective rate after treatment in the dragon pot group was 97.3%, which was significantly higher than that in the traditional group (78.4%). Before treatment, there were no significant differences in the Visual Analogue Scale (VAS) score, disease score, serum IL-6 and TNF-α levels between the two groups (P>0.05). After treatment, the VAS score of two groups were significantly lower than those before treatment (P<0.05), and the score in the fire dragon pot group was also significantly lower than that of the traditional group (P<0.05). Compared with the traditional group, the serum IL-6 and TNF-α levels in the two groups were significantly lower than those before treatment (P<0.05), and both were was also significantly lower in the dragon pot group than the traditional group(P<0.05). Conclusion Meridian massage combined with fire dragon pot comprehensive therapy in the treatment of wind-cold obstruction type cervical spondylosis can inhibit the levels of IL-6 and TNF-α, and can effectively promote the relief of the patients' condition and pain, thereby improving the overall therapeutic effect.

    Objective To explore the effect of TCM syndrome differentiation nursing combined with family support on the ability of daily living of patients with schizophrenia. Methods 90 patients with schizophrenia treated in Henan Provincial Psychiatric Hospital from February 2020 to February 2022 were selected as the main research objects. All patients were divided into observation group and control group by random number table, with 45 cases in each group. The patients in the control group received routine nursing and the patients in the observation group received TCM syndrome differentiation nursing combined with family support. The negative and positive symptoms of PANSS, the ability of daily living and social ability of the patients in the two groups and the satisfaction of their families were compared. Results There was no significant difference in the scores of PANSS between the two groups before nursing (P>0.05). After nursing, the scores of PANSS in the two groups decreased significantly, and the scores in the observation group were lower than those in the control group (P<0.05). Before nursing, there was no significant difference in the scores of Social Deficiency Screening Scale (SDSS) and Activities of Daily Living (ADL) between the two groups of patients (P>0.05). After nursing, the scores of social function deficiency screening scale (SDSS) and activity of daily living scale (ADL) in the two groups decreased significantly, and the scores in the observation group were lower than those in the control group (P<0.05). The nursing satisfaction of the patients in the observation group was significantly higher than in the control group (P<0.05). Conclusion The application of TCM syndrome differentiation nursing combined with family support for schizophrenic patients can further reduce the clinical symptoms of patients, improve their daily living ability and social function, and the satisfaction of family members is high, which is worthy of clinical application.
