    Observation on the Effect of Minimally Invasive Gastric Cancer Surgery in Patients with Gastric Cancer

    Objective To explore the clinical effect of minimally invasive gastric cancer surgery in patients with gastric cancer. Methods From June 2019 to January 2022, 100 patients with gastric cancer who were diagnosed with gastric cancer in Jining First People’s Hospital and had surgical indications were included as the analysis objects, and the patients were divided into groups according to the differences in surgical treatment plans. The 50 patients who received surgical intervention were set as the laparotomy group, and the 50 patients who received laparoscopic minimally invasive assisted radical gastrectomy for gastric cancer were set as the minimally invasive group. The perioperative indicators, serological indicators, and postoperative complication rates of gastric cancer patients after two surgical treatments were used to evaluate the clinical value of minimally invasive methods. Results The time required for surgery in the minimally invasive group was significantly longer than that in the laparotomy group, while the incision length, intraoperative blood loss, and postoperative exhaust time were significantly lower than those in the laparotomy group (P<0.05). After the operation, the white blood cells (WBC) and superoxide dismutase (SOD) of the patients in the two groups were significantly lower than those in the laparotomy group (P<0.05). The incidence of surgery-related complications before discharge in the minimally invasive group was significantly lower than that in the open group (P<0.05). Conclusion Minimally invasive gastric cancer surgery requires higher professional quality of equipment and operators, but it can effectively control the degree of trauma to the patient’s body. In terms of long-term efficacy, there is no significant difference from laparotomy, but the patient’s acceptance is higher, to improve overall comfort.

    Objective Based on the cardiodynamicsgram (CDG) data of acute myocardial infarct (AMI) patients and healthy people, a new early screening model was proposed to evaluate healthy people and AMI patients. Methods The training set included 1500 patients with AMI and 1500 healthy individuals. The SVM models were validated in 2000 individuals. ECG signal of AMI patients without or with mild symptoms was tested to obtain the CDG data of the ECG signal. The dispersion characteristics of CDG data from patients with AMI without or with mild symptoms were extracted, and then support vector machines (SVM) were used to construct an early screening model to screen and evaluate ECG signals in healthy population and patients with AMI. Results The quantitative values of the dispersion characteristics of patients with AMI were significantly different from those of healthy individuals. The CDG data of patients with AMI was more disordered than that of normal individuals. The accuracy of SVM model for AMI diagnosis was 84.05%. The average time consumed to analyze a patient by this model was 2 minutes. Conclusion The application of AMI early screening method based on the dispersion characteristics of CDG data and SVM model can effectively distinguish between patients with AMI and healthy individuals. This method provide an ancillary method of early screening of AMI.

    Objective Observe the effect of aromatic traditional Chinese medicine pillow combined with five-tone therapy on anxiety in patients with chronic renal failure and insomnia. Methods A total of 90 patients with chronic renal failure insomnia who were hospitalized in the Department of Nephrology, Suzhou Hospital of Traditional Chinese Medicine from June 2020 to June 2021 were selected as the research objects, and randomly divided into observation group and control group according to random number table sampling, 45 cases each. The control group received conventional treatment, and the observation group received aromatic medicine pillow combined with five-tone therapy. Both groups were treated for 4 weeks, and the scores of self-rating anxiety scale (SAS) and Pittsburgh Sleep Quality Index (PSQI) were observed before and after treatment. Results There was no difference in SAS score and PSQI score between the two groups before the intervention (P>0.05), but 4 weeks after the intervention, the SAS score and PSQI score deceased(P<0.05), and the observation group was better than the control group, the difference was statistically significant (P<0.05). Conclusion Aromatic medicine pillow combined with five-tone therapy can relieve anxiety and improve the sleep quality of the patients, which is worthy of clinical promotion.

    As one of the important processes of medical education after graduation, residential training is an important part of improving clinicians’ theoretical knowledge, practical ability and humanistic quality, cultivating medical talents with higher standards for medical and health undertakings, and continuously improving the quality of medical care in China. The appropriate assessment and evaluation mode to evaluate the quality of residential training is at the core of the standardized training system for resident doctors. Therefore, in order to meet the needs of the country and the people for high-quality medical talents, it is a general trend to explore a diversified evaluation system based on core competency training and evaluate the quality of residential training more comprehensively.     Objective To explore the application effect of electroacupuncture stimulation combined with clinical rehabilitation training and neurophysiological therapy (bobath technology) in spasm patients. Methods The 64 patients admitted to Binzhou Central Hospital from June 2021 to November 2021 were divided into control and observation groups.The control group took routine rehabilitation training, and the observation group used electroacupuncture stimulation combined with bobath technique based on the control group, and both groups were treated for 4 weeks.Treatment effects, spasticity status (modified Ashworth Spasticity Assessment Scale), motor function [Fugel-Meyer (FMA)], daily activity ability [Activities of Daily Life Scale (ADL)] and quality of life [Quality of Life Measurement Scale (QOL-BREF)] scores were compared. Results Comparison of the Ashworth, ADL, FMA, and QOL-BREF scores between the two pretreatment groups, No statistical difference (P>0.05). The total treatment response rate was higher in the observation group than in the control group, Statistically different (P<0.05); After the treatment, the Ashworth score was lower than that in the control group, The ADL score was higher than that in the control group, Statistically different (P<0.05). Score of upper limb motor function and lower limb motor function in the treated FMA of the observation group, Both were higher than those in the control group, Statistically different (P<0.05). In the treatment, psychological, physiological, social, environmental fields in QOL-BREF after the observation group, both were higher than those in the control group, statistical difference (P<0.05). Conclusion Electroacupuncture stimulation combined with clinical rehabilitation training and bobath technique can effectively improve the spasticity and motor function, and improve the quality of life.

    Objective To investigate the curative effect of Xiefei Lishui Recipe combined with chemotherapy on tuberculous pleurisy and its influence on pulmonary function and immune function. Methods A total of 96 patients with tuberculous pleurisy admitted to Shaanxi Tuberculosis Prevention and Control Hospital (the Fifth People’s Hospital of Shaanxi Province) from June 2021 to July 2022 were selected and divided into two groups by random number table method, with 48 cases in each group. The control group was treated only with chemotherapy, while the study group was treated with Xiefei Lishui Formula on the basis of the control group. The pleural thickness, pleural effusion drainage volume and absorption time before and after treatment were compared between the two groups lung function and peripheral blood Th1 cells, Th2 cells Th17 cells and Treg cells. Results There was no obvious difference in the pleural fluid flow between the two groups (P>0.05). The pleural thickness did not different between the two groups before treatment (P>0.05), decreased after treatment, and the experimental group was lower than the control group (P<0.05), and the pleural fluid absorption time in the experimental group was significantly shorter than that in the control group (P<0.05). The total response rate (95.83%) was higher than 89.58%, with no difference (P>0.05). Between FEV1% pred, FVC% pred, FEV1/FVC% Pred, RV% pred, TLC% pred before and after treatment (P>0.05), FEV1% pred, FVC% pred, TLC% pred were higher than before treatment, and FEV1/FVC% Pred, RV% pred were lower than before treatment (P<0.05). Th1 cells, Th2 cells, Th1/Th2, Th17 cells, Treg cells and Th17/Treg (P>0.05), after treatment, Th1 cells, Th1, and Treg cells (P>0.05), while Th1 cells, Th1/Th2, Th17 and Th17/Treg were significantly lower than the control group (P<0.05). Conclusion Compared with chemotherapy alone, the combination of Xiefei Lishui Recipe in the treatment of tuberculous pleurisy can further promote faster absorption of pleural fluid and improve immune function, but it can not improve the overall efficacy and improve pulmonary function.

    Objective To study the effects of Sanzi Yangqin Decoction attenuating lung injury in asthmatic mice by regulating JAK/STAT signaling pathway. Methods Fifty mice were randomly divided into normal group, model group, positive group, low-dose Sanzi Yangqin Decoction group and Sanzi Yangqin Decoction high-dose group. Ovalbumin was used to replicate the asthma mouse model. HE staining was used to detect mouse lung tissue, and to detect lung function.IL-6 and TNF-α levels in BALF were measured by ELISA,The protein expression levels of JAK, p-JAK, STAT3 and p-STAT3 were detected by Western Blotting. Results The normal group of mice has neat organization structure, clear alveolar structure, thin alveolar walls, no inflammatory cell infiltration in the alveoli, and rare collagen deposition in the alveolar epithelium and interstitium. Inflammatory cell infiltration, there is a lot of collagen deposition in the alveolar interstitium. The pathological improvement of the lung tissue in the positive group and the low-dose and high-dose groups of Sanzi Yangqin Decoction was improved compared with the model group, and the pathological improvement of the lung tissue in the high-dose Sanzi Yangqing Decoction group was the most obvious. Compared with the normal group, the FEV1 and PEF of the mice in the model group were significantly decreased (P<0.05); compared with the model group, the FEV1 and PEF of the positive group, the low-dose Sanziyangqin Decoction group and the high-dose Sanziyangqin Decoction group were significantly lower (P<0.05). were significantly increased (P<0.05). Compared with the normal group, the levels of IL-6, TNF-α, p-JAK, and p-STAT3 in the model group were significantly increased (P<0.05). The levels of IL-6, TNF-α, p-JAK and p-STAT3 in the dose group and the high-dose Sanziyangqin decoction group were significantly decreased (P<0.05). Conclusions Sanzi Yangqin Decoction can effectively improve the related pathological changes in asthmatic mouse models, and its mechanism of action may be to improve the inflammatory response of the lungs by regulating the JAK/STAT signaling pathway.
