Abstract: Objective To explore the clinical efficacy and impact on the severity of knee osteoarthritis caused by liver and kidney deficiency treated with knee bi decoction combined with traditional Chinese medicine fumigation and washing under the theory of "treating both muscles and bones". Methods A total of 80 patients with knee osteoarthritis of liver and kidney deficiency type admitted to our hospital from July 2022 to June 2023 were selected as the research subjects. The random number table method was used as the guiding basis to group the patients, and the specific groups were control group, observation group, with 40 cases in each group. The control group was treated with oral Celecoxib capsules (0.2 g once daily), while the observation group was treated with XiBi Tang combined with Chinese herbal fumigation (one dose of XiBi Tang daily, divided into morning and evening; Chinese herbal fumigation of the knee joint twice daily, half an hour each time) under the theory of "tendon and bone treatment". The clinical treatment effects, WOMAC index and ISOA indexof the two groups of patients were compared. The serum superoxide dismutase (SOD), nitric oxide (NO) and tumor necrosis factor (TNF) were compared between the two groups before and after treatment-α (TNF-α) Expression level and time for pain relief and joint function improvement. Results In terms of total effective rate of treatment, the observation group was higher than the control group (P<0.05). After treatment, the OMAC index and ISOA index of both groups of patients decreased, and the observation group was lower than the control group (P<0.05). After treatment, the SOD levels of both groups of patients increased, and the observation group was higher than the control group, including NO and TNF- α, the levels all decreased, and the observation group was lower than the control group (P<0.05). The time for pain relief and joint function improvement in the observation group was significantly lower than that in the control group (P<0.05). Conclusion Under the theory of "treating both muscles and bones", the combination of knee bi decoction and traditional Chinese medicine fumigation can improve the clinical efficacy of knee osteoarthritis with liver and kidney deficiency, reduce the severity of the condition, enhance knee joint function, improve levels of oxidative and inflammatory factors, and shorten the duration of clinical symptoms in patients.
