基于二十四节气研究天津地区脑梗死发病与气象因素的关系

Study on the Relationship Between the Incidence of Cerebral Infarction and Meteorological Factors in Tianjin Based on Meteorological Data

摘要: 目的 探讨天津地区脑梗死发病与气象因素的关系。方法 回顾性收集2019年1月—2020年12月天津中医药大学第一附属医院诊断为脑梗死的住院患者病历资料,并收集天津市市区同期气象数据。以二十四节气为节点,按照患者入院时间进行分组,分析二十四节气脑梗死入院人数分布情况,并分析二十四节气脑梗死入院高发时段与气象因素的相关性,探讨气象因素与脑梗死入院的关系。结果 本研究共纳入15 401例脑梗死病例,脑梗死入院的节气分布,差异有统计学意义(P<0.05),可分为春季入院高峰期(雨水-小满)、入院稳定期(芒种-秋分)、秋季入院高峰期(寒露-立冬)、入院递减期(小雪-立春)。单因素分析结果显示,日平均气温与气温日较差对脑梗死日入院人数有影响(P<0.05)。在调整日平均气压、日平均相对湿度后,在春季入院高峰期男性脑梗死患者日入院人数与气温日较差有关联[RR=1.023(1.003,1.044),P=0.026],在秋季入院高峰期男性脑梗死患者日入院人数与日平均气温有关联[RR=1.041(1.003,1.080),P=0.032]。结论 雨水-小满、寒露-立冬为脑梗死入院高峰,小雪-立春为脑梗死入院低谷,芒种-秋分入院人数相对均衡,气温日较差和日平均气温对脑梗死入院存在影响,在男性人群更敏感。

Abstract: Objective To explore the relationship between cerebral infarction and meteorological factors in Tianjin. Methods A total of 15 401 hospitalized cases diagnosed with cerebral infarction in the First Affiliated Hospital of Tianjin University of Chinese Medicine from January 2019 to Decmber 2020 were retrospectively collected, and collected with concurrent meteorological data in Tianjin. At the nodes of the 24 solar terms, patients were grouped according to the admission time, the distribution of cerebral infarction in the 24 solar terms was analyzed, and the correlation between the high incidence period of cerebral infarction in the 24 solar terms and meteorological factors was analyzed. To explore the relationship between meteorological changes and cerebral infarction admission. Results Spring admission peak period (Rain Water-Lesser Fullness of Grain), admission stable period (Grain in Ear-the Autumn Equinox), autumn-admission peak period (Cold Dew-the Beginning of Winter), decreasing period of admissions (Lesser Snow-the Beginning of Spring). The results of the univariate analysis showed that the poor daily average air temperature and the daily air temperature had an influence on the admission of cerebral infarction, due to the correlation among the various meteorological factors. Conclusion Rain Water-Lesser Fullness of Grain, Cold Dew-the Beginning of Winter is the peak of cerebral infarction admission, Lesser Snow-the Beginning of Spring is the trough of cerebral infarction admission, and the number of admissions in Grain in Ear-the Autumn Equinox is relatively balanced. Temperature Poor temperature and daily average temperature are the main meteorological factors of cerebral infarction admission, and the male population is more sensitive.
