呼吸康复训练对中老年单纯阻塞性睡眠呼吸暂停患者疗效的Meta分析

Effects of Breathing Rehabilitation Training on Middle-aged and Elderly Patients with Simple Obstructive Sleep Apnea: a Meta-analysis of Randomized Controlled Trials

摘要: 目的 评估呼吸康复训练治疗中老年单纯阻塞性睡眠呼吸暂停综合征(OSAS)的疗效。方法 本研究已经在PROSPERO平台注册(CRD42022331424)。检索外文数据库Pubmed、Embase、Web of science、Cochrane Library以及中国知网全文数据库、中国生物医学文献数据库、万方数据库、维普中文科技期刊数据库,获取关于呼吸康复训练治疗OSAS的随机对照试验的文献,检索时限从建库至2022年4月。通过Cochrane系统评价手册进行质量评估,Revman 5.4制作偏倚风险评估图,数据分析软件采用Stata 15.1软件。结果 最终获取文献7篇,研究对象共141例。Meta分析显示,呼吸康复训练后,试验组和对照组睡眠呼吸紊乱指数(AHI)评分比较,差异无统计学意义(MD=-0.13,95% CI为-0.52~0.26,P>0.05); 试验组Epworth嗜睡量表得分优于对照组(MD=-0.79,95% CI为-1.22~-0.36,P<0.05),匹兹堡睡眠质量指数(PSQI)评分优于对照组(MD=-1.23,95% CI为-1.66~-0.79,P<0.05)。结论 呼吸康复训练能够改善OSAS的临床症状,减少白天嗜睡,提高睡眠质量。

Abstract: Objective To evaluate the efficacy of respiratory rehabilitation training in the treatment of middle-aged and elderly patients with simple obstructive sleep apnea syndrome (OSAS). Methods The study has been registered on platform PROSPERO (No. CRD42022331424). Search PubMed, Embase, Web of science and Cochrane Library, China knowledge Network full-text Database, China Biomedical Literature Database, WANFANG, VIP to obtain randomized controlled trials of respiratory-related training in the treatment of OSAS. The search time limit is from the establishment of the database to April 2022. The quality is evaluated through the Cochrane system evaluation manual, the bias risk assessment chart is made by Revman5.4, and the data analysis software is based on Stata15.1 software. Results 7 literatures were obtained, with a total of 141 subjects. Meta analysis showed that after respiratory rehabilitation training, there was no significant difference in the score of sleep disturbance index(AHI) (MD=-0.13, 95% CI-0.52-0.26, P>0.05) between the test group and the control group, but the scores of Epworth sleepiness scale(MD=-0.79, 95% CI-1.22- -0.36,P<0.05) and PSQI sleep quality index (MD=-1.23, 95% CI -1.66- -0.79, P<0.05) in the test group were significantly better than those in the control group. Conclusion Breathing rehabilitation training can improve the clinical symptoms of OSAS, reduce daytime drowsiness and improve sleep quality.
