低频重复经颅磁刺激联合阿戈美他汀治疗脑卒中后抑郁患者的效果观察

Observation on the Effect of Low Frequency Repetitive Transcranial Magnetic Stimulation Combined with Agomestatin in the Treatment of Post-Stroke Depression Patients

摘要: 目的 研究低频重复经颅磁刺激联合阿戈美他汀治疗脑卒中后抑郁患者的效果。方法 前瞻性选择2020年1月~2021年12月沭阳铭和医院收治的70例脑卒中后抑郁患者,根据随机数表法分为对照组和观察组,每组35例。对照组单独服用阿戈美他汀治疗,观察组在阿戈美他汀的基础上联用低频重复经颅磁刺激。检测两组的治疗有效率、5-羟色胺(5-HT)、神经生长因子(NGF)、脑源性神经营养因子(BDNF)以及去甲肾上腺素(NE)水平,且评估两组的生活质量。结果 观察组治疗有效率明显高于对照组(P<0.05);治疗前,两组的NE、5-HT、BDNF和NGF水平比较,差异无统计学意义(P>0.05),治疗后,两组的NE、5-HT、BDNF和NGF水平均明显升高(P<0.05),且观察组的NE、5-HT、BDNF和NGF水平明显高于对照组(P<0.05);治疗前,两组的活动、语言、个性、精力、情绪、思维、社会功能、家庭角色、自理能力、上肢功能、视力和工作能力评分比较,差异无统计学意义(P>0.05),治疗后,两组的活动、语言、个性、精力、情绪、思维、社会功能、家庭角色、自理能力、上肢功能、视力和工作能力评分均明显升高(P<0.05),且观察组的活动、语言、个性、精力、情绪、思维、社会功能、家庭角色、自理能力、上肢功能、视力和工作能力评分明显高于对照组(P<0.05)。结论 低频重复经颅磁刺激以及阿戈美他汀联用对脑卒中后抑郁患者的效果较佳,能明显提高NE、5-HT、BDNF和NGF水平,改善患者的生活质量。

Abstract: Objective To investigate the effect of low frequency repetitive transcranial magnetic stimulation combined with agomestatin in the treatment of post-stroke depression patients. Methods A total of 70 patients with post-stroke depression who were admitted to Shuyang Minghe Hospital from January 2020 to December 2021 were prospectively selected and divided into two groups according to the random number table method. The control group was treated with agomestatin alone, while the observation group was treated with low-frequency repetitive transcranial magnetic stimulation on the basis of agomestatin. The effectiveness of treatment,levels of 5-hydroxytryptamine (5-HT), brain-derived neurotrophic factor (BDNF), norepinephrine (NE) and nerve growth factor (NGF) in the two groups were detected, and the quality of life of the two groups was evaluated. Results The effective rate of the observation group was significantly higher than control group (P<0.05). Before treatment, there was no significant difference in the levels of NE, 5-HT, BDNF and NGF between the two groups (P>0.05). After treatment, the levels of NE, 5-HT, BDNF and NGF in the two groups were significantly increased (P<0.05), and the levels of NE, 5-HT, BDNF and NGF in the observation group were significantly higher than those in the control group (P<0.05). Before treatment, there were no significant differences in the scores of activity, language, personality, energy, emotion, thinking, social function, family role, self-care ability, upper limb function, vision and work ability between the two groups (P>0.05). The scores of activity, language, personality, energy, emotion, thinking, social function, family role, self-care ability, upper limb function, vision and work ability of stroke patients were significantly increased (P<0.05). Personality, energy, emotion, thinking, social function, family role, self-care ability, upper limb function, vision and work ability scores were significantly higher than those in the control group (P<0.05). Conclusion Low-frequency repetitive transcranial magnetic stimulation combined with agomestatin has better effect on patients with post-stroke depression, can significantly increase the levels of NE, 5-HT, BDNF and NGF, and improve the quality of life.
