佐匹克隆联合曲唑酮治疗对老年抑郁症患者睡眠结构的影响

Effects of Zopiclone Combined with Trazodone Therapy on Sleep Structure in Elderly Patients with Depression

摘要: 目的 研究佐匹克隆联合曲唑酮治疗对老年抑郁症患者睡眠结构的影响。方法 选择2020年1月—2021年12月北京市昌平区中西医结合医院收治的96例老年抑郁症患者为研究对象,按随机数表法分为对照组和试验组。对照组47例,单用曲唑酮治疗;试验组49例,采用佐匹克隆联合曲唑酮治疗。检测两组患者的睡眠结构,且检测两组患者的血清多巴胺(DA)、去甲肾上腺素(NE)、皮质醇和5-羟色胺(5-HT)水平。结果 试验组的有效率稍高于对照组,差异无统计学意义(P>0.05)。治疗前,两组患者的睡眠效率、总睡眠时间、快速眼动期睡眠期、入睡潜伏期、非快速眼动睡眠期、觉醒次数和觉醒时间比较,差异无统计学意义(P>0.05)。治疗后,两组患者的睡眠效率明显增高,快速眼动期睡眠期、非快速眼动睡眠期明显延长,总睡眠时间明显增加(P<0.05);入睡潜伏期明显缩短,觉醒次数和觉醒时间均明显减少(P<0.05)。且试验组患者的睡眠效率明显高于对照组,快速眼动期睡眠期、非快速眼动睡眠期明显长于对照组,总睡眠时间明显多于对照组(P<0.05);入睡潜伏期明显短于对照组,觉醒次数和觉醒时间明显少于对照组(P<0.05)。治疗前,两组患者的血清DA、NE、皮质醇和5-HT水平比较,差异无统计学意义(P>0.05);治疗后,两组患者的血清DA、NE和5-HT水平均较治疗前明显升高(P<0.05),皮质醇水平均较治疗前明显降低(P<0.05),且两组患者血清DA、NE和5-HT水平对比差异无统计学意义(P>0.05),治疗后试验组患者的皮质醇水平明显低于对照组(P<0.05)。结论 佐匹克隆联合曲唑酮治疗老年抑郁症患者能提高疗效,且能有效改善其睡眠结构,值得应用。

Abstract: Objective To investigate the effect of zopiclone combined with trazodone therapy on sleep structure in elderly patients with depression. Methods 96 elderly depression patients admitted to Beijing Changping District Hospital of Integrated Traditional Chinese and Western Medicine from January 2020 to December 2021 were selected, and divided into two groups according to the random number table method. The control group of 47 patients were treated with trazolone alone, and 49 patients in the experimental group were treated with zopicone and trazolone. The sleep structure of the elderly patients with depression and the effeetive rate in the two groups was detected, and the levels of serum dopamine (DA), norepinephrine (NE), cortisol and serotonin (5-HT) were detected in the two groups. Results The response rate of the experimental group was slightly higher than that of the control group, but the differences were not significant (P>0.05). Before treatment, there were no significant differences in sleep efficiency, total sleep time, REM sleep period, sleep onset latency, non-REM sleep period, number of awakenings and awakening time between the two groups (P>0.05). After treatment, the sleep efficiency, REM sleep period, total sleep time, and non-REM sleep period of the two groups were significantly increased (P<0.05), and the sleep-onset latency, number of awakenings and awakening time were significantly decreased (P<0.05). The sleep efficiency, REM sleep period, total sleep time, and non-REM sleep period of the observation group were significantly higher than those of the control group (P<0.05), and the sleep onset latency, number of awakenings and awakening time were significantly lower than those of the control group group (P<0.05). After treatment, the levels of serum DA, NE and 5-HT in the two groups were significantly higher than those before treatment (P<0.05), and the levels of cortisol were significantly lower than those before treatment (P<0.05). After treatment, there were no significant differences in the levels of serum DA, NE and 5-HT between two groups (P>0.05). After treatment, the cortisol level in the experimental group was significantly lower than that in the control group (P<0.05). Conclusion Zopiclone combined with trazodone has a significant effect on elderly patients with depression, and can effectively improve their sleep structure, which is worthy of application.
