糖皮质激素联合来氟米特治疗IgA肾病患者的临床效果评价

摘要/Abstract

摘要： 目的研究糖皮质激素联合来氟米特治疗IgA肾病患者的临床效果。方法选取平邑县人民医院2020年1月~2021年1月收治的100例IgA肾病患者为研究对象,采用抽签法分为常规组和实验组,每组50例。常规组采用泼尼松治疗,实验组实施糖皮质激素联合来氟米特治疗。观察比较24 h尿蛋白定量、血清肌酐、收缩压、舒张压、肾小球滤过率（GFR）、尿血管细胞黏附分子-1 （VCAM-1）、Toll样受体-9蛋白（TLR-9）指标,治疗效果,临床症状各项指标等。结果治疗后,两组患者的各项指标都较治疗前有所改善,差异有统计学意义（P<0.05）,且与常规组比,实验组24 h尿蛋白定量、收缩压、舒张压、GFR、VCAM-1、TLR-9改善更明显,差异有统计学意义（P<0.05）。实验组的住院时间、症状改善时间、症状消失时间均明显比常规组短,差异有统计学意义（P<0.05）。实验组治疗有效率高于对照组,差异有统计学意义（P<0.05）。结论在治疗IgA肾病患者过程中,实施糖皮质激素联合来氟米特治疗,取得了良好的效果,有助于改善患者24 h尿蛋白,缩短治疗时间,提高治愈率,值得在临床应用。

Abstract: Objective To study the clinical effect of glucocorticoid combined with leflunomide on patients with IgA nephropathy. Methods A total of 100 patients with IgA nephropathy admitted to Pingyi County People’s Hospital were divided into 50 cases in each group by drawing lots from January 2020 to January 2021. The routine group was treated with prednisone, while the experimental group was treated with glucocorticoid combined with leflunomide. 24-hour urine protein quantity, serum myotoxicity, systolic blood pressure, diastolic blood pressure, glomerular filtration rate (GFR), urinary vascular cell adhesion molecule-1 (VCAM-1), Toll-like receptor-9 protein (TLR-9) were observed and compared; Treatment effect, clinical symptoms and other indicators were also evaluated and compared. Results After treatment, the indexes of the two groups were improved,with significant differences(P<0.05), and the 24-hour urinary protein quantity, systolic blood pressure, diastolic blood pressure, GFR, VCAM-1, TLR-9 group were significantly different in the experimental group(P<0.05). The average hospitalization time, symptom improvement time and symptom disappearance time of the experimental group were significantly shorter than those of the conventional group, and the difference was statistically significant (P<0.05). The effective rate of the experimental group was higher than that of the control group, and the difference was statistically significant (P<0.05). Conclusion Glucocorticoid combined with leflunomide in the treatment of patients with IgA nephropathy has achieved good results, which is helpful to improve the 24-hour urinary protein and plasma albumin, shorten the treatment time and improve the cure rate, which is worthy of clinical application.
