Abstract: Objective To explore the effect of health management platform on high-risk groups of cardiovascular and cerebrovascular diseases, observe and evaluate the quality of life. Methods A questionnaire survey was used to collect 1000 high-risk permanent residents of cardiovascular and cerebrovascular diseases in Zhenze Town, our district from January 2021 to February 2022.There were 500 patients in both groups. The patients in the control group were given health education, guiding medication and urging the improvement of lifestyle during the first questionnaire survey. The intervention group was based on the control group. In addition, the patients in the intervention group continued to be given health education, guiding medication and Supervise and urge the improvement of lifestyle and other interventions once for 6 months, with a total of 6 interventions and guidance. After the intervention and guidance, compare the awareness rate of chronic diseases, medication rate, quality of life score and the incidence of cardio cerebrovascular time between the two groups.Results After treatment, the awareness rate and medication rate of chronic diseases in the intervention group were significantly higher than those in the control group(P<0.05); The quality of life score of the intervention group was lower than that of the control group, and the difference was statistically significant(P<0.05); The incidence of cardiovascular events in the intervention group was lower than that in the control group, and the difference was statistically significant(P<0.05).Conclusion Regular health education and life intervention of health management platform can improve the awareness rate, medication rate and quality of life of high-risk groups of cardiovascular and cerebrovascular diseases, and reduce the occurrence of cerebrovascular events.
