颈部血管超声在颈内动脉闭塞患者侧支循环判断中的应用效果

Application Effects of Cervical Vascular Ultrasound in the Judgment of Collateral Circulation in Patients with Internal Carotid Artery Occlusion

摘要: 目的 探讨颈部血管超声在颈内动脉闭塞患者侧支循环判断中的应用效果。方法 选择2018年8月~2022年1月在汉中市南郑区人民医院诊治的72例脑卒中患者作为研究对象,所有患者都给予颈内动脉造影和颈部血管超声检查,记录影像学特征,判断侧支循环形成情况并进行诊断价值分析。结果 在72例患者中,颈内动脉造影判断为颈内动脉侧支循环形成42例,占比58.33%;侧支循环未形成30例,占比41.67%。两组的颈部血管狭窄程度超声判断结果比较,差异有统计学意义(P<0.05)。两组的颈部血管斑块性质超声判断结果比较,差异有统计学意义(P<0.05)。颈部血管超声判断为侧支循环形成为41例,颈部血管超声在颈内动脉闭塞患者侧支循环判断中的应用敏感性与特异性分别为97.62%和100.00%。ROC曲线分析显示颈部血管超声在颈内动脉闭塞患者侧支循环判断中的曲线下面积为0.773。结论 颈部血管超声在颈内动脉闭塞患者侧支循环判断中具有很高的敏感性与特异性,可有效判定颈部血管狭窄程度与斑块特征,可作为预防和诊断脑卒中的重要临床检查手段。

Abstract: Objective To explore the application effects of cervical vascular ultrasound in the judgment of collateral circulation in patients with internal carotid artery occlusion. Methods From August 2018 to January 2022, 72 cases of stroke patients who were diagnosed and treated in our hospital's neurosurgery department were selected as the research subjects. All patients were given cervical vascular ultrasound examination,imaging features were recorded,and the formation of collateral circulation were judged and given diagnostic value analysis. Results Among 72 patients, internal carotid artery angiography determined that internal carotid artery collateral circulation formed in 42 cases, accounting for 58.33%; Collateral circulation was not formed in 30 cases, accounting for 41.67%. There was statistical significance in the ultrasonic determination results of the degree of cervical vascular stenosis between the two groups(P<0.05). There was significant difference between the two groups in the ultrasonic evaluation results of the nature of cervical vascular plaques(P<0.05). A total of 41 cases were judged as collateral circulation by cervical vascular ultrasound. The sensitivity and specificity of cervical vascular ultrasound in the determination of collateral circulation in patients with internal carotid artery occlusion were 97.62% and 100.00%. ROC curve analysis showed that the area under the curve of cervical vascular ultrasound in the determination of collateral circulation in patients with internal carotid artery occlusion was 0.773. Conclusion Cervical vascular ultrasound has high sensitivity and specificity in judging collateral circulation in patients with internal carotid artery occlusion, and it can effectively determine the characteristics of cervical vascular stenosis and plaque. means of inspection.
