半夏厚朴汤联合氧疗治疗老年肺心病的疗效及对心肌功能的影响

Efficacy and Effect of Banxia Houpu Decoction Combined with Oxygen Therapy on Senile Pulmonary Heart Disease and Myocardial Function

摘要: 目的 探讨半夏厚朴汤联合氧疗治疗老年肺心病的疗效及对心肌功能的影响。方法 选择2021年7月—2023年3月吉林省吉林中西医结合医院收治的80例老年肺心病缓解期患者作为研究对象,采用随机数表法分为两组,各40例。对照组予以氧疗治疗,研究组予以半夏厚朴汤联合氧疗治疗。治疗前后进行动脉血气分析、肺功能、超声心动图检查以及检测血清脑钠肽(BNP)水平,记录3个月内急性加重次数。结果 治疗后,两组超声心动图仅mPAP明显下降(P<0.05),其余指标无明显改变(P>0.05),研究组mPAP明显低于对照组(P<0.05);两组血清BNP水平均明显下降(P<0.05),研究组明显低于对照组(P<0.05);两组肺功能均不同程度改善,研究组第一秒用力呼气量(FEV1)、一秒率(FEV1/FVC)明显高于对照组(P<0.05);两组动脉血气指标均明显改善(P<0.05),研究组PaO2水平明显高于对照组,PaCO2水平明显低于对照组(P<0.05);研究组急性加重次数明显少于对照组(P<0.05)。结论 半夏厚朴汤联合氧疗治疗老年肺心病缓解期患者,可更有效地降低肺动脉高压,提高肺功能,改善低氧状态,降低血清BNP水平,有助于减少急性加重风险。

Abstract: Objective To investigate the effect of Banxia Houpu Decoction combined with oxygen therapy on senile pulmonary heart disease and the effect on myocardial function. Methods A total of 80 elderly patients with pulmonary heart disease in stable stage from July 2021 to March 2023 were selected as research objects.Randomized number table method was used to divide 40 cases into two groups, the control group was given oxygen therapy, the study group was given Banxia Houpu decoction combined with oxygen therapy. Chinese medicine symptom score was evaluated before and after treatment, arterial blood gas analysis, lung function, echocardiography and serum brain natriuretic peptide (BNP) level were detected, and the number of acute exacerbations within 3 months were recorded. Results After treatment, only mPAP in the two groups showed a significant decrease in echocardiography (P<0.05), while other indicators showed no significant changes (P>0.05). The mPAP in the study group was significantly lower than that in the control group (P<0.05); The serum BNP levels in both groups decreased significantly (P<0.05), and the study group was significantly lower than the control group (P<0.05); Both groups showed varying degrees of improvement in lung function, with FEV1 and FEV1/FVC significantly higher in the study group than in the control group (P<0.05); Both groups showed significant improvement in arterial blood gas indicators (P<0.05). The levels of PaO2 in the study group were significantly higher than those in the control group, while PaCO2 was significantly lower than those in the control group (P<0.05); The number of acute exacerbations in the study group was significantly lower than that in the control group (P<0.05). Conclusion Banxia Houpu Decoction combined with oxygen therapy can effectively improve arterial blood gas analysis, reduce pulmonary hypertension, decrease serum BNP level, improve lung function to a certain extent, and help reduce the risk of acute exacerbation in elderly patients with pulmonary heart disease in remission.
