Abstract: Objective To explore and analysis the effects of Modified Buyang Huanwu Tang on cerebral blood flow and vascular endothelial function in elderly patients with posterior circulation ischemic isolated vertigo. Methods From February 2020 to February 2022, 70 elderly patients with posterior circulation ischemic isolated vertigo who were diagnosed and treated in Xuzhou Hospital of Traditional Chinese Medicine, Jiangsu Province, were selected as the research subjects, and the patients were divided into the traditional Chinese medicine group and the control group according to the principle of 1:1 by drawing lots on envelopes, with 35 cases in each group. The control group was given conventional western medicine, and the traditional Chinese medicine group was given Modified Buyang Huanwu Tang on the basis of the control group. Observe the curative effect of the two groups of patients, and compare the neurological function, daily living ability, cerebral blood flow (whole blood viscosity, fibrinogen) and serum neuropeptide Y (NPY) before and after treatment. Results The total effective rates of the traditional Chinese medicine group were significantly higher than the control group (P<0.05). After treatment, the NIHSS scores of the two groups were significantly lower than those before treatment (P<0.05), and the ADL scores were significantly higher than those before treatment (P<0.05), the NIHSS scores of the traditional Chinese medicine group were significantly lower than that in the control group, and the ADL scores were significantly higher than that in the control group. After treatment, the whole blood viscosity and fibrinogen in the two groups were significantly lower than those before treatment (P<0.05), and the traditional Chinese medicine group was also significantly lower than the control group (P<0.05). The NPY content of the two groups after treatment were significantly higher than that before treatment (P<0.05), and the traditional Chinese medicine group was also significantly higher than the control group after treatment (P<0.05). Conclusion The application of Modified Buyang Huanwu Tang in elderly patients with posterior circulation ischemic isolated vertigo can promote the improvement of cerebral blood flow, and can also improve the vascular endothelial function of patients, improve the therapeutic effect of patients, improve the state of nerve defects, and improve the quality of life, it can improve the patient’s ability of daily living.
