    Analysis of the Application of Insect Drugs in Rheumatoid Arthritis

    Objective To investigate the application of insect drugs in the treatment of rheumatoid arthritis (RA) by famous old Chinese medicine, and to provide some reference for the clinical application of insect drugs. Methods In this study, the modern medical records of traditional Chinese medicine practitioners using insect drugs to treat RA were collected, a structured medical records database was built, and the frequency statistics, association analysis and cluster analysis of prescriptions were carried out using the cloud platform of ancient and modern medical records to present the application rules of insect drugs in RA in the form of data. Results There were 115 statistical data of symptoms and 32 types of symptoms, among which the most frequent were the syndrome of blood stasis blocking collaterals and the syndrome of damp-heat blocking. There were 188 prescriptions of insect-containing drugs, involving 213 kinds of traditional Chinese medicine, of which 28 kinds were insect-containing drugs, and high-frequency insect-containing drugs included pangolin, scorpion, and Wuzao snake. From the perspective of prescription composition, the proportion of insect drugs in prescriptions is not high. From the point of view of dosage, the dosage of insect drugs in the prescription is small. From the point of view of TCM flavor, the use of the four qi middle-temperature drugs and flat drugs is the most frequent, and the use of the five tastes of Gan, Xin and bitter is the most frequent, mainly under the liver and spleen channels. A total of 21 insect-containing drug pairs were obtained by association rules, including 6 pairs with frequency ≥30 times, and 1 group of multi-flavor drug combinations and 3 groups of drug pairs were obtained by cluster analysis. Conclusion Pangolin, scorpion, Wuzao snake, Earthdragon, centipede and other insect drugs are commonly used in the prescription of traditional Chinese medicine in the treatment of RA, and most of them are used in combination with the drugs for toning deficiency, dispelling wind and removing dampness, promoting blood circulation and removing blood stasis, among which the scorpion and Astragalus pair are used most frequently.

    Objective To explore the effect of Four-dimensional stereoscopic analgesic acupuncture adjuvant therapy on pain degree and quality of life of cancer pain patients. Method A total of 70 cancer pain patients were admitted to Weichang Manchu and Mongolian Autonomous County Traditional Chinese Medicine Hospital of Hebei Province from May 2021 to May 2023 were selected, and patients were randomly selected by computer and divided into control group and study group, 35 cases in each group. The control group received three-level ladder pain relief treatment, while the study group received Four-dimensional stereoscopic analgesic acupuncture on the basis of the control group. The total effective rate, pain mediators [serum serotonin (5-HT), endothelin-1 (ET-1), prostaglandin E2 (PGE2)] levels, numerical rating scale (NRS) score, World Health Organization Quality of Life-Brief (WHOQOL-BREF) score and incidence of adverse reactions were compared between two groups. Result The total effective rate of the study group was 94.29% (33/35), which was higher than that of the control group [74.29% (26/35)], the difference was statistically significant (P<0.05). The NRS score of both groups decreased after two courses of treatment, and the decrease of the study group was greater than that of the control group, the difference was statistically significant (P<0.05). The WHOQOL-BREF score of both groups increased after two courses of treatment, and the increase of the study group was greater than that of the control group, the difference was statistically significant (P<0.05). Serum levels of PGE2, 5-HT and ET-1 were decreased in both groups after two courses of treatment, and the decrease in the study group was greater than that in the control group, the difference was statistically significant (P<0.05). The incidence of adverse reactions in control group and study group was 17.14% and 11.43%, respectively, and there was no statistical significance between groups (P>0.05). Conclusion The adjuvant treatment of Four-dimensional stereoscopic analgesic acupuncture can effectively relieve cancer pain, decrease the levels of pain mediators, and improving the quality of life for patients.

    Objective To assess the efficacy and safety of rivaroxaban compared to low molecular weight heparin (LMWH) for thromboprophylaxis in cancer patients, aiming to provide clinical guidance on anticoagulant therapy for preventing cancer-related thrombosis. Methods A retrospective analysis was conducted involving 193 patients with malignant tumors from Yulin Red Cross Hospital between June 2022 and March 2023. Patients were divided into the rivaroxaban group (n=98) and the LMWH group (n=95). The incidence of thrombosis, bleeding, and mortality within one month and six months post-treatment were compared between the two groups. Results There were no statistically significant differences in the rates of thrombosis, bleeding, or mortality between the two groups at one month and during the six-month follow-up(P>0.05). Conclusion Rivaroxaban demonstrates comparable efficacy and safety to LMWH for thromboprophylaxis in malignant tumors,or can be used as a reasonable alternative to refusal or inability to tolerate subcutaneous injection.

    Objective Investigate the current status of compliance with rehabilitation training after discharge among stroke patients, and analyze the main factors influencing their compliance with rehabilitation training after discharge. Methods This study selected discharged stroke patients who met the inclusion and exclusion criteria from grade II-A and above hospitals in Zhangzhou City from May to October 2023 as the research subjects. The study collected relevant information from the patients using a general information survey and a functional exercise compliance scale for stroke patients. The primary objective was to assess the current status of patients' compliance with rehabilitation training after discharge, and to analyze the factors influencing their compliance with rehabilitation training post-discharge. Results The level of compliance with rehabilitation training among stroke patients after discharge was moderate. Comparison of the scores of compliance with rehabilitation training after discharge among patient groups with different age ranges, types of caregivers, BI (Barthel Index) scores, and BMI (Body Mass Index) revealed statistically significant differences (P<0.05). Conclusion The level of compliance with rehabilitation training among stroke patients after discharge from hospitalization is moderate and is influenced by factors such as age, type of caregiver, BI (Barthel Index) score, and BMI (Body Mass Index).

    Objective To explore the nursing effect of film fenestration combined with gauze dressing change in PICC puncture point exudate. Methods A total of 40 patients with PICC puncture point exudation in the Department of oncology of the Second Affiliated Hospital of Anhui Medical University from January 2019 to January 2021were selected as the research objects for retrospective study, and they were divided into control group and observation group according to different nursing methods, with 20 cases in each group; The control group was treated with routine nursing, and the observation group was treated with film sticking and window opening combined with gauze dressing change on the basis of the control group. The healing time of puncture point exudation and the incidence of infection, catheter displacement, tube removal and contact dermatitis were compared between the two groups. Results The average healing time of puncture point exudation in the observation group was significantly lower than control group, the difference was statistically significant; The total incidence of infection, catheter displacement, catheter detachment and contact dermatitis in the observation group was significantly lower than control group , the difference was statistically significant. Conclusion The application of film fenestration combined with gauze dressing change in patients with PICC puncture point exudation can shorten the healing time of fluid leakage at the puncture point.

    Objective To explore the intervention effect of combined application of acupoint application and Five Elements Music Therapy on pediatric bronchial asthma, as well as its impact on disease symptoms and negative emotions. Methods From January 2021 to November 2023, 80 children with pediatric bronchial asthma were admitted and randomly divided into a control group (n=40, receiving routine Western medicine comprehensive care) and an observation group (n=40, receiving routine Western medicine comprehensive care combined with acupoint application and Five Elements Music therapy) using a random number table method. The disease symptom relief time, negative emotions, and respiratory function of the two groups were compared. Results The relief time of chest tightness, shortness of breath, wheezing and cough in the observation group were shorter than those in the control group (P<0.05). Before intervention, there was no difference in the total score of the Screen for Child Anxiety Related Emotional Disorders (SCARED), the total score of the child medical fear scale (CMFS), peak expiratory flow (PEF), maximum mid expiratory flow (MMEF), and forced vital capacity (FVC) between the two groups of children (P>0.05); After intervention, the total score of SCARED and CMFS in the observation group were lower than those in the control group, while PEF, MMEF and FVC were higher than those in the control group (P<0.05). Conclusion The combination of acupoint application and Five Elements Music Therapy can significantly improve disease control, accelerate symptom relief, alleviate negative emotions, and improve respiratory function in children with bronchial asthma.

    Objective To explore the application effect of nursing projects in the abdominal injection of low molecular weight heparin calcium in stroke patients. Methods Select 60 stroke patients who received abdominal injection of low molecular weight heparin calcium and were admitted to Qingxian People's Hospital in Hebei Province from February 2023 to April 2023 as the control group for routine nursing intervention; 56 stroke patients who received abdominal injection of low molecular weight heparin calcium and were admitted to Qingxian People's Hospital in Hebei Province from June 2023 to August 2023 were selected as the observation group for nursing intervention. Compare the differences in subcutaneous bleeding, pain level, positive attitude, and satisfaction between two groups of patients after injection under their respective nursing interventions. Results The subcutaneous bleeding rate of patients in the observation group after injection of low molecular weight heparin calcium was lower than that of the control group, and the difference was statistically significant (P<0.05); The Pain Assessment Scale (PANAS) scores of the observation group patients were lower than those of the control group at 1 and 24 hours after injection, and the difference was statistically significant (P<0.05). Before nursing, there was no statistically significant difference in the positive and negative emotion index between the two groups of patients (P>0.05). After nursing, the positive emotion index scores of the observation group patients were significantly higher than those of the control group, while the negative emotion index scores were lower than those of the control group, and the differences were statistically significant (P<0.05); The overall satisfaction of the observation group patients with nursing services was better than that of the control group, and the differences were statistically significant (P<0.05). Conclusion Nursing programs can effectively reduce the incidence of subcutaneous bleeding in stroke patients after abdominal injection of low molecular weight heparin calcium, improve pain levels, enhance positive emotions, and improve patient satisfaction with nursing work. The application effect is ideal and further application can be considered.

    Objective To explore the influencing factors of rumination status in patients with postoperative chemotherapy for colorectal cancer, and to provide some reference for formulating corresponding countermeasures. Methods A total of 117 patients with colorectal cancer who received postoperative chemotherapy in the Sixth Affiliated Hospital of Sun Yat-sen University from January 2024 to March 2024 were selected as the subjects of this cross-sectional survey. The general data, rumination level, mental resilience level and perceptive social support level of patients with colorectal cancer were investigated by questionnaire, and the factors affecting rumination level of patients with colorectal cancer were analyzed. Results The score of invasive rumination was (12.92±5.06), and the score rate was 43.07%. Purposive rumination score (14.50±3.62), scoring rate of 48.33%; The correlation study indicated that the score of intrusive rumination was negatively correlated with the score of mental resilience and perceived social support, while purposive rumination had a positive correlation with the above two indicators(P<0.05). Multiple linear regression analysis revealed that gender, educational level, psychological resilience score, and perceptive social support score combined to affect the variability of intrusive rumination and purposive rumination, explaining 79% and 40.1% of the difference, respectively (P<0.05). Conclusion Patients with colorectal cancer generally lack active deep thinking, and the main influencing factors are gender, education level, mental resilience level and social support recognition level. In view of this, medical personnel should pay special attention to the psychological bearing problems of female patients and those with lower education, provide them with additional care and psychological assistance, and strive to enhance patients' ability to cope with pressure and consolidate their social support network.
    Cost-Effectiveness Analysis of Amlodipine Benzene Late Versus Irbesartan for Essential Hypertension in the Elderly

    Objective To analyze the cost-effectiveness of irbesartan and amlodipine besylate in the treatment of essential hypertension in the elderly. Methods A total of 70 elderly patients with essential hypertension admitted to our hospital between September 2023 and July 2024 were included in this study.The patients were divided into two groups by the random number table method, with 35 people in each group, who were given oral treatment of amlodipine besylate and irbesartan respectively.The treatment effect, blood pressure changes before and after treatment, treatment cost and the occurrence of adverse effects were compared between the two groups. Results Amlodipine benzlate group was compared with irbesartan group (P>0.05). After 8 weeks of treatment, systolic, diastolic and mean arterial pressure in both groups were lower than before treatment, and amlodipine benzene late group was lower than irbesartan group (P<0.05). The direct cost of amlodipine group and average daily cost were lower than RMB (P<0.05). The C/E in the amlodipine benzlate group was better than that in the irbesartan group. The overall incidence of adverse effects in the amlodipine phenlate group and irbesartan group(P>0.05). Conclusion Amlodipine benzene late has similar therapeutic effect to irbesartan in treating essential hypertension in the elderly, but better performance in reducing blood pressure, reducing treatment cost and improving cost-effectiveness ratio.
