支架取栓术介入对急性缺血性脑卒中患者血管内皮生长因子、D-二聚体水平的影响

Effects of Stent Thrombectomy on the Levels of Vascular Endothelial Growth Factor and D-dimer in Patients with Acute Ischemic Stroke

摘要: 目的 探讨支架取栓术介入对急性缺血性脑卒中患者血管内皮生长因子、D-二聚体水平的影响。方法 选择2019年1月~2022年2月在徐州医科大学附属医院诊治的前循环急性缺血性脑卒中患者84例作为研究对象,根据治疗方法把患者分为溶栓组和支架取栓组,各42例。支架取栓组给予支架取栓术介入治疗,溶栓组给予静脉溶栓治疗,检测两组血管内皮生长因子、D-二聚体表达水平变化情况。结果 治疗后,支架取栓组总有效率显著高于对照组(P<0.05)。两组治疗后大脑中动脉收缩期峰值流速(peak systolic velocity,PSV)、舒张期末流速(end-diastolic velocity,EDV)明显高于治疗前(P<0.05),支架取栓组明显高于溶栓组(P<0.05)。两组治疗后血清血管内皮生长因子、D-二聚体含量明显低于治疗前(P<0.05),支架取栓组也明显低于溶栓组(P<0.05)。支架取栓组治疗期间脑过渡灌注、静脉血栓、感染等并发症发生率显著低于溶栓组(P<0.05)。结论 支架取栓术介入在急性缺血性脑卒中患者的应用能抑制血清血管内皮生长因子、D-二聚体的表达,提高大脑中动脉血流速度,从而整体提高治疗效果,降低并发症的发生率。

Abstract: Objective To investigate the effects of stent thrombectomy on the levels of vascular endothelial growth factor and D-dimer in patients with acute ischemic stroke. Methods From January 2019 to February 2022, 84 cases of patients with anterior circalation infarction diagnosed and treated in Affiliated Hospital of Xuzhou Medical University were selected as the research subjects. According to the treatment method, the patients were divided into a thrombolysis group and a stent thrombectomy group, with 42 cases in each group. The stent thrombectomy group was given interventional therapy with stent thrombectomy, and the thrombolysis group was given intravenous thrombolysis. The changes in the expression levels of vascular endothelial growth factor and D-dimer in the two groups were detected. Results After treatment, the total effective rates of stent thrombectomy group were significantly higher than thrombolysis group (P<0.05). The peak systolic velocity (PSV) and end-diastolic velocity (EDV) of the middle cerebral artery in the two groups at treatment were significantly higher than those before treatment (P<0.05), and the stent thrombectomy group were significantly higher than the thrombolysis group (P<0.05). The levels of serum vascular endothelial growth factor and D-dimer in the two groups after treatment were significantly lower than those before treatment (P<0.05). The stent thrombectomy group were also significantly lower than the thrombolysis group (P<0.05). The incidence rates of cerebral transitional perfusion, venous thrombosis, infection and other complications in the stent thrombectomy group during the observation period were significantly lower than that in the thrombolysis group (P<0.05). Conclusion The application of stent thrombectomy in patients with cerebral infarction can inhibit the expression of serum vascular endothelial growth factor and D-dimer, it can also improve the blood flow velocity of the middle cerebral artery, thereby improving the overall treatment effect and reducing the incidence of complications.
