CRP、LDH、PCT联合SF对小儿支原体感染疾病严重程度及预后的诊断价值分析

Diagnostic Value of CRP LDH PCT Combined with SF in the Severity and Prognosis of Mycoplasma Infection in Children

摘要: 目的 分析C反应蛋白(CRP)、乳酸脱氢酶(LDH)、降钙素原(PCT)联合铁蛋白(SF)对小儿支原体感染疾病严重程度及预后的诊断价值。方法 回顾性分析大荔县医院2019年7月—2022年5月收治的150例肺炎支原体感染患儿的临床资料,其中病情较轻有82例(轻症组)、病情较重有68例(重症组)。检测两组的血清CRP、LDH、PCT、SF水平,使用Pearson相关性分析患儿的CRP、LDH、PCT、SF水平和临床肺部感染评分(CPIS评分)的关系;随访2个月,通过ROC曲线下面积(AUC)评价联合检测CRP、LDH、PCT、SF对预后不良的预测效能。结果 检测血清CRP、LDH、PCT、SF水平发现重症组的数据比轻症组更高(P<0.05);经Pearson相关性分析,患儿血清CRP、LDH、PCT、SF水平均与CPIS评分呈正相关(P<0.05);所有患儿均获得随访,其中治愈118例、预后不良32例;经多因素Logistic回归分析,血清CRP、LDH、PCT、SF指标均是患儿预后不良的独立预测因素(P<0.05);经ROC曲线分析,血清CRP、LDH、PCT联合SF预测肺炎支原体感染患儿预后不良的AUC为0.912,明显大于单一指标CRP的0.666、LDH的0.589、PCT的0.712和SF的0.683。结论 血清CRP、LDH、PCT、SF与小儿支原体感染疾病严重程度密切相关,四者联合预测患儿预后不良的效能较好。

Abstract: Objective To analyze the diagnostic value of C-reactive protein (CRP), lactate dehydrogenase (LDH), procalcitonin (PCT) combined with ferritin (SF) in the severity and prognosis of mycoplasma infection in children. Methods Clinical data of 150 children with mycoplasma pneumoniae infection admitted to Dali County Hospital from July 2019 to May 2022 were retrospectively analyzed. Among them, 82 cases were mild (mild group) and 68 cases were severe (severe group). Serum CRP, LDH, PCT and SF levels of the two groups were detected, and Pearson correlation analysis was used to analyze the relationship between CRP, LDH, PCT and SF levels and clinical pulmonary infection score (CPIS score). The area under ROC curve (AUC) was used to evaluate the predictive efficacy of CRP, LDH, PCT and SF in poor prognosis. Results The levels of CRP, LDH, PCT and SF in the severe group were higher than those in the mild group (P<0.05). By Pearson correlation analysis, serum CRP, LDH, PCT and SF levels were positively correlated with CPIS score (P<0.05). All patients were followed up, of which 118 were cured and 32 had poor prognosis. Multivariate Logistic regression analysis showed that serum CRP, LDH, PCT and SF were independent predictors of poor prognosis (P<0.05). Through ROC curve analysis, the AUC of serum CRP, LDH, PCT combined with SF predicting poor prognosis in children with Mycoplasma pneumoniae infection was 0.912, significantly higher than the single indicator CRP of 0.666, LDH of 0.589, PCT of 0.712, and SF of 0.683. Conclusion Serum CRP, LDH, PCT and SF are closely related to the severity of mycoplasma infection in children.The combination of these four factors has a good efficacy in predicting poor prognosis of children,which is worthy of further study and application.
