CT联合磁共振对糖尿病足骨质及血管改变早期诊断价值

Early Diagnostic Value of CT Combined with Magnetic Resonance Imaging in Bone and Vascular Changes in Diabetic Foot Patients

摘要: 目的 探究电子计算机断层扫描(CT)联合磁共振成像(MRI)对糖尿病足骨质及血管改变的早期诊断价值。方法 选取2021年1月—2022年12月中国人民解放军东部战区空军医院收治的80例糖尿病足患者作为研究对象,均行CT与MRI检查,观察糖尿病足CT及MRI影像学表现,对比单一及联合检查对糖尿病足骨质及血管改变的诊断结果,以数字减影血管造影(DSA)为“金标准”,分析单一及联合检查对下肢动脉病变的诊断效能。结果 CT联合MRI检查对软组织肿胀的检出率高于CT检查,骨端骨质破坏的检出率高于MRI检查,骨质疏松及神经性骨关节病变的检出率高于CT及MRI检查,差异有统计学意义(P<0.05);CT联合MRI检查对下肢动脉狭窄的检查结果与DSA检查结果一致性很好(Kappa=0.935);CT联合MRI检查对糖尿病足下肢血管狭窄诊断的灵敏度、准确度及阴性预测值高于单一检查,差异有统计学意义(P<0.05)。结论 CT联合MRI可以更加综合、准确地检查出糖尿病足骨质及血管改变情况,相比单一检查,对下肢动脉狭窄的诊断具有更高的灵敏度、准确度及阴性预测值。

Abstract: Objective To explore the early diagnostic value of CT combined with magnetic resonance imaging (MRI) in diabetic foot bone and vascular changes. Methods A total of 80 patients with diabetic foot admitted to Eastern Theater Air Force Hospital, People's Liberation Army from January, 2021 to December, 2022 were selected as the research objects, and all of them were examined by CT and MRI. The CT and MRI of diabetic foot were observed, and the diagnostic results of single and combined detection for diabetic foot bone and vascular changes were compared. With digital angiography (DSA) as the gold standard, the diagnostic efficacy of single and combined detection for lower extremity arterial diseases was analyzed. Results ①The detection rate of CT combined with MRI was higher than that of CT examination for soft tissue swelling, higher than that of MRI examination for bone end bone destruction, and higher than that of CT and MRI examination for osteoporosis and neuropathic osteoarthropathy, and the difference was statistically significant (P<0.05). ②The results of CT combined with MRI in detecting arterial stenosis of lower limbs are in good agreement with those of DSA (kappa=0.935). ③The sensitivity, accuracy and negative predictive value of 3) CT combined with MRI in the diagnosis of diabetic lower extremity vascular stenosis were higher than those of single detection, and the difference was statistically significant (P<0.05). Conclusion CT combined with MRI can detect the changes of diabetic foot bone and blood vessels more comprehensively and accurately. Compared with single detection, CT combined with MRI has higher sensitivity, accuracy and negative predictive value in the diagnosis of lower extremity arterial stenosis.
