目视管理联合六西格玛管理模式在静脉用药调配中心的应用效果

Visual Management Combined with Six Sigma Management Mode in Pharmacy Intravenous Admixture Services Application Effect

摘要: 目的 探究目视管理联合六西格玛管理模式在静脉用药调配中心(PIVAS)管理中的效果。方法 从系统调取2023年7月—12月常规管理模式下处方436 896张记为对照组,2024年1月—6月实施目视管理联合六西格玛管理模式后处方421 749张记为观察组。采用系统随机抽样各1 300张。比较两组配药效率、配药质量、不良事件发生率,以及病区医护人员对PIVAS工作的满意度。结果 观察组的每人每小时配药数量高于对照组,观察组的每瓶配药时间短于对照组,差异有统计学意义(P<0.05)。观察组的配药及时率、准确率、成品输液合格率方面高于对照组,差异有统计学意义(P<0.05)。观察组的配置差错率、药物破损率、不合理医嘱、不良事件率低于对照组,差异有统计学意义(P<0.05)。 观察组病区医护人员对PIVAS的成品输液质量、药品发放的准确性,药品发放的及时性,各批次液体量分配合理性,解决临床反馈问题及时性等方面的满意度高于对照组,差异有统计学意义(P<0.05)。结论 目视管理联合六西格玛管理模式在提高PIVAS工作效率和质量的同时能降低不良事件的发生、提高病区医护人员满意度。

Abstract: Objective To explore the effect of visual management combined with six Sigma management mode in the management of intravenous drug dispensing center (PIVAS). Methods A total of 436,896 prescriptions under the conventional management mode from July to December 2023 were recorded as the control group, and 421,749 prescriptions under the visual management combined with six Sigma management mode from January to June 2024 were recorded as the observation group. A systematic random sampling of 1300 pieces was used. The dispensing efficiency, dispensing quality, incidence of adverse events and satisfaction of medical staff on PIVAS were compared between the two groups. Results The dosage per person per hour in the observation group was higher than that in the control group, and the dosage time per bottle in the observation group was shorter than control group, the difference was statistically significant (P<0.05). The timely dispensing rate, accuracy rate and qualified rate of finished infusion in the observation group were higher than control group, and the differences were statistically significant (P<0.05). The allocation error rate, drug damage rate, unreasonable doctor's order rate and adverse event rate of observation group were lower than control group, and the difference was statistically significant (P<0.05). The satisfaction of medical staff in the observation group was higher than that in the control group on the quality of finished PIVAS infusion, the accuracy of drug delivery, the timeliness of drug delivery, the rationality of liquid distribution in each batch, and the timeliness of solving clinical feedback problems, the difference was statistically significant (P<0.05). Conclusion Visual management combined with six Sigma management can improve the work efficiency and quality of PIVAS, reduce the occurrence of adverse events and improve the satisfaction of medical staff in ward.
