    Study on the Nursing of Children with Congenital Biliary Atresia Treated with Laparoscopic Kasai Operation

    Objective To investigate the nursing methods and effects of laparoscopic hepaticojejunostomy (Kasai procedure) for children with congenital biliary atresia. Methods 116 children with congenital biliary atresia admitted to the First People's Hospital of Urumqi from January 2018 to June 2021 were selected as study subjects. They were divided into the control and the observation groups using a random number table, with 58 cases in each group. The control group was given conventional nursing intervention and the observation group was given rehabilitation nursing intervention. The nursing effects and the incidence of complications (infection, cholangitis, peritoneal effusion and liver failure) were compared between the two groups. Results The incidence of complications in the observation group was lower than that in the control group, the difference has statistically significant (P<0.05). Time of drinking and eating, gastric tube removal, anal venting, anal defecation, recovery of bowel sounds and hospital stay was shorter in the observation group than that in the control group, the difference has statistically significant (P<0.05). Conclusion The combined implementation of rehabilitation nursing intervention during laparoscopic Kasai operation for children with congenital biliary atresia can further improve the nursing effect, effectively reduce the complication rate, and shorten the postoperative recovery time and average hospital stay in affected children. This method is worthy of clinical application.

    Objective To analyze the clinical nursing effect of follow-up management nursing mode in patients undergoing digestive endoscopic resection. Methods A total of 98 patients who received digestive endoscopic resection in Nangang Campus of Heilongjiang Provincial Hospital from January 2019 to March 2021 were selected as the research objects. Grouping method was random number table method. The control group (n=49) took routine nursing care of digestive endoscopy, and the study group (n=49) added follow-up management nursing care on the basis of routine nursing plan. The incidence of adverse events, satisfaction with nursing care and evaluation of nursing quality by head nurse were compared between the two groups. Results The incidence of adverse events in the study group was lower than that in the control group, the difference has statistically significant (P<0.05); The patients' satisfaction with nursing in the study group was significantly higher than that in the control group (P<0.05), the difference has statistically significant; The scores of communication, service attitude and operation skills of nursing patients in the study group were significantly higher than those in the control group, the difference has statistically significant (P<0.05). Conclusion It can significantly reduce the risk of adverse events and improve the head nurse's evaluation of nursing quality by adding the follow-up management nursing mode to the routine nursing. The patients' satisfaction is high and it has high application value.

    Objective To evaluate the effects of ultrasound-guided transverse abdominal muscle plane block on postoperative analgesia of liver. Methods 46 patients under liver surgery in The Second Affiliated Hospital of Mudanjiang Medical College from January 2021 to December 2021 were selected and divided into control group and research group by random number table method. Each group contained 23 patients. The research group was treated with ultrasound-guided transverse abdominal muscle plane block while the control group was injected with 0.9% sodium chloride injection. The postoperative VAS score, Ramsay score, heart rate, mean arterial pressure and the dosage of general anesthesia for the patients were compared. Results At the T0 stage, heart rates in both groups has no statistically significantly difference (P>0.05); At T1, T2, and T3 stages, the heart rate of the study group was significantly lower than that of the control group, and the difference was statistically significant (P<0.05); At the stage of T0, there was no statistically significantly difference in mean arterial pressure for the two groups, but at the stages of T1, T2 and T3, (P<0.05). And the dosages of sufentanil, propofol and remifentanil for the patients in research group were lower than control group (P<0.05). At the stage of T0, there was no statistically significant difference in Ramsay score among the two groups (P>0.05), whereas at the stages of T1, T2 and T3, research group afforded a higher Ramsay score than control group, the difference was statistically significant (P<0.05). In addition, though there was no statistically significant difference in VAS score between the two groups at T0 stage (P>0.05), at the stages of T1, T2 and T3 the VAS score of research group was statistically significantly difference with control group (P<0.05). Conclusion sTransverse abdominal muscle plane block applied to patients with liver surgery could promote the sedation effect, stabilize the postoperative heart rate and amean arterial pressure, and reduce the dosage of anaesthetic agents.

    Objective To analyze the clinical effect of musculoskeletal ultrasound in the diagnosis of gouty arthritis of foot and ankle. Methods One hundred and twenty patients with suspected gouty arthritis of foot and ankle admitted to Mudanjiang Hospital of Traditional Chinese Medicine from June 2019 to June 2020 were selected as the research objects, and their clinical data were analyzed retrospectively. The subjects were examined by musculoskeletal ultrasonography and MRI for the first clinical diagnosis, with the final clinical diagnosis as the gold standard.The diagnosis results of the two methods was compared. Results The sensitivity and negative predictive value of musculoskeletal ultrasound in diagnosing gout of foot and ankle were significantly lower than those of MRI, the difference has statistical significant (P<0.05), but there was no statistically significant difference in specificity, diagnostic accuracy and positive predictive value between them (P>0.05); The two diagnoses were in good agreement (Kappa = 0.545). Conclusion Musculoskeletal ultrasonography has high accuracy in diagnosing gouty arthritis of foot and ankle, and has high clinical application value.
    Effect of Dapoxetine Hydrochloride on Sperm Quality and Psychological Stress in Patients with Premature Ejaculation

    Objective To study the effect of dapoxetine hydrochloride on sperm quality and psychological stress in patients with premature ejaculation. Methods One hundred and ninety patients with premature ejaculation admitted to Bethune Hospital in Shanxi Province from June 2018 to June 2021 were selected as the research objects, and were randomly divided into control group (95 cases) and observation group (95 cases). The control group was given sertraline hydrochloride tablets, and the observation group was given dapoxetine hydrochloride tablets. Both groups were treated continuously for 2 months. The scores of self-rating anxiety scale (SAS), self-rating depression scale (SDS), Intra-vaginal ejaculation latency time (IELT), scores of premature ejaculation scale and semen related indexes were compared before treatment and two months after treatment. Results After 2 months of treatment, the scores of SAS and SDS in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05). After 2 months of treatment, IELT in the two groups was higher than that before treatment, after 1 month of treatment, after 1 month of treatment, and after 2 months of treatment, IELT in the observation group was higher than that in the control group, the difference was statistically significant (P<0.05). After 2 months of treatment, the scores of sexual satisfaction, ejaculation control ability, communication difficulty between partners and negative emotion related to premature ejaculation in the two groups were higher than those before treatment, and the scores in the observation group were higher than those in the control group, the difference was statistically significant (P<0.05). Compared with before treatment, after 2 months of treatment, there was no significant difference in total sperm count, sperm density and percentage of forward motility sperm between the two groups (P>0.05). Conclusion Sertraline hydrochloride and dapoxetine hydrochloride can improve sperm quality in patients with premature ejaculation, but dapoxetine hydrochloride can alleviate psychological stress and improve ejaculation control ability, which is helpful to improve sexual life quality of patients.

    Objective To explore the preventive effect of high-quality nursing intervention on upper limb lymphedema in breast cancer patients after operation. Methods One hundred and fifty breast cancer patients in Chifeng Hospital from March 2020 to September 2021 were retrospectively selected and divided into two groups according to nursing methods, with 75 cases in each group. The upper limb lymphedema, nursing effect, quality of life, negative psychology and nursing satisfaction of the two groups were statistically analyzed. Results The incidence of upper limb lymphedema in high quality nursing intervention group was lower than that in routine nursing intervention group (P<0.05); The excellent and good nursing rate of patients in the high-quality nursing intervention group was higher than that in the routine nursing intervention group (P<0.05); After nursing intervention, the scores of material life, physical function, mental health and social function of patients in both groups were higher than those before nursing intervention, and the difference was statistically significant (P<0.05); Before nursing intervention, the scores of material life, physical function, mental health and social function were not statistical significantly different between the two groups (P>0.05). After nursing intervention, the scores of material life, physical function, mental health and social function of patients in high-quality nursing intervention group were higher than those in routine nursing intervention group, and the difference was significant (P<0.05). Before nursing, there was no statistical significant difference in the proportion of drug resistance, fear, inferiority and other negative psychology between the two groups (P>0.05). After nursing, the proportion of drug resistance, fear, inferiority and other negative psychology in the evidence-based nursing group was lower than that in the routine nursing group, and the difference was significant (P<0.05). Conclusion Compared with routine nursing intervention, postoperative high-quality nursing intervention for breast cancer patients can effectively prevent lymphedema of upper limbs, and has better nursing effect. It can effectively improve patients' quality of life, reduce patients' negative psychology and improve patients' nursing satisfaction, which is worthy of clinical application.

    Objective To analyze the clinical effect of ultramicroangiography in differentiating the benign and malignant nature of breast diseases in the process of clinical diagnosis. Methods The clinical data of 67 patients with breast lesions treated in Hongqi Hospital affiliated to Mudanjiang Medical College from May 2019 to May 2021 were analyzed retrospectively. All patients were examined by ultrasound. The blood flow classification of breast lesions was Adle blood flow classification. The blood flow detection rates of color Doppler flow imaging (CDFI) and superb microvascular imaging (SMI) were compared. The distribution characteristics of vascular morphology in CDFI and SMI were analyzed, the vascular distribution patterns in the lesions were classified one by one, and the differences between benign and malignant lesions were compared. Results The detection rate of blood flow in breast lesions by SMI was significantly higher than that by CDFI. There were significant differences in Adler blood flow grade and vascular morphology between benign and malignant breast lesions by CDFI and ultramicro-angiography (SMI). The vascular morphology of benign lesions was mainly hypovascular, linear, curved and dendritic, while that of malignant lesions was mainly crabfoot-shaped, and the blood vessels were thick and twisted. Conclusion SMI can significantly improve the detection rate of blood vessels in breast lesions, and this technique mode can clearly observe the Adler blood flow classification of breast lesions and analyze the morphological characteristics of blood vessels in order to further distinguish between benign and malignant breast lesions.
