Abstract: Objective Amiodarone is the most commonly used antiarrhythmic drug, and interstitial pneumonia is its serious adverse reaction. This study aims to explore the clinical feature and treatment prognosis of amiodarone-induced diffuse interstitial pneumonia. Methods A case of amiodarone-induced diffuse interstitial pneumonia was retrospectively analyzed in terms of clinical manifestations, imaging feature, treatment and outcome. Results The elderly male patient appeared to have symptoms of cough, dyspnea after use of amiodarone for 1.5 months. The course of disease develops rapidly, followed by type Ⅰ respiratory failure. After symptomatic treatment, withdrawal of amiodarone, non-invasive ventilator assisted breathing, administration of hormones, the clinical symptoms disappeared. Conclusion The diagnosis of amiodarone-induced diffuse interstitial pneumonia should be based on medication history, combined with clinical and imaging findings. The key to successful treatment is to discontinue amiodarone, give corticosteroid and ventilator therapy in severe patients.
